This is a combined synopsis/solicitation Request for Quotation (RFQ) for commercial items prepared in accordance with the format in subpart   12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number 36C24719Q0811 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal acquisition Circular 2019-01 (01-22-2019). The acquisition procedures at FAR Part 13 are being utilized. Provisions and clauses incorporated by reference or attached have the same force and effect as if they were given in full text. The full text of the Federal Acquisitions Regulations (FAR) can be accessed on the internet at http://www.acquisition.gov/far and full text of the Veterans Affairs Acquisition Regulations (VAAR) can be accessed at http://www.va.gov/oal/library/vaar/. The combined synopsis/solicitation, henceforth referred to as solicitation, is 100% set-aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB). The applicable NAICS code is 238330, Flooring Contractors and the Small Business size standard is $15.0M. All Offerors must be verified through the Vendor Information Pages (VIP) as an SDVOSB to be considered for award. Offerors not registered at https://www.vip.vetbiz.gov/ will be considered non-responsive and ineligible for further consideration of award. Offerors must also be registered as an SDVOSB under the applicable NAICS, with no active exclusions in the System for Award Management (SAM) at http://www.sam.gov. The Department of Veterans Affairs intends to award a Firm-Fixed Price contract for the removal of existing flooring and installation of new flooring in various areas within the Carl Vinson VA Medical Center in accordance with all specifications described in the attached statement of work. PLACE OF PERFORMANCE Carl Vinson VA Medical Center 1826 Veterans Blvd Dublin GA 31021 (Multiple Areas) STATEMENT OF WORK (See Attachment A for Statement of Work and Attachment E for the Diagram of Areas Requiring Service) BASIS OF AWARD The Government will award a firm fixed price contract to the responsible offeror whose quote, conforming to the solicitation, represents the lowest-price technically acceptable (LPTA) offer. The following factors shall be used to evaluate offers: Technical Capability, Past Performance, and Price. Nonprice factors will be rated on an Acceptable/Unacceptable basis. Failure to meet any of nonprice factors will result in a Unacceptable rating and eliminate the offeror from further consideration for contract award. To be considered technically acceptable, each offeror must show: Capability statement which demonstrates that the company has the expertise, experience, and resources to perform the services as described in the statement of work. Three (3) past performance references for whom the company provided the same or comparable services of installing commercial flooring as detailed in the Statement of Work. References must include the contact s name, business name, email address, phone number, a description of the services performed, and the federal contract number (if applicable). Prices will be evaluated for fair and reasonableness. All pricing should be considered FOB Destination and must be good for 180 calendar days after close of solicitation. All quotes shall be submitted on Attachment B - Price Cost Schedule. Offers must complete the Price Cost Schedule by inserting the offerors proposed contract line items, description, units, quantities, and totals in the appropriate spaces. The Government will evaluate offers for award purposes by adding the total of all line item prices to ensure that pricing is complete, and an award will be made to the responsible offer whose price is the LPTA. SITE VISIT Though not mandatory, contractors are encouraged to attend the site visit as scheduled below: Date: August 1, 2019 Time: 10:00 AM EDT Location: Carl Vinson VA Medical Center 1826 Veterans Blvd, Bldg 29 Room 110-29 (Engineering Conference Room) Dublin, GA 31021-3620 Offerors wishing to attend the site visit are asked to confirm their attendance by emailing to Lasonja.harvey@va.gov. This will be the only site-visit and additional arrangements will not be made for prospective offers to survey the work areas. ADDITIONAL INSTRUCTIONS QUESTIONS. This announcement constitutes the only solicitation and interested parties are solely responsible for monitoring this solicitation and any amendments thereto. Any questions pertaining to this solicitation, including the site visit, MUST be emailed to Lasonja.harvey@va.gov no later than August 5, 2019, 12:00 PM EDT. No questions will be accepted after this deadline. Answers to any questions submitted before the deadline will be posted in an Amendment to the solicitation at http://fbo.gov/ no later than 2:00 PM EDT, August 6, 2019. The solicitation number must be included on all documents. OFFERS. A strict 7-page submission is enforced. Offeror s entire submission shall not exceed 7 single-sided pages (to include past performance evaluation, quotes, and any attachments). Pages beyond this amount will be removed and not evaluated. All quotes must be submitted on Attachment B -Price Cost Schedule and files shall be in PDF format. All documents must include the Solicitation Number and be submitted in a single email to Lasonja.harvey@va.gov no later than 3:00 PM (EDT) on August 8, 2019. Offers received after this date and time will not be accepted.
