RFI 36C25619Q1459 7 DISCLAIMER: THIS IS A SOURCE SOUGHT NOTICE ONLY. THIS SYNOPSIS IS NOT A REQUEST FOR QUOTE, PROPOSAL, OR BID. This notice is not a solicitation as defined by FAR 2.101, therefore it shall not be construed as a commitment by the Government to enter into a contract, nor does it restrict the Government to an acquisition approach. All information contained in this RFI is preliminary as well as subject to modification and is in no way binding on the Government. Information submitted in response to this notice is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. The Government is requesting that restrictive or proprietary markings not be used in response to this notice. If a solicitation is released, it is will be synopsized in the Federal Business Opportunities (FedBizOpps) website or GSA. It is the responsibility of the interested parties to monitor these sites for additional information pertaining to this RFI. The anticipated Small Business Administration (SBA) size standard for the North American Classification code (NAICS) 492110 is 1,500 employees. 1. Purpose: The Michael E. Debakey Veteran Affairs Medical Center (MEDVAMC) hereby issues the following Sources Sought Notice as a Request for Information (RFI). This RFI is being used for Market Research purposes only to seek contractors with the capability to provide all labor, tools, materials, supplies, equipment, vehicles/transportation, supervision, information security, and insurance required to perform Laboratory Courier Services to and from the MEDVAMC in accordance with the attached Draft Scope of Work (SOW). 2. Place of Performance: The work to be performed under this requirement will be performed primarily in the Houston, TX geographic area. The courier service will be transporting collected routine laboratory specimens, STAT laboratory specimens, with hotshot availability for emergency deliveries between the MEDVAMC, local reference laboratories, laboratory suppliers, local airports and the MEDVAMC s Community Based Outpatient Clinics identified below. 2.1. Main Street, Tomball, TX 77375 3. Opportunity: The MEDVAMC is seeking information from potential contractors on their ability to provide these services. THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. All capable Service Disabled Veteran Owned Small Business (SDVOSB) concerns are encouraged to provide responses to this RFI to assist the MEDVAMC in determining potential levels of competition available in the industry. NOTE: All Questions and information shall be answered and provided in Sections 4 through 7 for the government to consider whether the contractor has the capability to meet the government requirement. 4. Responses Requested: The MEDVAMC requires the following questions answered in this RFI: Answers that are not provided shall be considered non-responsive to the Request for Information. a. Is the Contractor licensed/certified by the State of Texas to properly handle and transport Laboratory specimens? Contractor shall attach documentation of this License/certification to be viewed by the government to be considered as part of the Market Research and responsiveness to this Sources Sought notice. b. Does the Contractor possess the required vehicles with complete Spill Kits for a bio-hazard cleanup for the performance of these services? Contractor shall attach evidence of this information to be viewed by the government to be considered as part of the Market Research and responsiveness to this Sources Sought notice. c. Prospective contractors shall provide their point(s) of contact name, address, telephone number, and email address. In addition, contractors shall provide the company's business size, and Data Universal Numbering System (DUNS) Number. d. Is your company a small business (SDVOSB, VOSB), SDBs, HUBZone, or 8A concern? NAICS Code: 492110, PSC Code: R602, Courier and Messenger Services. Please provide proof of qualifications. e. Is your company available under any Government Wide Agency Contract (GWAC), General Services Administration Schedules (GSA), Indefinite Delivery Indefinite Quantity (IDIQ), and/or Blanket Purchase Agreement (BPA)? If so, please list the contract number and a brief summary of the products and services provided. 5. Capability Statement: Provide your capability statement to demonstrate your company s understanding and ability to perform Regulated Medical Waste Services. At a minimum: Include descriptive identification/information on fleet of vehicles on-hand for this type of service. Provide descriptive emergencies and emergency response time after receipt of call from agency. List all certifications and licenses required to perform RMW services in the state of Texas. 6. Relevant Experience: Please provide Government or Commercial agency relevant experience within the last three (3) years. Information shall include: Contract Number Name/Point of Contact (POC) POC phone number POC email address Brief description of how the contract referenced at 6(a) relates to the technical capability within the pest control industry for the interior and exterior of government and/or commercial buildings. 7. Subcontracting: If subcontracting, provide subcontractor s business name, address, phone number, and DUNS. Identify specific task(s) to be performed by the subcontractor and prime contractor. Contractor shall possess the capability to provide all requirements and objectives. 