- My name is Eric McCray, Contract Specialist from the Hampton VAMC, Hampton Virginia. The requirement below is for the Johnson City Vet Center located at 2203 McKinley Rd, Suite 254, Johnson City, TN 37604. THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does not constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. Market research is being conducted to solicit Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Small Business, Small Disadvantaged Business, 8(a), Woman Owned Small Business or HUBZone interests. The Department of Veteran Affairs Medical Center (VAMC) is trying to determine whether to set aside an acquisition to provide readjustment counseling services at the Johnson City Vet Center. Contractor shall provide readjustment counseling services to eligible veterans located in rural communities where the veterans reside (greater than 50 miles from the nearest Vet Center) within the following counties Jackson, Macon, Swain, Cherokee, Clay, and Graham. The counseling may include traditional psychotherapeutic modalities, but the purpose of psychotherapy (or any modality) must be, as a primary task, to address readjustment difficulties derived from military duties and the homecoming experience NAICS Code: 621330 Office of Mental Health Practitioners (except Physicians) Contractors is applicable to this acquisition; with a size standard of $7.5 Million. If your company is interested in performing these services, please notify our office of your intent. In response, please include the following: A positive statement of your intent to submit a quote as a prime contractor (capability statement) If any teaming or subcontracting possibilities are contemplated, please provide subcontractor s name and breakdown of labor and its performance cost Evidence of experience in work similar in type and scope to include contract numbers, Project titles, award dollar amounts, points of contact and telephone numbers Past performance references with points of contact and phone numbers Identify whether the firm falls into any of the socioeconomic categories listed above Your response is required by Monday 15 July 2019 by 4pm EST. All capable Service Disabled and Veteran Small Businesses are encouraged to respond. You must be register in The System for Award Management (SAM).
