- Attachment 1 36C24819Q1440 Page 1 of 1 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, C.W. Bill Young VA Healthcare System, Contracting Office, Bay Pines, FL. 33744 has a need to purchase Physiological Monitoring Systems. The requested brand name or equal is Spacelabs Healthcare. The referenced manufacturer is Spacelabs Healthcare, L.L.C. The RFQ # 36C24819Q1440. The government anticipates making an award as a firm-fixed price contract resulting from this solicitation. The method of evaluation for this requirement shall be Lowest Price Technically Acceptable. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-05 (08-13-2019). The combined synopsis solicitation is: 100% Set-Aside for Service Disabled Veteran Owned Small Business (SDVOSB). The NAICS Code is 334510 - Electromedical and Electrotherapeutic Apparatus Manufacturing; the size standard is 1,250 employees. Note, Interested SDVOSB s Offerors must be listed and registered in Vetbiz.gov to be considered for Award. This solicitation is being issued as an evaluation for SDVOSB concerns. If award cannot be made, the solicitation will be cancelled and the requirement resolicited. Responses shall be on an all or none bases, no partial submissions will be accepted. The Government reserves the right to make no award at all. The Government intends to award a Firm Fixed Price award for this requirement. The evaluation type used for this requirement shall be Lowest Price Technically Acceptable. A Brand Name or Equal Description of the product is required. This permits prospective contractors to offer products other than those specifically referenced by brand name. All offers must work with existing equipment that has already been purchased and is currently in use at the station and also to meet standardization requirements throughout the hospital. If substituting other than equipment listed, must provide proof that substituted equipment will interact without issue and interface with existing equipment. Failure to provide adequate descriptive literature with the quote will be grounds for rejection of quotation. All quotes are due NLT 9/3/2019 @ 07:00 AM EST via e-mail to: elena.bass@va.gov. The Subject Line of the response e-mail shall include RFQ 36C24819Q1440. Offers received after the exact time specified in the solicitation are considered late and may at the discretion of the Contracting Officer be considered if the action would not unduly delay the acquisition or are deemed to be in the best interest of the Government. Attention: Quotes shall be only be submitted using only the attached Pricing Schedule Excel document. Please read all the tabs of the document and modify only the fields highlighted in green, including the Cover Sheet. You are reminded that representatives from your company SHALL NOT contact any Bay Pines VA Health Care System personnel to discuss this RFQ during this RFQ process.
