Manufacturers Name: ____________________________________________________________________________ Brand: _______________________________________________________________________ Model No.:____________________________________________________________________ STATEMENT OF WORK VA PITTSBURGH HEALTHCARE SYSTEM CEILING LIFT HARDWARE AND INSTALLATION Title of Project: Ceiling Lift Hardware and Installation Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Contractor shall provide all labor, materials, tools and equipment necessary to install lifts. Background: The VA Pittsburgh Healthcare System has a need for the below scheduled items and installation services. The Patient lifts are essentially a hoist system for moving patients from their beds. The equipment to be purchased is not capable of storing any VA sensitive information. The patient lifts will be installed in Building 1 at the University Drive Campus. Specifics: Provide labor and materials to and install/replace a new 600 lb. motor and accessories Perform weight-load testing Provide Level 3 ICRA for all installations if ceilings are to be opened/exposed Supply power to the lift. Receptacle shall be located above ceiling. Design: Perform site investigation to verify site conditions during design and 48 hours prior to install Drawings Clearly display all ceiling fixtures and equipment. Signed by Project Manager before install. The install shall be performed within 2 weeks of start date. SECURITY & PRIVACY CONTROL: Contractor will be on station to provide service and therefore required to take the generalized privacy training: Privacy Training for Personnel Without Access to VA Computer Systems or Direct Access to or the Use of VA Sensitive Information Training is required upon hire and annually thereafter as long as the contract is in place. Contractor can self-enroll in TMS, take training course # VA20939, and provide a copy of the completed training certificate to the VA Point of Contact or, contractor can read the text version and provide a copy of the completed training certificate to the VA Point of Contact. (JAN 2008) 852-219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items. (ix) METHOD OF AWARD The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. Price will be the only evaluated factor. Award may be made without discussions.
