Description
Department of Veterans Affairs, Network Contracting Office 2, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide a comprehensive vehicle for Mini-Vidas. SERVICES REQUIRED Contractor is to provide preventative maintenance at the East Orange Campus of NJ Healthcare System during the term of Aug 08, 2019 through Aug 7, 2020 with the provision of 4 option years. INTERESTED & CAPABLE RESPONSES: NCO 2 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services. Please respond with your: Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address), GSA Contract number if applicable, and DUNS number. Capability statement must provide detailed and specific information that would demonstrate a capability to perform the tasks as stated in the attached Statement of Work. The results of this notice will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice. STATEMENT OF WORK PREVENTATIVE MAINTENANCE OF MINI VIDAS ANALYZER Background: Laboratory Service, Microbiology Department at VA New Jersey Health Care System, East Orange Campus is requesting preventative maintenance on government owned multiparametric immunoanalyzer Mini Vidas. The Mini Vidas is a compact automated immunoassay system based on the Enzyme Linked Fluorescent Assay (EFLA) principles which provides accurate on-demand test results; up to 36 tests per hour. Equipment: Model # Mini Vidas Serial # 1011555 Scope or Tasks (for services): The contractor shall provide all necessary labor, equipment, tools, materials, parts and calibration necessary to perform annual preventative maintenance on the Mini Vidas immunoanalyzer as required by the NJHCS. It is the intention that the service will be performed in accordance with manufacture guidelines, and specifications and will be done in accordance with industry standard practices. The contractor shall clean the Solid Phase Receptacle (SPR) block, strip preparation trays, plastic trays, reagent trays, optical lenses and bar code reader. The Contractor shall also test the display, keypad, printer and audio output as well as calibrate the optical lenses and pipetting system. Documentation: o The service representative must report to Biomedical to sign in upon arrival and sign out upon leaving. o The service representative is required to leave a service report at the time work is completed prior to leaving the premises with any available Biomedical Shop personnel obtaining a signature. o The service report shall contain at least the following information: § Work performed § Parts used § Time spent for labor and travel o If the work is not completed prior to leaving the premises, a service report will be left, as per the above procedures, indicating the equipment status. o No payments will be made if the above procedures are not in compliance. - Special Requirement: o The contractor must supply themselves with their own maintenance/repair manuals along with any other software required to properly perform preventative maintenance of the equipment in question. o Service representatives are required to supply the NJHCS with Material Safety Data Sheets (MSDS) on any chemicals that are brought into the NJHCS prior to usage. The MSDS are to be given to the Safety Officer. A set of MSDS s must also remain at the site where the chemical is used. o Vendor must comply with usage of Personnel Protective Gear as defined in 29CFR 19.10. o Vendor must comply with all VA policies and regulations. Work Outside of Scope: o Any work to be performed that lies outside the scope of this requirement and is not covered under the preventative maintenance, must be pre-approved by the contract specialist/officer. This approval must be in the form of a modification to the contract or a separate purchase order. Failure to obtain prior authorization will result in Non-Payment of charges incurred, as they will be considered inclusive to the maintenance. Hours of Coverage: o All work performed will be during the hours of 7:30AM to 5:00PM, Monday through Friday excluding weekend and federal holidays. Warranty: Parts replaced under the preventative maintenance shall be covered under a minimum 1-year warranty.
