THIS REQUEST FOR INFORMATION (RFI) SOURCES SOUGHT IS ISSUED SOLELY FOR MARKET RESEARCH AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research only to make appropriate acquisition decisions and to gain knowledge of Small Business Capability, including Service Disabled Veteran Owned Small Businesses and Veteran Owned Small Businesses (SDVOSB/VOSB) interested and capable of performing the work. A capability statement that addresses the organization s qualifications and ability to perform as a contractor specifically for the work described in paragraph 5 below. Respondents are also encouraged to provide specific examples (e.g. contract number, point of contact information) of the Contractor s experience providing the same or similar supplies/services to that described in paragraph 5 below. In addition, SDVOSB/VOSB Contractors are asked to acknowledge that they understand the limitations on sub-contracting pursuant to FAR 52.219-14, Limitations on Subcontracting, which will be included in the impending solicitation. Further SDVOSB/VOSB contractors are asked to respond that they are capable of providing the requested services keeping within the parameters of this clause. Lean Six Sigma Green Belt Training. PLACE OF PERFORMANCE. VA Greater Los Angeles Healthcare System, 11301 Wilshire Blvd., Los Angeles, CA 90073. BACKGROUND. The Veteran Affairs Greater Los Angeles Healthcare System (VAGLAHS) requires a contractor to provide Lean Six Sigma Green Belt Training. VAGLAHS has the largest and most complex Homeless program in the country with over 16,000 Veterans served annually and about 600 staff members. VAGLAHS needs continued education and training for existing staff on best practices, in addition to training for new staff recruited in Community Engagement and Reintegration Service (CERS) programs. There is a need to provide clinical training using Lean Six Sigma Green Belt Training due to the clinically demanding population served in the Greater Los Angeles programs. The VA is committed to rapidly expanding training programs to assist staff members. SCOPE. The contractor shall provide the following training: Lean Six Sigma Green Belt Training. This course will teach Lean Six Sigma Green Belt training by providing a good understanding of the Define-Measure-Analyze-Improve-Control (DMAIC) process and an understanding of how process changes can be implemented. The training will also teach about SIPOC diagrams and Critical-to-Quality (CTQ) elements like Cost, process, safety, and delivery. The goal is to gain an understanding of the Cost of Poor Quality (COPQ) and how it is measured in different industries. This course will also teach participants how to understand customer feedback through the Voice of the Customer (VOC) and determine the requirements for their product. Participants will work to determine internal processes and what causes variation by prioritizing issues related to their product. DELIVERABLE SCHEDULE, DELIVERABLES AND PERFORMANCE MONITORING. Delivery Schedule. One (1) Lean Six Sigma Green Belt Training. Training held for twenty (20) staff members. Lean Six Sigma Green Belt certification of completion for each participant. Training evaluation summary report. Upon completion, Participants shall be able to: Describe the Six Sigma tools, understand and apply the same principles in a Green Sigma Project. Design and contribute to a concrete process framework complying with Lean Six Sigma Green Belt requirements. Understand roles and responsibilities that support Lean Six Sigma Green Belt and attain the leadership skills to function as a member of a Six Sigma team. Recognize specific analysis techniques that can be applied to a process. Prepare for the ASQ (American Society for Quality) Green Belt Certification exam. Review of Deliverables. Deliverables: Performance Monitoring Deliverable/Service Requirement Measurement/Metric Surveillance Method/Performed By Frequency/Due Date Acceptable Quality Level Development of curriculum and training materials Training content and materials will incorporate all elements outlined in the SOW as well as any additional agreed upon elements identified during the post-award conference follow-up discussion Review of draft training presentation and materials/COR and Training Specialist At least one month before first training of Training Type #1 100% of all agreed upon elements are identified in training materials. Development of post-training knowledge survey Post-training survey developed by vendor will incorporate all agreed upon training elements between vendor and VA. Review of draft training presentation and materials/COR and Training Specialist At least one month before first training of Training Type #1 100% of all agreed upon elements are identified in the post-training survey Training Implementation Training will include agreed upon elements during actual training presentation. Review of finalized training materials, direct observation by COR Training Specialist, and review of participant feedback. First training to occur within four weeks after the training presentation and post-training evaluation surveys are completed. Subsequent trainings will be reviewed by the COR/Training Specialist on an announced or unannounced basis. 