Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Ref: 5990011 Bard Ventralight ST Mesh with Echo 2 Positioning System 4.5 Circle x 2 Ref: 5990015 Bard Ventralight ST Mesh with Echo 2 Positioning System 6 Circle x 2 Ref: 5991015 Bard Ventralight ST Mesh with Echo 2 Positioning System 4 x 6 Ellipse x 2 Ref: 5991520 Bard Ventralight ST Mesh with Echo 2 Positioning System 6 x 8 Ellipse x 2 Ref: 5991823 Bard Ventralight ST Mesh with Echo 2 Positioning System 7 x 9 Ellipse x 2 Ref: 5992025 Bard Ventralight ST Mesh with Echo 2 Positioning System 8 x 10 Ellipse x 2 Bard Ventralight ST Mesh with Echo 2 Positioning System Total of 12 pieces Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The familiarity of the product will be important, so it is important to use similar products. Time spent training staff and residents to use different products from different vendors will take away from the time that is spent performing procedures as well as create confusion among O.R. staff on determining which products from which brands they are using impacting patient care. Additional cost will be incurred training staff and residents by implementing a different product from a different brand. Description of market research conducted and results or statement why it was not conducted: Queried SAC, NAC and GSA which resulted in zero (0) awards in place for the Bard Mesh as requested. Recent market research conducted by Denise Maynard and the Bard brand name J&A approved by the Executive Director of VHA Regional Procurement Office Central on 9 Aug 2019. I emailed all authorized distributors of BD (owns BARD), It was determined that not all BARD products could be provided by other vendors. The email to the authorized vendors resulted in one (1) vendor that can provide the requirement. This requirement does not meet the rule of two (2) set-aside for SDVOSB/VOSB and since no non-manufacture rule waiver in place, a set-aside would not meet the non-manufacture rule policy. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
