- COMBINED SYNOPSIS SOLICITATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 36C10E19Q0233 The solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2019-02 (Effective 05/06/19 & 06/05/19). This solicitation is set-aside exclusively for Service-Disabled Veteran-Owned Small Business concerns. To be considered for award prospective offerors must be a verified SDVOB concern in the VA Center for Verification and Evaluation (CVE) Veteran Information Pages (VIP) pages at the date and time set for receipt of offers. An Offeror who is not a verified SDVOB concern at the date and time set for receipt of offers will have their offer rejected as non-responsive and will not be considered for award. The applicable NAIC Code for this solicitation is 541380, Testing Laboratory. Drinking Water Quality Test and Analysis ITEM NO. DESCRIPTION QTY UNIT UNIT PRICE AMOUNT 0001 The contractor shall provide all supplies, services, equipment, tools, labor, facilities, transportation, and supervision to test the drinking water and perform analysis for the Cleveland Veterans Affairs Regional Office in accordance with the attached Statement of Work. Period of Performance: 14 calendar days from issuance of a notice to proceed. 1 JB $___________ $___________ Description of Requirement REQUEST FOR QUOTE FOR WATER QUALITY TESTING AND ANALYSIS CLEVELAND VBA REGIONAL OFFICE 1.0 INTRODUCTION This Request for Quote (RFQ) is to provide sampling and analysis of the drinking water at the VA Regional Office (VARO) located at 1240 E. 9th Street, Cleveland, Ohio, and to recommend corrective action based on the analyses. Sampling will be taken from the AJC Federal Building Water Fountains specified herein. 2.0 SCOPE OF WORK The project will evaluate the quality of drinking water utilized by the VA Regional Office, Cleveland, Ohio. The Contractor will provide a firm-fixed price for performing the work described within this document. Sampling and screening analysis of potable water from drinking fountains (bubblers) located on floors 9, 10, 11, 12, and 13 of the AJC Federal Building. The evaluation of drinking water will be conducted in the following 3 Tasks: 2.1 Task One: Post-award Kick off meeting. The Contractor will participate in one telephonic kick-off meeting with appropriate individuals as determined by the Cleveland VA Regional Office facility coordinator (FC) and Contracting Officer. At the kick-off meeting, (expected to last 1 hour or less), the Contractor and applicable Cleveland VARO staff will review the list of tasks to be accomplished; exchange contact information; make arrangement for sampling the water; and briefly review the performance requirements and collection procedures stipulated herein. Access to the Facility and arrangements for VA personnel to escort the Contractor throughout the floors during collection times will also be addressed. Finally, any concerns and/or assumptions by the Contractor will be promptly addressed. Upon conclusion, the Contractor will have a clear understanding of what the VARO expects of the Contractor, and visa versa, what the Contractor can expect of the Agency. The Contractor will be responsible for recording the minutes of the meeting. The contractor may be expected to attend other meetings as deemed necessary for the completion of this project by the VARO. These meetings will be in the form of conference calls. 2.1.1 Acceptable Quality Limits: 100% Compliance as verified by Contracting Officer Representative (COR). Contractor will attend all post-award meetings via telephone. 2.1.2 Deliverables: Contractor will provide a copy of the post-award kick-off minutes to the Contracting Officer, Facility Coordinator, and COR. 2.2 Task Two: Site Visits and Water Analysis An Environmental Technician will collect samples of potable water from a total of twenty (20) drinking fountains (bubbler-type) located throughout Floors 9, 10, 11, 12 and 13 of the AJC Federal Building. The Contractor will contact the facility coordinator (FC) for the Cleveland VARO to make appropriate arrangements for the collection of samples. If the Contractor encounters problems contacting the FC or making appropriate arrangements, they shall contact the Chief, Support Services for assistance. A master schedule will be given to the FC and Contracting Officer; the FC will be notified regarding any deviation from the agreed upon schedule. The Contractor will visit the facility, collect the required samples, and perform the appropriate off-site analysis. All samples will be labeled, properly preserved, and submitted to a certified analytical laboratory for processing. The contractor shall be familiar with U. S. Environmental Protection Agency (EPA) established legal limits (National Primary Drinking Water Regulations) in drinking water. Sampling Procedures: Static First Draw Samples: First draw samples will be taken in the morning, before the fountains have been used. Post Flush Samples: After first draw sample is taken, the fountain will be allowed to run to flush water from the fountain and bring water in from the distribution system. A post flush sample will then be taken. The contractor shall collect an adequate number of Quality Assurance/Quality Control (QA/QC) samples. Labeling - All samples will be labeled, properly preserved, and submitted to a certified analytical laboratory to be screened for the target analytes as outlined in the parameters for testing. Testing and Analysis - Required for each sample in accordance with U. S. EPA regulatory limits and standards for Drinking Water. Parameters for testing - Request for testing of specific contaminants that can be potentially impacted by the building's water distribution system include the following: Lead Copper pH turbidity total coliform bacteria and fecal coliform bacteria 2.2.1 Acceptable Quality Limits: 100% compliance as verified by Contracting Officer Representative (COR). 2.2.2 Deliverables: Contractor shall provide a list of all Environmental Technicians who will perform on-site collection of samples and provide the Contracting Officer and COR with a copy of the Environmental Technicians certification. 