This project is for replacement of sections of existing ductwork serving Radiology Dept. at VA Medical Center Northport NY NAICS Code: 238220 Plumbing, Heating, and Air Conditioning Contractors Size Standard: $15 Million Due Date: Proposals are due on Monday August 5, 2019 11:59 PM ET. Site Visit A site visit will be conducted on Monday July 29, 2019 11:00 AM ET. Statement of Work Radiology Filtration Upgrade Project Number: 632-19-115 Objectives This project is to furnish all labor and materials to replace sections of existing ductwork serving the Radiology Suite, VA Medical Center Northport NY, Radiology MER, Basement Bldg. 200N. Work includes, but is not limited to the following: All work will be coordinated with the COR ahead of time. All work to occur during one weekend. Project Duration to be two weeks with the actual work and downtime limited to a consecutive Saturday-Sunday. Unit must be in operation during weekdays. This Other Than Normal Working Hours/Off Hours work must be requested on a VA supplied form (via email) at minimum 72 hours in advance. Demolition, removal from site and disposal of portion of existing outside air ductwork and insulation Compliance with Infection Control requirements: Seal ductwork opened to the Radiology suite at the OA connection to the mixed air plenum while the work is going on. Place sticky mats at the Mechanical Room door inside of the mechanical room. Other requirements as indicated on the infection control permit. Provide and install new Carbon Filters and MERV 9 Pre-Filters (including two complete sets of spare filters of each) as per Attached Filter Schedule. Provide and install new galvanized ductwork, filter housing and insulation. Approximate sizes as follows Approximately 6 feet transition from 60 x 26 duct to the filter housing and then again from the filter housing to the 60 x 26 duct. Provide Magnehelic Differential Pressure Gauges with all necessary accessories for mounting on filter housing. Provide an access door on the transition ductwork to access existing damper to remain. Adjust existing damper location and control wiring as required to fit the new ductwork as necessary. Minimum duct gauge to be 20 gauge. Contractor to verify all duct sizes and lengths in the field. Filter housing shall be as per attached specifications. Provide two (2) complete sets of each type of filter, one of which will be turned over to the VA as a spare set. This applies to each filter bank, so 3 carbon filters and 3 prefilters will be given to the COR as the spare filters. 1 Year Warranty on parts & labor. Note that this contract does not involve VA sensitive information/data. Performance Period: All Submittals must be approved by VA prior to beginning work. Work will begin after issuance of Notice to Proceed. Project completion time after all submittals have been approved is 10 days. Contractor has 20 calendar days to submit all products to be used during the project. The total project duration from NTP will be 60 days allowing for material lead time. Place of Performance: U.S. Department of Veterans Affairs VA Medical Center Northport NY 79 Middleville Rd., Building 200, Radiology MER, Basement Bldg. 200N Northport, NY 11768 The proposal shall be in writing, original and one 1 copy, an itemized breakdown (i.e. number of hours, trades, task and materials) and each of the trades and areas listed as separate line items, with percentage of sub-contractor work. Please refer to the attached drawing Radiology Filtration Upgrade Mechanical Room Elevations and Specification sections 01 00 00 General Requirements, 01 35 26 Safety Requirements, 23 31 00 HVAC Ducts and Casings, and 23 40 00 HVAC Air Cleaning Devices. Request for interruption of any services may be approved by the COR, however contractor shall first make a request in writing (email, letter, etc.) requesting any interruption of any service two weeks in advance. Contractor shall be responsible to plan and coordinate all work with the COR who will coordinate and provide access. Contractor is responsible for safety, record keeping, security monitoring, and inspection of area. Maintain Certified Payroll and Daily Logs. Contractor shall provide the COR with a list of contractor staff participating in the work and shall submit in writing an ID request with company name, address, telephone number, name of each individual contractor employee and position title, SS#, home address, copy of OSHA; see VA Specification for ID request. Prior to commencing work, general contractor shall provide proof a OSHA CERTIFIED "competent person" with a 30-hour safety certificate (CP) (29 CFR 1926.20(b) (2) will maintain a presence at the work site whenever the general or sub-contractors are presence and all other employees shall have a 10-hour safety certificate issued by OSHA or other relevant competency training, as determine by the DVA. A Certificate of Insurance (COI) shall be provided upon request. Inspection/Acceptance VA CO and COR will conduct final inspection and acceptance of contractor work. Payment - Contractor shall submit 1 invoice at completion of work. All work shall be performed as described and as specified. Any other work not listed above but otherwise required as per Specification are included as the Statement of Work.
