This announcement constitutes the only solicitation; Quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-03. The associated North American Industrial Classification System (NAICS) code for this procurement is 238330, with a small business size standard of $15M. Services The Department of Veterans Affairs Hudson Valley Healthcare System is seeking the following: Installation of Quartz Epoxy Floor System Kitchen floor. Period of Performance is 25 calendar days to limit down time in kitchen (begin work within 20 days of Award and finish within five days). The contractor must provide containment of dust and smells before any floor prep is started. The contractor must maintain the containment during the work until job is completed and accepted by the VA. The contractor is to prep the floor to receive new Quartz Epoxy Floor System. Area size is approximately 28 x 36 plus 6 cove base. Contractor is required to submit color samples for approval prior to the start of work. Contractor is required to ensure proper pitch to drains for floor drains within the space. Contractor is required to ensure that all tools and equipment is in good working condition. Contractor is required to conform to VHA s security and privacy requirements as described below: All contractor personnel must have in their possession for inspection by VA Police Department a valid state or government issued identification such as drivers license or passport. Personnel may be required to surrender their ID to the VA Police to obtain a temporary day pass at the discretion of VA Police. ID will be returned upon leaving US Government property.
