Responders are solely responsible for all expenses associated with responding to this sources sought. Sources Sought DENTAL LAB SERVICES The Department of Veterans Affairs in Altoona, Pennsylvania requires a contract for dental services for patients at the Altoona VA Medical Center (AltVAMC), which is located at 2907 Pleasant Valley Blvd. Altoona, PA 16602. The Contractor shall provide all labor, supervision and all other resources required to deliver dental lab services to include work by qualified dental lab personnel, materials, repairs, and finished products, for and pertaining to the items listed in the schedule, in accordance with the contract documents. The contracted dental lab will fulfill the standards set by our current responsiveness to the Veterans of the Altoona VAMC population as determined by current standards of customer satisfaction or established wait times for dental products. The contractor must be capable of meeting and maintaining the Turn-Around Times (TAT) stated in the solicitation. The Altoona VAMC intends to award a five year contract for these services. General Requirement The Department of Veterans Affairs Medical Center, Dental Laboratory, Altoona, PA seeks to contract with a dental laboratory to provide all steps in the fabrication of fixed and removable prosthetics in support of our veteran population. This contract will be for a base period for one year from date of award, plus four (4) option years. B. Specific Requirements The Contractor shall use procedures, techniques and current technology that are considered to be the standards in the industry. The Contractor shall provide items that meet the esthetic and functional requirements of the patient. The Contractor shall be familiar with mold guides by Trubyte Bioblend and with shade guides by Trubyte Bioform and the VITA 3D Tooth guide. High quality materials shall be utilized from ISO 9001/CE registered companies. Laboratory shall perform internal technical audits to ensure consistent, high quality, and timely restorations. The Contractor shall provide the following dental laboratory services to the VA Medical Center, Altoona, Pennsylvania: Preparation of Models (pouring impressions, trimming, study, final). Mounting models from provided impressions and articulation of cases. Fabrication of custom trays (rigid with handles). Fabrication of base plates with wax occlusal rims for full and partial dentures. Set up of denture teeth for full and partial dentures (including one reset as needed). Processing and finishing in acrylic of full and partial dentures (including placement of ID tag). Repair of full and partial dentures (reline, rebase, replace tooth, addition of tooth or clasp). Reline options offered including PermaSoft or soft reline equivalent. Design, construction, and finishing of metal frameworks for cast partial dentures. Fabrication of acrylic partial dentures with wrought wire clasps. Construction of diagnostic wax-up models. Fabrication of interim maxillary dentures. Fabrication of dental implant supported full and partial dentures (including locator attachments). Provision of blank prescription forms to be completed by VA dentist. On-site consultation of cases as needed, contractor will not have access to patient information. Fabrication of orthotic appliances and soft occlusal guards. Fabrication of surgical stents for implant placement (templates). Fabrication of porcelain fused to metal crown/bridge, full contour Zirconia crown/bridge, EMax ceramic crown/bridge, and surveyed crowns. Construction of Maryland bridge appliances. Fabrication of Fixed Partial Denture Bridge (multiple units restorations). Fabrication of Implant abutments, custom abutments, and implant crowns (screw and cement retained). In addition to the listed services, the following requirements are to be provided by the Contractor for such services: The Contractor must identify the specific type of metal used in each prosthesis at the time of delivery. The Contractor shall provide metal alloy manufactured in the United States of America to fabricate all required services. The contract laboratory shall identify by affixing the most recent manufacture label to provide a description of the metal alloy components and percentages used in the fabrication of each of the requested dental prostheses. The Contractor shall provide a thorough infection control process for all incoming and outgoing cases. All completed cases must be disinfected before the product is returned to the VAMC Altoona. The Contractor shall provide daily personalized pickup and delivery service to ensure that casework arrives on time and safely and no additional cost to the VAMC Altoona. The Contractor shall provide local reline service with same day turnaround of all dentures (relines and rebases) as needed. The Contractor shall provide same day repair service of all dentures and partials as needed. The Contractor shall provide on-site, same day professional case consultation at no additional cost to the Altoona VAMC. The Contractor shall provide same day all ceramic crown service as needed. The Contractor s Officer Representative (COR) and the Contracting Officer (CO) shall be notified if the contracted laboratory intends to subcontract any work under this contract to another dental laboratory. The subcontractor must be located in the United States of America and the Contracting Officer must provide approval to Contractor to subcontract services. The attached Quality Assessment of Dental Laboratory Procedures will be used to measure/assess the quality of the product produced. Fabrication Materials 1. The awarded contractor will use Base Metal Alloys to fabricate and complete all required prostheses. The Base Metal Alloy for Frameworks must be Co 62.7%, Cr 28.5%, Mo 6.0%, Mn,1.0%, Si<1.0%, C,1.0%, Ni<1.0%. The Base Metal Alloy used for crown and bridge must be Ni 72%, Cr 13%, Molybdenum 5%, Ti 4.2%, Al 2%, Intermetallic Beryllide<1.8%. The contractor will provide Alloy Labels issued by the vendor to identify the content elements used for each case. 2. The contractor shall use porcelain compatible with the Base Metal Alloy to ensure superior quality and esthetics. 