SOURCE SOUGHT THIS IS A SOURCES SOUGHT ANNOUNCEMENT IS FOR INFORMATION & PLANNING PURPOSES ONLY; IT IS NEITHER A SOLICITATION ANNOUNCEMENT NOR A REQUEST FOR PROPOSALS OR QUOTES AND DOES NOT OBLIGATE THE GOVERNMENT TO AWARD A CONTRACT. THE PURPOSE OF THIS SOURCES SOUGHT ANNOUNCEMENT IS FOR MARKET RESEARCH TO MAKE APPROPRIATE ACQUISITION DECISIONS AND TO GAIN KNOWLEDGE OF POTENTIAL QUALIFIED SERVICE DISABLED VETERAN OWNED SMALL BUSINESSES, VETERAN OWNED SMALL BUSINESSES, 8(A), HUBZONE AND OTHER SMALL BUSINESSES INTERESTED AND CAPABLE OF PERFORMING THE WORK. DOCUMENTATION OF TECHNICAL EXPERTISE MUST BE PRESENTED IN SUFFICIENT DETAIL FOR THE GOVERNMENT TO DETERMINE THAT YOUR COMPANY POSSESSES THE NECESSARY FUNCTIONAL AREA EXPERTISE AND EXPERIENCE TO COMPETE FOR THIS ACQUISITION. SERVICES DISABLED VETERAN OWNED SMALL BUSINESS, VETERAN-OWNED SMALL BUSINESS, 8(A), HUBZONE, WOMEN OWNED SMALL BUSINESS, SMALL DISADVANTAGED BUSINESS, OR SMALL BUSINESS HUBZONE BUSINESS AND (I) MUST PROVIDE A CAPABILITY STATEMENT THAT ADDRESSES THE ORGANIZATIONS QUALIFICATIONS AND ABILITY TO PERFORM AS A CONTRACTOR FOR THE WORK DESCRIBED BELOW. Amputation Peer Program NAICS code 624120 PSC U008 New Base plus 4 Option years. September 30, 2019 to September 29, 2024. DESCRIPTION/SPECIFICATIONS: Amputation Peer Program Scope of Work: The contractor shall provide all manpower, resources and materials necessary to expand the ASoC Peer Program. This includes a Train-the-Trainer program for 25 Amputation Rehabilitation Coordinators (ARCs). The Train-the-Trainer program shall be a hands-on, in-person training that equips the ARCs to meet the needs of Veterans with limb loss through a peer program. Trainer candidates shall experience a training that focuses on the characteristics of adult learning, effective training techniques, facilitation tips, and role playing to ensure learning transfer. Trainer candidates shall train a live class at the conclusion of the program to test their skills. Once the program is complete the ARCs shall be certified trainers that will in turn train and certify Veteran amputee peer mentors. The instructor of this program shall be a Certified Master Trainer through the Association for Talent Developer (ATD). In addition, the contractor shall provide education materials to be distributed to Veterans with new amputations. This includes magazines, pamphlets and books. These materials enhance patient centered care in amputation rehabilitation and support the needs of both the providers and Veterans alike. The contractor shall also provide corporate memberships to the 7 Regional Amputation Centers and the 18 Polytrauma Amputation Network Sites. Background: In the past year the VA provided care to over 90,000 Veterans with amputations, with over 45,000 of those being major limb amputations. In addition, the VA performed over 10,000 amputations last year. It is anticipated that with the large population of Veterans with diabetes this number could rise dramatically in the coming years. In order to support the needs of both the healthcare provider and patients alike it is necessary to have educational materials that provide information and resources to cope with having an amputation. Research has also shown that peer to peer support is extremely important in recovery and returning to the highest level of function and is a key recommendation in the VA/DoD Clinical Practice Guideline. As a result, it is of utmost importance that VA provide this service as part of the overall rehabilitation process. Access to educational materials, training and a peer to peer programs will place VA in a unique position of providing the best amputation care. Period of Performance: The government requests four (4) additional one-year options. Base plus four years: expected dates are (base) September 30, 2019 with four option years, ending in September 29, 2024. Government Recognized Holidays: Unless specifically authorized in writing by the Contracting Officer, no services will be provided and no charges will be incurred and/or billed to any order on this contract on any of the observed Federal Holidays. There are 10 Federal holidays set by law (USC Title 5 Section 6103). Under current definitions, four are set by date: New Year's Day January 1 Independence Day July 4 Veterans Day November 11 Christmas Day December 25 If any of the above falls on a Saturday, then Friday shall be observed as a holiday. Similarly, if one falls on a Sunday, then Monday shall be observed as a holiday. The other six holidays are set by a day of the week and month: Martin Luther King's Birthday Third Monday in January Washington's Birthday Third Monday in February Memorial Day Last Monday in May Labor Day First Monday in September Columbus Day Second Monday in October Thanksgiving Fourth Thursday in November Place of Performance: Trainings will occur in Knoxville, TN and Seattle, WA. Type of Contract: The Government anticipates award of a Firm Fixed-Price contract. Contract Pre-Performance Meetings: The contractor shall participate in regularly scheduled planning teleconference meetings throughout the performance of the contract. General Requirement & Task: The purpose of the contract is to obtain training and supplies, educational materials, and one-year corporate memberships for 7 Regional Amputation Centers and 18 Polytrauma Amputation Network sites across America. Educational materials will be distributed to a VA warehouse where they can be ordered by Amputation Clinic providers. Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this SOW. Formal Acceptance or Rejection of Deliverables: The Government will review each deliverable and provide feedback/comments. The contractor shall have five business days to incorporate feedback/comments and make appropriate revisions. The contractor shall provide the revised version of each deliverable to the Contracting Officer s Representative (COR). The COR will review and determine final acceptance by the Government. The COR will notify the contractor of final acceptance within five business days. Changes to the SOW: Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor. Contractor Reporting Requirements: Contractor will contact the COR within five (5) to notify when the educational documents are shipped Travel: Two episodes of travel for training of the Amputation Rehabilitation Coordinators. Contractor Experience Requirements: The instructor of this program shall be a Certified Master Trainer through the Association for Talent Developer (ATD). https://www.td.org/certification Confidentiality and Disclosure: It is agreed that: The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this contract, are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer at the conclusion of the contract. The Contracting Officer will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this contract. No information shall be released by the contractor. Any request for information relating to this contract, presented to the contractor, shall be submitted to the Contracting Officer for response. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the Contracting Officer. Contractor Security Requirements: The Contractor will not access VA Information Technology (IT) systems and will not access sensitive/personnel identifiable information (PII).
