Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2019-01, dated December 20, 2018. Solicitation 36C26219Q1137 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued under as a Total Service Disabled Veteran Owned Small Business Set-Aside. NAICS Code is 561621 and the Small Business Administration s (SBA) size standard $20.5 Million. The Government is seeking a contract to acquire 32 surveillance cameras and installation, 16 for each of the following: 2 southwest and 4 southwest wings. that at a minimum meets the following salient characteristics with a brand name or equal product for the VA Loma Linda Healthcare System: Salient characteristics: 1. Must provide Digital PTZ only (fixed lens, aimable by technician during installation) 2. Minimum 1080p resolution 3. Must provide MPEG-4 Part 10/AVC video compression 4. Must Supports password protection, IPv4 compatibility 5. PoE powered (no separate power sources) 6. IP-42 water and dust resistance 7. Must Include ceiling mount Brand Name information: Axis M3045-V Network Cameras See attached SF 1449 Solicitation for further details. Place of Performance: VA Loma Linda Healthcare System (VALLHS), 11201 Benton Street, Loma Linda Ca. 92357. Estimated Period of Performance: SEPTEMBER 1, 2019 TO MARCH 1, 2020. Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on an Acceptable/Unacceptable evaluation process. Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm s DUNS#, total price and a completed copy of FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items or a copy of offeror s annual offeror representations and certifications (ORCA) document from www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items.
