This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. This solicitation is being issued as a(n) Request For Quotation (RFQ). 3. This solicitation document and incorporated provisions and clauses are those in effect through: Federal Acquisition Circular 2019-03 Defense Federal Acquisition Regulation Public Notice 20190628 Air Force Federal Acquisition Circular 2018-0525 4. This acquisition is set-aside for 100% Small Business Concerns The North American Industry Size Classification System (NAICS) code associated with this requirement is 238330 The Small Business Size Standard associated with this NAICS is $15M Requirement Information Description: For this requirement the Contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform all work in accordance with the specifications of the Statement of Work (25 Jun 19). Work includes both the installation and/or replacement of: ceramic tile, cove base, patching floor, carpet tile and providing labor to remedy unusual requirements related to the installation or replacement of flooring materials. NOTE: Measurements are provided as estimates only. As prescribed in FAR 12.301(b)(1), the following addendum is provided for this solicitation and hereby amends any language therein: 1. Questions may be submitted to the POCs and Contracting Officer listed above no later than 13 AUG 2019 at 2:00PM CDT. Questions submitted after this date may not be answered. 2. Quotes in response to this solicitation must be submitted via email to the POCs and Contracting Officer listed above no later than the established deadline. 3. Quotes must be valid through 30 SEP 2019. 4. All price quotations must be submitted on this document and it must be filled in entirely; all technical submissions must be submitted on a separate document. 5. A site visit will be conducted on 8 AUG 2019 at the Missile Alert Facility (MAF) site. Due to MAF sites being restricted areas, all personnel planning on attending the site visit must be added to an Entry Authorization List (EAL). This is accomplished utilizing Attachment 4 - EAL Submittal Form. This form must be filled out for each employee which will be in attendance at the site visit. Legible photocopies of the front and back of the employee's ID must fit onto page two. Once the form is filled out, save the document as a PDF. All EAL forms must be submitted no later than 29 AUG 2019 to SrA Tyler Rowell at tyler.rowell@us.af.mil and SrA Ryan Onesto at ryan.onesto@us.af.mil. As prescribed in FAR 12.301(c), the following addendum is provided for this solicitation and hereby amends any language therein: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose quote is conforming to the solicitation and will be most advantageous to the Government, price and other factors considered. The following factors are in no particular order and shall be used to evaluate offers: TECHNICAL - The offeror shall submit documentation that provides proof that materials that are intended to be used meet the minimum standards of the contract. This factor will be evaluated on an Acceptable or Unacceptable basis. PRICE - The total evaluated price will be calculated by multiplying the unit price by the quantity for each line item.