3. The contractor shall use teeth, acrylic and waxes approved for use by ADA in dental appliances. Fabrication Requirements The awarded contractor (dental laboratory) will perform the following services: CROWN AND BRIDGE All procedures from pouring preliminary impressions to the final restoration for all porcelain fused to metal, cast post and core for single unit, veneers, all porcelain and multiple units restorations, including all ceramic crowns and bridges. The price per Crown and/or unit of Fixed Partial Denture bridgework may include metal rest seats, porcelain butt margins, metal occlusal surfaces, metal guide planes, metal collars, precision attachment, ERA attachments with guide planes, contour undercuts (in metal and porcelain) for Removable Partial Denture clasping or retrofitting to an existing Removable Partial Denture as requested. Repairs (soldering, porcelain addition) to existing crowns and/or fixed partial denture frameworks, or existing metal frameworks will also be provided as needed. Dental implant supported Crowns and Fixed Partial Dentures will be fabricated and provided to include custom angled abutments for cemented and screw retained crowns and fixed partial dentures, implant retained bars, substructures for implant retained complete and partial dentures. The selected contractor may be required to pour impressions and articulate casts as needed (as for model work on conventional crowns and fixed partial denture restorations). C. Service Turn-around Time Completed work shall be returned through daily deliveries, Monday through Friday, excluding Federal Holidays, to the Altoona VAMC located at 2907 Pleasant Valley Boulevard. Pick-ups and deliveries must be accomplished between the hours of 8:00 a.m. 4:00 p.m. The Contractor shall adhere to the following turn-around requirements: All complete and partial denture steps shall be completed within five (5) working days of pickup. All work must meet the required Turn-Around Times. Five (5) Working Days: Complete and Removable Partial Denture, processed acrylic resin Tooth Arrangements, Complete and Removable Partial Dentures Custom Occlusal Guards 4. Diagnostic wax-up, five teeth or more Three (3) Working Days: Diagnostic wax-up, four teeth or less Tooth Rearrangements, Complete and Removable Partial Dentures Forty-eight hours: Complete and Removable Partial Denture, record bases and occlusion rims Complete and Removable Partial Denture, record bases and occlusion rims, stabilized/processed Transitional Removable Partial Denture, processed Twenty-four hours: Complete and Removable Partial Denture acrylic resin repairs Complete and Removable Partial Denture, tooth additions/repairs Removable Partial Denture, clasp repair/replacement and wrought wire clasp retained by acrylic resin Custom impression trays Custom impression trays, altered case impression Poured impressions Articulation of cast Cast duplications Fluoride application tray Complete and Removable Partial Denture, relines Complete and Removable Partial Denture, rebase Flawed work returned. Altoona VAMC Responsibilities VA Dentists will provide legible dental service prescriptions and accurate impressions and bite registrations. If a prescription is not legible or clearly understood, the Contractor shall contact VA s Contracting Officer s Representative (COR) to obtain a clear and legible prescription. E. Corrective Actions 1. Contractor: a. The Contractor shall, at no additional expense to VA, re-make any dental prosthesis that is ill-fitting or structurally flawed due to Contractor error. For example: excessive porosity in acrylic, distorted framework or inadequate adherence of denture teeth to acrylic. If prosthesis failure is due to Contractor s failure to adhere to prescription, remake is also done at Contractor s expense. If Altoona VAMC s dentist provides inadequate or incomplete instructions, VA will bear the cost of remake. b. If there is structural failure of the acrylic base, the partial framework (including clasps), or more than a single incidence of teeth de-bonding from acrylic base, the repair will be the responsibility of the Contractor for a period of 12 months. B.4 SPECIAL CONTRACT REQUIREMENTS A. QUALITY CONTROL The Contractor shall develop a Quality Assurance Plan (QAP) which will provide the Contractor s strategy for monitoring performance internally, providing performance information to the Government, and reporting in compliance with the PWS. Within the QAP, the Contractor shall propose and maintain an established reporting system to promptly notify VA of any problems, issues, risks, or concerns with workflow performance, programmatic improvement, security, and schedule adherence. Quality control is the exclusive responsibility of the Contractor. The work performed under this contract shall be subject to evaluation by the COR/CO via the Quality Assurance Surveillance Plan (QASP) and the Quality