This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2019 04 effective 08/07/2019. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 1000 employees. The VA Southern Nevada Healthcare System at at 5406 E EL CAMPO GRANDE AVENUE SUITE 150, Las Vegas, NV 89115 is seeking to purchase 2 Radiofrequency Ablation Multi-Generators and electrodes. Shall be equipped with multi-start option and an individual-start option to deliver RF energy. Must be able to recognize electrodes and heats accordingly. Must be able to differentiate procedure types automatically. Must have a saved setting options, to allow users to save specific procedure information with its saved settings options, displays procedure information to review and print, information can be stored for each needle placement during procedure, and offers multiple settings for each physicians. Must have safety features Must be able to automatically control temperature for every cannula and at the same time, perform temperature safety check all throughout the procedures. Must have a self-diagnostic test feature digitally displays when the equipment is turned on and it verifies internal hardware, software and other connected accessories. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material that are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States, or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside of the continental United States. Cost of installation of replacement material and parts shall be borne by the contractor.(End of Clause) This solicitation does not have a Defense Priorities and Allocation System (DPAS) rating. All quoters shall submit the following: 1 copy of the quotation. This is an open market combined synopsis/solicitation for products as defined herein. The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