100% of all agreed-upon elements presented during the training. Knowledge gained by training participants Training participants will demonstrate knowledge acquired through the training Online post-training survey/COR and Training Specialist Within one week after each Training Type #1 session There will be an average score of 90% for the post-training survey. SECURITY REQUIREMENTS. No access to the Department of Veteran Affairs computer system is required. The C&A requirements do not apply, a Security Accreditation Package is not required. GOVERNMENT-FURNISHED EQUIPMENT (GFE)/GOVERNMENT-FURNISHED INFORMATION (GFI). The requesting service will be providing media support; projector, speakers, seating arrangements, and space location for training. PERIOD OF PERFORMANCE. The period of performance shall be from September 1, 2019 through August 31, 2020. HOURS OF OPERATION. The contractor is responsible for conducting business, between the hours of 8:00 A.M. PST and 5:00 P.M. PST, Monday through Friday except Federal Holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The contractor shall, at all times, maintain an adequate workforce for the uninterrupted performance of all tasks defined within this SOW when the Government shall keep in mind that the stability and continuity of the workforce are essential. RECOGNIZED HOLIDAYS. The contractor is not required to work on Federally recognized holidays: New Year s Day Martin Luther King Jr. s Birthday President s Day Memorial Day Independence Day Labor Day Columbus Day Veteran s Day Thanksgiving Day Christmas Day CONTRACTING OFFICERS REPRESENTATIVE (COR) No service shall be provided without the approval of the Contracting Officers Representative (COR) or his/her designee of this contract. The Contractor shall not accept any instructions issued by any other person(s) other than the Contracting Officer or his/her delegated representative acting within the limits of his/her authority. Delegation of Authority letter(s) shall be forwarded to the Contractor after contract award identifying the COR(s) and all representatives of VAGLAHS authorized to order services and to accept or reject services from the Contractor. This designation shall be furnished in writing to the Contractor prior to commencement of the contract. CHANGES. The Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which may affect the contract terms and conditions quality, quantity, price, and/or delivery. The Contractor shall contact the Contracting Officer for approval in the event there are changes prior to proceeding. No other individual has the authority to make this binding change and/or commitments. In the event that the Contractor commits to such changes at the direction of any person other than the Contracting Officer, the changes shall be considered to have been made without proper authority and no adjustment shall be made in contract price to cover any increase in costs incurred as a result thereof. NORMAL WORKING HOURS. Normal working hours are defined as 7:00 A.M. to 3:30 PM during weekdays Monday through Friday, excluding weekends and National Holidays. Contractor to work during normal working hours unless otherwise coordinated with the VA COR. If the Contractor proposes different hours of operation, and the Government accepts the Contractor s proposal, the Contractor s proposed hours of operation shall prevail. OVERTIME AND HOLIDAYS. Any overtime or holiday pay that may be entitled to Contractor personnel performing under this contract shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government. SECURITY BACKGROUND CHECKS. All Contract employees who require access to the Department of Veterans Affairs computer system shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to Sub-Contractor personnel requiring the same access. The Contractor at the time of award shall certify that their employees working under this contract have had Security Background prior to working at the VAGLAHS. VA POLICY & REGULATIONS. Parking Policy. It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not invalidate or make reimbursement for parking violations of Contractor s personnel under any circumstances. Smoking Policy. Smoking is not permitted within or around the VA Healthcare System facilities, except in designated areas. Smoking is allowed in smoking shelters and in all outside areas that are 40 feet away from entrances to buildings unless indicated otherwise. Enclosed patios connected to facility buildings are considered part of the building and as such are non-smoking areas. Badges. All Contractor personnel shall be required to wear VA provided identification (I.D.) badges above the waist at all times while on the VA grounds. Contractors shall be required to coordinate with COR to obtain the VA provided I.D. badge for all staff. All VA provided I.D. badges shall be returned at the end of the contract or upon completion of service. Failure to wear ID badges may result in removal from any of the VA facilities and/or otherwise referred to Federal Property.
