Description
THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. IAW FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought/RFI only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 493110 (size standard of $27.5 Million). Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide a Storage Service meeting following State of Work (SOW) for VA Greater San Diego Healthcare System. A base year plus 4-year option year is requested. Statemen of Work (SOW) BACKGROUND Pathology & Laboratory Medicine Service at the San Diego VA Medical Center provides a full range of clinical testing. In addition, it provides diagnostic services related to surgical pathology and cytology as well as an autopsy service. Anatomic pathology involves the microscopic analysis of cells and tissues. This requires processing of tissues from fixation to the state where they are completely infiltrated with a paraffin wax so that they are ready for section cutting on a microtome. Cut sections are mounted on glass slides and stained. The stained slides allow for microscopic analysis and diagnosis by our pathologists. Once pathology cases are completed, the paraffin blocks and glass slides are stored in cabinets for future use if needed. SCOPE This is a request for a contract for off-site storage, retrieval, and transportation of paraffin blocks and glass slides that VA San Diego is mandated to retain and no longer has the physical space to store on-site. The off-site storage of these pathology materials is to include temperature controlled space, organizing the materials, round-the-clock security and automated tracking and management support. Services security and facilities must meet NARA 36 CFR Part 1234 storage requirements and federal regulations. GOVERNMENT-FURNISHED PROPERTY The glass slides and paraffin blocks will be sent to storage in the original cabinets purchased by VA. CONTRACTOR-FURNISHED PROPERTY The contractor shall provide barcode labels and transmittal slips to be used by VA in organizing materials for storage. CONTRACTOR STORAGE FACILITY REQUIREMENTS The Contractor shall provide long-term or short-term storage of paraffin blocks and glass slides in a climate controlled room at temperatures below 80 F and secure from damage (such as flooding). MATERIAL INDEXING Each VA glass slide cabinet and paraffin block cabinet will have a barcode label for retrieval. The VA uses retention schedules to manage the items. Accurate retrieval of each cabinet is essential due to time constraints for a specific purpose. SECURITY REQUIREMENT The Contractor must have 24-7 security systems in place for the VA designated storage area. Only the contractor employees are allowed into the storage areas. Materials being transported to storage should be in enclosed, locked vehicles by the Contractor s permanent staff. All vehicles are required to be secure at all times whether parked or in transit. PROGRAM MANAGEMENT SERVICES Contractor shall provide program management personnel who have an understanding of the VA Records Management system. The named Program Manager shall provide a single point of contact for dealing with receiving, retrieval and storage of pathology materials. Contractor shall provide labor to relocate glass slide cabinets and paraffin block cabinets from the Pathology & Laboratory Medicine Service to their off-site storage location. Contractor shall inventory all storage cabinets transitioned to their storage location and provide an on-line ordering system that will allow VA to retrieve the cabinets as necessary. EMERGENCY RETRIEVAL Contractor shall be capable of retrieving and delivering a glass slide cabinet or paraffin block cabinet on an emergency basis twenty-four hours a day and three hundred and sixty-five days per year. Cabinets may be delivered in Contractor owned and staffed vehicles or by courier. A 24-hour delivery requirement from the time of formal notification is required. CONTRACTOR RESPONSIBILITIES Contractor must provide for transportation, pick-up and delivery, and storage of all pathology materials required to be stored off-site from the San Diego Pathology & Laboratory Medicine Service. Contractor shall maintain a current inventory of all records. Contractor is responsible for maintaining liability insurance for the duration of the contract and for following all pertinent federal and state regulations with regard to transportation and destruction of confidential documentation. The contractor certifies that all data will be protected until final destruction. The contractor shall be the point of contact for liability should an incident occur (for mitigation purposes) after the pathology materials are picked up by the contractor. The Contractor delivers and picks up new materials from the facility as requested by the histology lab service VA San Diego Pathology lab. No guarantees are made as to the estimated pick-up and delivery frequencies or volumes or the quantities. The Contractor shall be available for emergency pick-up or delivery. Transportation of materials Contractor must communicate and correct any labeling differences between the VA and contractor prior to taking custody. The Contractor shall be responsible for material indexing and retrieval at the cabinet level. The Contractor shall maintain a current inventory of all pathology materials in storage. All contractor vehicles utilized in this contract shall be insured and maintain current state vehicle registration. It is the Contractor s responsibility to ensure that all contractor employees possess a valid State Driver s license. The contractor or his/her employees while performing under this contract shall use no personal vehicles. VA HEALTHCARE SYSTEM RESPONSIBILITIES Provide the Contractor 24 hours notice for a scheduled pick-up. Maintain a current inventory of all pathology materials in storage. PERIOD OF PERFORMANCE The period of performance shall be for one year from date of award. Period of Performance requested 09/01/2019 through 08/31/2020 Requested base year plus 4 option years. Estimated Usage for One Year: Trip pick-up: 12 per month Open Shelf Permanent Withdrawal (Destruction Process): 150 each The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications.
