This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2019-03. The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, with a small business size standard of 750 employees The Milo C. Huempfner VA Community Based Outpatient Clinic located at 2851 University Avenue, in Green Bay, Wisconsin 54311 is seeking to purchase (1) Brand Name or Equal MASS Medical (9) Scope Storage Cabinet. (Stock number DAS-9P-LHG-COMP). The acronym BNOE in this document means Brand Name or Equal. All interested companies shall provide quotations for the following BNOE item: Supplies Line Item Description Quantity Unit Price Total Price 0001 MASS Medical (BNOE) (Stock number DAS-9P-LHG-COMP). (must include the following salient characteristics:) *Powder coated metal nine (9) scope storage cabinet that delivers hospital regulated medical grade air through the lumens. * Nine (9) SAS scope holders. * TruHepa fan. * Scope separator rack. * Locking hinged tempered glass doors. * Padded walls. * Removeable drip tray. * Quick disconnect air manifold. * Door activated cabinet lighting * Six (6) foot 120V/10amp NEMA 5-15 wall cord. * Flexible scope storage cabinet that provides HEPA filtered, pressurized airflow into the cabinet and continuous air pressure through each lumen scope. * Allows scopes to hang vertically and freely. * Must meet AORN, SGNA, JC requirements for flexible scope storage * Must have HEPA filter removing up to 0/3um. * Must have self-contained air compressor system * Must have hinged not roll up doors and must be glass. * Must have max air pressure of 25PSI * Must hang at minimum 9 flexible scopes vertically and freely * Must plug in 120V wall plug * Must have door activated LED lighting * Must have capability to hang scope 100 long without touching bottom of cabinet. *Must have Two filtered ambient air vents. *Must have Easy-to-clean removeable drip tray *Must have No contact between scopes and drip tray *Scopes must be kept separated from other scopes *Scopes must be hand straight, not coiled *Must have Positive loading and unloading so user knows scopes are securely in place *Contiguous holder with no welds, glues, or cavities, preventing scope contamination *Rotating scope holders make loading and removing scopes safe, easy and secure. *Scope holders engineered to protect scope heads and support umbilicals. *Designed for colonoscopes, upper GI scopes, EUS scopes, enteroscopy scopes, and other specialty endoscopes. *Must have dimensions of 18.875 D x 57.5 W x 91.9375 H. 1 0002 MASS Medical (BNOE)(Stock number (DAS-truConnect) Scope connector and attaching system (must include the following salient characteristics:) * MASS Medical scope connector and system to attach to the MASS Medical Storage DAS line of scope drying cabinets. For Olympus Colon, Gastro and Endoscopic Scopes. Must connect to PCF-H190L, CF-Q190L, GIF-HQ190 model Olympus scopes. 9 0003 Installation (must be ONLY performed by the Original Equipment Manufacturer Services Delivery shall be provided no later than 60 days after receipt of award of contract. Delivery terms will be FOB destination. The contractor shall delivery line item 0001 and 0002 to Milo C. Huempfner VA CBOC, 2851 University Avenue, Green Bay, WI, 54311. The contractor will coordinate with the Original Equipment Manufacturer for complete installation, which needs to be done within a week after delivery. Line items 0003 must be completed by the Original Equipment Manufacturer. The delivery/task order period of performance is within the next 60 days. The government will follow guidance in 13.106-3(a)(1) for fair and reasonableness price determination. The government will evaluate all vendor supplied salient characteristics of offerors who provide an equal offer. For vendors who offer an equal quote, a side-by-side list of each salient characteristic must also be supplied to the CO at the same time the quote is submitted. The following solicitation provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items (OCT 2018) The Prevention of Gray Market Equipment entering VA Medical Centers: (a) Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.   All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.   Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.   FAR 52.212-3, Offerors Representations and Certifications Commercial Items (OCT 2018) Offerors must complete annual representations and certifications on-line at http://orca.bpn.gov in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. All quotations shall be sent to the shari.lee@va.gov by the closing date of July 30, 2019 at 5:00 PM CST. This is an open-market combined synopsis/solicitation for 1 (9) scope storage cabinet as defined herein.    The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quotes shall list exception(s) and rationale for the exception(s).
