- Elevator Inspection Services for the Salt Lake City and Cheyenne VAMC s Scope of Work: The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, certifications, and other services to provide elevator inspection services. The contractor shall perform to the standards in accordance with this PWS. Place of Performance-VA Medical Centers: 660- 500 Foothill Blvd. Salt Lake City, UT 84148-000 442- 2360 East Pershing Blvd Cheyenne, WY 82001 Period of Performance: Base period + 4(1)-year options. Operation Constraints: Medical Center Operations M-F 8:30-4:30 PM National Holidays Observed: New Year s Day, Martin Luther King s Birthday, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. Requirements: Provide elevator Inspection and other designated equipment in accordance with the ASME-A17.1 and the NFPA Life Safety and Fire Code, and any applicable State codes. GFE: There will be no Government Furnished Equipment Deliverables: A comprehensive report will be provided for each elevator inspection. Reports shall be provided within 45 calendar days of inspections.
