- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number for this requirement is 75D301¬-19-Q-70699 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control & Prevention (CDC) intends to award a fixed-price purchase order for this requirement. All work shall be performed according to the attached Statement of Work (SOW). The quote format is at the discretion of the Offeror. Quote shall be clear, concise, and shall include sufficient detail based on the solicitation requirement. Quotes are required to meet solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub-factors. It is the Offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website https://www.acquisition.gov/. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-87-2 (6 April 2016). This acquisition is a total small business set aside under North American Industry Classification System (NAICS) code 423710, Hardware Merchant Wholesalers and small business size standard of 150 employees. A fixed-price purchase order will be issued in writing to the successful Offeror. To be eligible to receive an award resulting from this solicitation, the Contractor must be registered in the System for Award Management (SAM). Offerors may register in SAM at http://www.sam.gov. For questions regarding registration, contact Federal Service Desk at (866) 606-8220. Offerors shall include FAR 52.212-3, Offeror Representations and Certifications with Offer or indicate if it is available online at http://www.sam.gov. No telephonic quotes will be processed. All electronic responses must be received by the Government no later than 10:00am ET on Tuesday, 1 August 2019. An electronic quote shall be emailed to the Contracting Officer at the email address in this notice. All questions must be received no later than 10:00am ET on Tuesday, 23 July 2019.
