PM&RS Request EER 52387, SEPG #106071 Digital Swallow Workstation (DSW) Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Modified Barium Swallow (MBS) study is a motion x-ray procedure of oropharyngeal swallowing that is jointly conducted by the speech pathologist and radiologist to evaluate swallow physiology and to identify the presence, timing, etiology and extent of aspiration and the immediate effects of therapeutic maneuvers on swallow performance. The study (CPT code 92611) is accompanied by a behavioral swallow examination (CPT code 92610) which often guides the focus of the MBS study. These two CPT codes are among the most frequently used codes in the SPC and may have the most critical impact on the patients healthcare status in terms of preventing choking and asphyxiation, aspiration pneumonia and poor oral intake of nutrition, hydration and medication. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Chillicothe VAMC Speech Department requires a system that will interface with CPRS in order for interfacility providers to review records and stills. The system must have the capability to manage exams with a built-in database; view exams with side-by-side, frame-by-frame, etc.; a high resolution exam view; download worklist from HIS, upload exams to PACS system, view exams across all workstations. The Camera s CMOS sensor provides SD and HD output. The process must be compatible with K series videoscopes. The requested DSW comes with a Digital Imaging and Communication in Medicine (DICOM) system which interfaces with the Picture Archiving and Communication System (PACS) and the Veterans Integrated System Technology and Architecture (VISTA) Imaging systems. This upgrade will permit fluorographic studies to be viewed on any computer on our campus and at other VA hospitals that use PACS and VISTA Imaging (e.g., VA ENT). This is an important feature because the providers can readily view these studies and expedite care accordingly, reducing reduplicative procedures at other facilities, as well as reducing travel costs and time. Description of market research conducted and results or statement why it was not conducted: This non-competitive requirement is being sole sourced to a SDVOSB based on 38 U.S.C. 8127 (c). Per VAAR 819.7007 Sole Source Awards To A Verified Service Disabled Veteran-Owned Small Business, a Contracting Officer may award a contract to a verified SDVOSB concern using procedures other than competitive procedures provided the award price will not exceed $5m, a Justification is prepared and posted, the SDVOSB concern is responsible with respect to performance and that the award can be made at a fair and reasonable price that offers best value to the Government. In addition, the Contracting Officers determination whether to make a sole source award is a business decision wholly within the discretion of the Contracting Officer. This requirement will be sole sourced to Thundercat Technologies. Thundercat Technologies is the exclusive authorized federal distributor of Pentax Medical products and services.