Assessment of Dental Laboratory Procedures located in Section D. The final plan shall be submitted to the CO and COR within 30 days of contract award. B. PERSONNEL The Contractor must furnish an up-to-date list of all Certified Dental Laboratory Technicians that are assigned laboratory work from VAMC Altoona. The list must be updated as needed and/or certified as accurate and complete every six months. (1) The Contractor must comply with all FDA Regulations, ANSI Standards and Buy American Act policies in fabrication of all dental prostheses. All materials used for VA cases must be fabricated and assembled in the United States. E. CONTRACT ADMINISTRATION a. Notwithstanding the Contractor's responsibility for total management during the performance of this contract, the administration of the contract will require maximum coordination between the Government and the Contractor. b. The Contracting Officer is the only person authorized to approve changes or modify any of the requirements of this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes that shall affect price, quantity or quality of performance of this contract. c. The COR shall be responsible for the overall technical administration of this contract as outlined in the COR Delegation of Authority. QUALITY ASSESSMENT OF DENTAL LABORATORY PROCEDURES ______________________________ I. Complete Dentures A. Casts, Diagnostic and Master 1. Base is 10mm thick at the thinnest portion of the cast 2. 4mm wide land area throughout the circumference of the cast 3. Sulcular depth 3mm on buccal, to follow the contour of the buccal vestibule 4. Sulcular depth 3mm on lingual to follow the contour of the vestibule 5. Free of bubbles and defects 6. All anatomic areas of impression included on cast 7. Tongue area, flat across the arch 8. Base is parallel to the plane of occlusion or residual ridge B. Impression Trays 1. Rigid and fabricated from the appropriate material 2. Covers all indicated anatomy 3. Handles do not interfere with border molding movements, formed as prescribed, so as not to displace facial musculature 4. Impression material retentive features included in the tray as prescribed C. Record Base 1. Is fabricated from appropriate material and well adapted to the residual ridge 2. Covers all indicated anatomy 3. Is extended to the land area 4. Is adequately thick for rigidity 5. Is smooth and free of sharp area D. Occlusion Rim, Maxillary 1. Is fabricated from the prescribed base plate wax 2. Is 10mm wide in the posterior section 3. The posterior sections are placed over the crest of the residual ridges 4. Is straight from the buccal aspect of the record base border to the buccal aspect of the occlusion rim 5. Taper in width to 3mm in the anterior section 6. Anterior section angles 5 degrees anteriorly from the horizontal 7. Anterior is 22mm long from the extent of the labial flange 8. Posterior is 18mm long from the extent of the buccal flange 9. All wax is smooth, without voids 10. Adheres well to the record base 11. The occlusal plane is flat, generally parallel to the residual ridge E. Occlusion Rim, Mandibular 1. Is fabricated from the prescribed baseplate wax 2. Is 10mm wide in the posterior section 3. The posterior sections are placed over the crest of the residual ridges 4. The posterior sections stop at the beginning of the upward incline of the residual ridges 5. Is straight from the buccal aspect of the record base border to the buccal aspect of the occlusion rim 6. Tapers in width to 3mm in the anterior section 7. Anterior section angles 5 degrees anteriorly from the horizontal 8. Anterior is 18mm long from the extent of the labial flange 9. Posterior extends to the top of the retro-molar pad 10. All wax is smooth, without voids 11. Adheres well to the record base 12. The occlusal plane is flat, ideally parallel to residual ridge F. Wax Trial Dentures, Maxillary 1. The midline of the arranged teeth matches the marked midline of the occlusion rims 2. The facial surfaces of the anterior teeth are placed in the same plane as the contoured facial surface of the occlusion rims 3. The incisal edges of the anterior teeth are placed in the same plane as the incisal surface of the occlusion rim 4. The inter-dental wax fills the embrasures up to the contact points 5. Adequate inter-dental wax fills the embrasures to allow for finishing of the acrylic resin 6. A 4mm band of simulated attached gingiva is present, elevated to simulate buccal bone 7. The palatal area is 2mm thick 8. A 1mm band of wax is palatal to the posterior teeth to allow for finishing of the acrylic resin 9. The midline of the arranged teeth matches the marked midline of the occlusion rims 2. The facial surfaces of the anterior teeth are placed in the same plane as the contoured facial surface of the occlusion rims 3. The incisal edges of the anterior teeth are placed to develop the horizontal and vertical overlap necessary for the individual clinical requirements 4. The occlusal plane of the mandibular posterior teeth is at the appropriately indicated height of the retro-molar pads (1/2 2/3 up the RMP 2mm inferior to the highest portion of the RMP) 5. The central groove line of the mandibular posterior teeth is placed over the crest of the residual ridge 6. The number of posterior teeth is in harmony with space available from the distal of canine to the beginning of the ascending ramus 7. The inter-dental wax fills the embrasures up to the contact points 8. Adequate inter-dental wax fills the embrasures to allow for finishing of the acrylic resin 9.