2.3 Task Three: Reporting Requirements Corrective Action Plan Summary The Contractor shall prepare a Corrective Action Plan summary report. The report shall include analytical results, chain-of custody documentation, analytical results summary tables, sampling locations, copies of field notes, conclusions, and recommendations for further action based on regulatory standards. The Contractor will attend a telephonic meeting to present results of the project. Reports are generally completed seven to ten days after all field and laboratory work is completed. 2.3.1 Acceptable Quality Limits: 100% compliance as verified by the COR. 2.3.2 Deliverables: (1) The Corrective Action Plan Summary Report shall be submitted and shall include a Table of Contents, field notes, and all Tables, Figures, and Appendices. (2) The Corrective Action Plan Summary Report shall be typed single-spaced on both sides of the paper. The size of pages shall be 8.5 by 11 inches. Front covers shall contain the title of the document, the installation name, date, city and state. Text shall be clear, concise, address only issues relevant to the project and grammatically correct in a manner suitable for reading by persons not professionally trained for the technical subject discussed. 1. Provide an Adobe Acrobat PDF file, and Microsoft Word of the complete final report. Any bound submitted documents must be represented electronically in the Adobe Acrobat file for each accompanying report. 2. Analytical Results and Laboratory QA/QC data Provide any Analytical or Laboratory Results in this folder and all QA/QC data. This information must be provided as originally submitted by the analytical laboratory the Contractor shall not modify any of this information but ensure it is valid. 3.0 CONTRACTOR REQUIREMENTS 3.1 General Requirements (1) Government Property - All documents, maps, photographs, graphics, mailing lists, computer files and the like developed by the Contractor in completing the requirements of this SOW are Government property and will be delivered to the Cleveland VA Regional Office upon completion of this project. (2) Data Security - The Contractor shall not release any data, reports, or materials collected or developed while completing this project without the expressed written consent of the Contracting Officer or his/her representative. (3) Site visits No more than two Contractor personnel shall attend facility site visit. All contract personnel shall produce a government picture ID before being allowed entry to the installation. (4) Notification - The Contractor shall verify the date of the visit in advance with the facility coordinator (FC) and shall submit a notification letter or email to the FC no later than five (5) days prior to the site visit. A copy of the notification shall also be provided to the Contracting Officer. At a minimum, the notification letter shall include a) purpose of visit, b) date and approximate duration of the scheduled visit, c) names and telephone numbers of the individuals conducting the visit, and d) a request that the facility have personnel at the in-briefing who are knowledgeable as to the locations of the water quality sampling points and can allow access if needed. (5) Briefings The Contractor shall conduct an informal in-briefing with the FC upon arriving at the facility and perform the following: a) Introduce Contractor personnel conducting the visit, b) review the purpose of the visit, c) the Contractor shall arrange for any needed access. Contractor shall conduct an informal out-briefing upon completion of the site visit. 3.2 Deliverables (1) The Contractor shall submit a copy of the kick off meeting minutes to the Contracting Officer and COR within 5 days of the kick off meeting. 4.0 SCHEDULE Based on the time requirements outlined in the SOW, the following schedule has been established for completion of this project. Description Milestone Date Notice to Proceed (NTP) - To Be Determined Kick Off Meeting Within 2 days of NTP Kick Off Meeting Minutes submitted Within 3 days of NTP Site Visits completed 7 days from NTP Final reports submittal 14 days from NTP. If the Contractor requires more time than allowed by the schedule, the Contractor must discuss this with the COR and explain why more time is needed. Provided there is a legitimate reason for the extension, the schedule will be adjusted by the Contracting Officer. 5.0 ADDITIONAL INFORMATION/REQUIREMENTS 5.1. POINTS OF CONTACT (POC). The following Points of Contact will be provided by the following agencies. 5.1.1. GOVERNMENT CONTRACTING AGENCY The government contracting agency, will assign a Contracting Officer who will represent the VA Regional Office (VARO) on contractual matters relating to this Scope of Work. The CO will serve as the POC with the Contractor and the VARO for resolution of deviations, changes, inadequacies, and questions related to the Scope of Work and other contractual issues. In addition, a COR for this project will be assigned and all contact information will be provided at the post-award kick-off meeting. All invoices will be submitted to the COR electronically at the email address provided. Additionally, all invoices sent via email will also be sent to the Contracting Officer. Invoices shall be submitted electronically via email only and shall include the Contractor s name, address, Tax-ID, DUNS #, Contract Number and Obligation Number. 5.1.2. CONTRACTOR The contractor shall assign a Project Manager who will serve as the POC with the COR on contractual matters relating to the completion of this Scope of Work. 5.2 SECURITY The General Contractor is responsible for assuring that all subcontractors working on the project and their employees also comply with the standards outlined here. All general Contractor s employees, subcontractors, and material suppliers entering the VBA property consent to search by VA Police at any time. The areas of search shall be all encompassing including but not limited to: their person; their personal effects; as well as all Contractor vehicles, tool boxes and all containers. Weapons are not permitted on VA property. If work requires the Contractor or its employees to have entry into the Regional Office, they must be escorted at all times by the COR or a designated Government representative. No photography of VA premises is allowed without written permission of the Contracting Officer. Period of Performance: 14 Days from issuance of the notice to proceed.
