Small Business, Service Disabled Veteran Owned, Veteran owned, Women Owned, etc Email Address Phone Number Physical Address Capability statement pertaining to Scope of Work YMCA FITNESS CLASS INSTRUCTOR & PERSONAL TRAINING INSTRUCTOR BUTLER VA HEALTH CARE SYSTEM 353 North Duffy Road Butler, PA 16001 Performance Period 10/1/2019 9/30/2024 (includes 5 option years) 1. Title of Project: Fitness Classes and Personal Training for Veterans and Employees 2. Scope of Work: The contractor will provide a fitness instructor and personal trainer to instruct a safe, effective and appropriate program of basic exercise for apparently healthy individuals who have completed and signed a consent form and liability waiver. The instructor must have current cardiopulmonary resuscitation (CPR) certification. Certification must be on file prior to instructing any classes. Instructor will provide TB test results prior to starting work. Instructor will have at least one year of experience in fitness class instruction (certification preferred). Personal Trainer will have at least one year of experience in personal training (certification preferred). Instructor will provide notice of any class changes a week before the class. Instructor will ensure all participants sign-in prior to each class. Instructor will ensure all participants have completed and signed a consent form and liability waiver before participating in their first class. The completed/signed consent form and liability waivers will be kept by the Butler VA Health Care System staff (Health Promotion & Disease Prevention Program Manager or designee) and will be available to the instructor prior to each class. Instructor will educate participants on exercise safety. Instructor will document attendance and participation in the medical record of each participant after the completion of each class or personal training session. Instructors shall comply with all VA requirements and trainings as needed to gain access to the VA IT network, including access to CPRS (the government patient medical records system). Requirements include: Completion of (2) online trainings through the VA TMS online training system. Acquiring a VA Contractor PIV badge (including multiple visits to the VA for fingerprints and background check as well as actual issuance of the badge). ISSO/PO Comments: Contractor will be required to obtain a VA Contractor PIV card in accordance with VAPHS Policy. The PIV card will service as a VA ID badge and must be worn at all times while on-site. Include the appropriate risk designation of the contractors based on the PDAT determination. The following language is required from VA Handbook 6500.6: Appendix C: Paragraphs #2a, b, e, #3a, c, d, e, f, j, k, #6a, b, #7(all), #9(all) Contractor employee(s) must sign Appendix D Contractor Rules of Behavior Contractor must meet all VA requirements to obtain/maintain computer access. Contractor must complete VA Privacy and Information Security Awareness and Rules of Behavior and Privacy and HIPAA Focused Training courses prior to the performance of the contract and annually thereafter. Training must be completed in VA s TMS system (https://www.tms.va.gov/SecureAuth35/). Contractors must use the TMS Managed Self Enrollment method to complete the training in TMS. The COR must ensure that all contractors are validated in the PIH domain. Proof of training completion must be verified and tracked by the COR. A local business associate agreement is required. 3. Background: From the U.S. Department of Health and Human Services: Being physically active is one of the most important actions that people of all ages can take to improve their health. The evidence is clear physical activity fosters normal growth and development and can make people feel better, function better, sleep better, and reduce the risk of a large number of chronic diseases. Health benefits start immediately after exercising, and even short episodes of physical activity are beneficial. Even better, research shows that just about everyone gains benefits: men and women of all races and ethnicities, young children to older adults, women who are pregnant or postpartum (first year after delivery), people living with a chronic condition or a disability, and people who want to reduce their risk of chronic disease. The evidence about the health benefits of regular physical activity is well established, and research continues to provide insight into what works to get people moving, both at the individual and community level. Achieving the benefits of physical activity depends on our personal efforts to increase activity in ourselves, family, friends, Veterans, and colleagues. Action is also required at the school, workplace, facility, and community levels. Today, about half of all American adults 117 million people have one or more preventable chronic diseases. Seven of the ten most common chronic diseases are favorably influenced by regular physical activity. Yet nearly 80 percent of adults are not meeting the key guidelines for both aerobic and muscle-strengthening activity, while only about half meet the key guidelines for aerobic physical activity. The lack of physical activity is linked to approximately $117 billion in annual health care costs and about 10 percent of premature mortality. 4. Performance Period: The performance period will be on the fiscal year calendar (October 1 September 30). Contractor will provide a fitness instructor to instruct classes to Veterans and employees five days a week (nine hours total each week). Contractor will provide a personal trainer to deliver personal training to Veterans and employees two days a week (four hours total each week). Classes will be held in the Butler VA Health Care System s Wellness Center except for on Federal holidays when classes will be cancelled. Federal Holidays are as follows: New Year s Day, January 1st or the Friday preceding or Monday following as designated. Martin Luther King s Birthday, 3rd Monday in January President s Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, July 4th or the Friday preceding or Monday following Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October Veteran s Day, November 11th or the Friday preceding or Monday following as designated Thanksgiving Day, 4th Thursday in November Christmas Day, December 25th or the Friday preceding or Monday following as designated. 5. Type of Contract: This contract will be under a firm-fixed-price agreement. The contractor will be paid an hourly rate for each hour-long class or personal training hours and for documentation in the medical record of participants after each class. 6. Cancelled Classes: The contractor will provide at least 24 hours notice to cancel a class. The contractor will notify the Health Promotion & Disease Prevention Program Manager (HPDP-PM) immediately if emergency conditions such as severe weather, hazardous road conditions, or personal emergencies prevent the instructor from showing to a scheduled class. For situations beyond the contractor s control, such as facility maintenance of the Wellness Center, the contractor will only be paid for a scheduled class when less than 24 hours notice has been provided to the contractor. The contractor will be paid for scheduled classes when the instructor is present, but no participants show up. In this case, the instructor must wait at least 15 minutes, and then may leave upon signing a dated, blank sign-in sheet indicating no participants and notifying the HPDP-PM. 7. Emergencies: The instructor will call facility medical response staff for any participant injuries or accidents during a class or personal training session. If necessary, the instructor will call 911 for medical assistance. The instructor is responsible to administer care as deemed appropriate (CPR, etc.) for the circumstance until medical care arrives. 8. VA Security Policies: Instructors are required to complete Privacy and HIPAA training and Privacy and Information Security training. The C&A requirements do not apply. A Security Accreditation Package is not required. A Local Business Associate Agreement between Butler VA Healthcare and Butler family YMCA is required for this contract.
