- SUBJECT: Professional Labor Services Contract DISCLAIMER: This Sources Sought is issued solely for information and planning purposes and does not constitute a solicitation. Neither unsolicited proposals nor any other kind of offers will be considered in response to this Sources Sought. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought. All information received in response to this RFI that is marked Proprietary will be handled accordingly. Responses to the Sources Sought will not be returned. At this time, questions concerning the composition and requirements for a future Request for Quote will not be entertained. Subject: The Department of Veterans Affairs is conducting market research in order to provide a service agreement for the VAMC Martinsburg 510 Butler Avenue, WV 25405. The government anticipates awarding a Firm Fixed Price award base and 4 optional year contract that will provide a Service agreement Professional Labor Services Contract, see below attachment for details. OBJECTIVE: See attachment. RESPONSE INSTRUCTIONS: Name and Address of Company. Any small business designation as certified by the Small Business Administration. A through description of experience in managing support requirements of this nature. 5.0 CONTACT INFORMATION: Any questions related to this Sources Sought shall be directed to mark Smith at Mark.Smith25@va.gov All information regarding Capabilities Statements or any other proprietary information relative to this Sources Sought shall be submitted via email to Mark.Smith25@va.gov no later than 12:00 pm EST on 9/6/2019 A.3 STATEMENT OF WORK SCOPE OF WORK The Department of Veterans Affairs Medical Center has a requirement to provide the following contractors: Two general laborers: To perform internal moves of furniture and supplies as identified by the COR in support of Medical Center Operations including reactivation of areas after renovation by construction. Two mid-level electricians: To perform the work of a Journeyman Electrician without more than normal supervision. Which must include experience in operating, maintaining, and repairing equipment, controls and devices related to primary and secondary electrical systems. Two Carpenters: To perform tasks involved in the operation, maintenance, and repair of the Martinsburg Veterans Affairs Medical Center building services that include carpentry and painting. Two Plumbers: To perform Install, modify, and/or repair and adjust new and existing building utility equipment to include but not limited to: waste closets, tubs, showers, lavatories, urinals and toilets, steam and hot water heating equipment, pressure reducing stations, circulating pumps, steam, electric water heaters and kitchen equipment. Two Air Conditioning Equipment Mechanics: To perform a variety of duties concerned with the installation, troubleshooting, repair, relocating, overhaul, and modification of various special-purpose refrigeration and air conditioning units and systems. Work will be performed during work hours of 7:00a.m.- 3:30p.m. with a 15-minute break mid-morning and a 15-minute break mid-afternoon in addition to a 30-minute lunch break at/around noon. Weekends and Federal Holidays are excluded from performance. The following Federal Holidays will be observed (and any other day specifically declared by the President of the United States to be a national holiday): New Year s Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. The Contracture employees will report to Building 314B. Contract staff will sign in and out on a time sheet each day work is performed to keep track of time and attendance. This time sheet will be faxed to the contractor weekly. If a contract employee does not report to work on a given day, he/she must contact their Supervisor with information that they will not be reporting for duty. The COR will perform a weekly check of work performed by the contractor to assure quality requirements. Any deviations will be corrected by the contractor before proceeding to the next process. The contract employees will not require computer access. Contractors shall be required to wear a company uniform provided by their company, which clearly displays the name of the company and the individual, and shall be of a neat, clean appearance. The contractor s company will also need to provide the appropriate footwear and PPE required for their duties. WORKER'S COMPENSATION The contractor agrees to procure and maintain while the contract is in effect, Worker's Compensation and Employers Public liability Insurance in accordance with the laws of the State of West Virginia. EVIDENCE OF COVERAGE Before commencing work under the contract, the contractor shall furnish to the contracting officer a certificate of insurance indicating that the coverage outlined in the contractor's responsibilities clause has been obtained and that such coverage may not be changed or cancelled unless 30 days advance written notice is provided to the contracting officer. SAFETY REQUIREMENTS In the performance of this contract, the contractor shall take such safety precautions as the Contracting Officer or designee may determine to be reasonably necessary to protect the lives and health of occupants of the building. The contracting officer will notify the contractor of any non-compliance with the foregoing provisions and the action to be taken. The contractor shall after receipt of such notice immediately correct the conditions to which attention has been directed. IDENTIFICATION PARKING, SMOKING, CONDUCT, AND VA REGULATIONS The Contractor's employees shall abide by all VAMC station policies and regulations. Contractor employees are subject to the rules of the Medical Center, applicable to their conduct, security, safety, etc. The Contractor's employees shall wear visible identification at all times while on the premises of the VAMC. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VAMC Police/Security Service. The VAMC will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Smoking is prohibited inside any buildings at the VAMC. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.
