Description

This is a Request for Quotes from available and verified Small Business which can provide a navigation tracking system with cancer registry that can track patients from all tumor/cancer sites and record oncology specific information to help Nurse Navigators better manage patients through the entire continuum of care, including scheduling treatment plans, prompting follow-ups, tracking barriers to treatment. Improve the Cancer Care program s ability to manage appointments, referrals and increase patient retention. All of which would be required to meet the standards of the Commission on Cancer s accreditation program for or the Oncology Service at the Southeast Louisiana Veterans Health Care System (SLVHCS) Oncology Service, located at 2400 Canal Street, New Orleans, LA 70119. If you are not an eligible Small Business please do not submit a quote as it will not be evaluated. Please review the entire Statement of Work (SOW) and schedule for list and quantity of the desired equipment. Quotes will be evaluated and determined to be technically acceptable or not technically acceptable based on functionality of the desired equipment and the best value to the government. All prospective contractors shall be checked against the System for Award Management (SAM) Exclusions list after receipt of offers, bids, and proposals and before any transaction (award, modifications for new work, exercise options, or otherwise extend the duration of current contracts or orders, etc.) is made with the contractor. Eligibility is determined as vendors must be registered and verified in the website SAM. If vendors are not registered in SAM the quote will not be evaluated. IAW FAR 4.1102(a), the prospective contractors shall be registered in the SAM database prior to award of a contractor or agreement.   Inquiries should be made at https://www.sam.gov/SAM/.
