- COMBINED SYNOPSIS/SOLICITATION NOTICE 36C26219Q0975 Responses are due by 12:00 pm (PST), July 15, 2019 COMBINED SYNOPSIS/SOLICITATION NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016. Solicitation 36C26219Q0975 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued as unrestricted. NAICS Code is 334510 Electromedical and Electrotherapeutic Apparatus Manufacturing and the Small Business Administration s (SBA) size standard 1000 Employees. The Government is seeking a vendor who can provide the following items for the VA Los Angeles Healthcare System: Part Number Description Qty COREMB120 COREr Mobile Precision Guided Therapy System Set of Accessories: CORE Mobile system with trackball controller, Phased Array PIM, Medical grade printer, 19" monitor, DICOM Network connection, Blu-ray/DVD drive, Operator's Manual and seven (7) year Software Support Agreement ("SSA") which provides for unspecified Updates to the CORE software releases. 1 EA End User License Agreement 1 EA FFR02 COREr Mobile FFR Included Includes PIMffr and PIMffr Cable 1 EA CPAD01 COREr Control Pad Included Bedside touchscreen controller offering system control from the sterile field 1 EA PRN01 COREr Printer 1 EA Installation Cost One (1)-Year Warranty Three (3) Year COREr Mobile Extended Service Coverage Included Extended Service to commence upon expiration of one (1) Year Warranty. 1 EA This solicitation is for brand name (exact match) only and offerors shall be an authorized distributor by the manufacture and must be able to offer full manufacture warranty all at a fair and reasonable price offering best value to the Government.