8. Attachment 1: Use format to submit Contractor s recommendations, CLIN Structure, etc. 9. DRAFT Scope of Work (SOW) Laboratory Courier Services General Information: The Pathology and Laboratory Services at the Michael E. DeBakey VA Medical Center (MEDVAMC) is pursuing a contract with a base year (12 months), plus 4 (four) 1 (one) year option periods for Laboratory Courier Services. Veterans from around the country are referred to the MEDVAMC for specialized diagnostic care, radiation therapy, surgery, and medical treatment including cardiovascular surgery, gastrointestinal endoscopy, nuclear medicine, ophthalmology, and treatment of spinal cord injury and diseases. The MEDVAMC is home to a Post-Traumatic Stress Disorder Clinic; Network Polytrauma Center; an award-winning Cardiac and General Surgery Program; Liver Transplant Center; VA Epilepsy and Cancer Centers of Excellence; VA Substance Abuse Disorder Quality Enhancement Research Initiative; Health Services Research & Development Center of Innovation; VA Rehabilitation Research of Excellence focusing on mild to moderate traumatic brain injury; Mental Illness Research, Education and Clinical Center; and one of the VA s six Parkinson s Disease Research, Education, and Clinical Centers. In late 2012, the MEDVAMC received official designation as a Kidney Transplant Center. Description of Services: This requirement is to establish a Laboratory Courier Service Contract (specimen pickup, urgent laboratory item pickup and delivery) for the MEDVAMC. A fully-licensed courier company shall provide pickup and delivery within the Houston, TX market place. Payment shall be made monthly, on a daily per stop basis. The points of contact shall be authorized laboratory personnel at the MEDVAMC. Scope: The MEDVAMC requires laboratory courier services. The courier service is for transporting collected routine laboratory specimens, STAT laboratory specimens, STAT laboratory supplies, with hotshot availability for emergency deliveries between the MEDVAMC, local reference laboratories, laboratory suppliers and local airports to include pick-up and drop-off. The Vendor shall provide all necessary management, labor, equipment, vehicles, transportation, information security (HIPAA), and insurance to provide lab courier services to and from the MEDVAMC. Unscheduled courier services will need to be available 24 hours a day, 7 (seven) days a week. All work shall be performed in accordance with applicable federal, state, and local regulations or ordinances; information security (HIPAA), dangerous goods, diagnostic specimens or bio-hazardous materials. Period of Performance: The period of performance shall be for one (1) 12-Month Base Year, plus four (4) 12-month option years; to be exercised at the discretion of the government. The Period of Performance is estimated to be as follows: Base Year: 12/15/2019 through 12/14/2020 Option Year 1: 12/15/2020 through 12/14/2021 Option Year 2: 12/15/2021 through 12/14/2022 Option Year 3: 12/15/2022 through 12/14/2023 Option Year 4: 12/15/2023 through 12/14/2024 Hours of Operation: Contractor shall assure adequate staffing to provide continuous courier services on an as-needed basis at all times (24 hours/day), on a year-round basis (365 days a year). Main Street, Tomball, TX 77375 MEDVAMC Laboratory Personnel Requirements: The MEDVAMC send out laboratory personnel will place the specimen in a sealed tamper evident container and prepare it for pickup/delivery. Once prepared the MEDVAMC personnel will call the Vendor and notify them that the specimen &/or item is ready for pickup. Laboratory Personnel will continually evaluate pick-up/delivery requirements to identify opportunities for improvement. As improvements are identified authorized personnel will work with the Vendor to create/design pick-up/delivery schedules when appropriate. Courier Requirements: The Vendor shall deliver/pick-up routine laboratory specimens, STAT laboratory specimens, STAT laboratory supplies on an as needed basis &/or on an agreed upon delivery schedule. Laboratory specimens will be properly packaged by the MEDVAMC personnel for transportation. All specimens shall be transported in accordance with all Department of Transportation (DOT) and OSHA Codes of Federal Regulations (CFR) 49 and 29. The Vendor shall notify the Send Out Laboratory at 713-794-7332 (M-F 0730-1600) or the Chemistry Pit area at 713-794-7333 (for all other times); if there will be a delay in picking up specimens and if any unusual event impedes normal pick-up/delivery. This shall be done immediately upon realization that the schedule cannot be maintained. Authorized Laboratory Personnel are: Some containers/packages may require special handling and will be marked Fragile or This Side Up. The Vendor shall comply with all special handling instructions marked on containers/packages. Items must be kept out of contact with environmental hazards such as dust, snow, and rain. The Vendor personnel drivers shall not leave vehicles in extreme heat or cold while transporting collected routine laboratory specimens, STAT laboratory specimens, STAT laboratory supplies, between the MEDVAMC, local reference laboratories, laboratory suppliers and local airports to include pick-up and drop-off. Should specimens and/or transported items be lost, broken, or otherwise damaged, the Vendor shall be required to submit a preliminary summary incident report to the COR within 24 hours of the incident. The Vendor shall provide a written incident report explaining the details of the incident; specifically, what caused the incident (who, what, where, when, and how) within seventy-two hours of the incident. Bloodborne Pathogens: The Vendor and the Vendor personnel shall comply with the effective VA Medical Center Policy (and Occupational Health requirements in accordance with OSHA Blood Borne Pathogens (BBP) Law and the OSHA Tuberculosis Compliance Directive, enforceable under OSHA s General Duty Clause) governing healthcare workers infected with a blood borne pathogen. If the Vendor obtains information that a person providing services under this requirement may be infected with a blood borne pathogen, the Vendor shall immediately advise the COR. HIPAA Compliance: The Vendor must adhere to the provisions of Public Law 104-191, Health Insurance Portability and Accountability Act (HIPAA) of 1996 and the National Standards to Protect the Privacy and Security of Protected Health Information (PHI). In addition, all VA security policies and applicable confidentiality statues such as 38 U.S.C. §5701 (VA Claims Confidentiality), 38 U.S.C. §5705 (Confidentiality of Healthcare Quality Assurance Review Records), 38 U.S.C. §7332 (Confidentiality of certain medical records), as well as 45 C.F.R. Parts 160, 162, and 164 (Health Insurance Portability and Accountability Act) should be followed. Vendor Vehicles: Vendor vehicles and storage areas used in the performance of this requirement shall be licensed and meet the minimum requirements as mandated by the state of Texas. Vehicles shall have the company name/logo prominently displayed. Each vehicle used in the performance of this requirement shall be adequately stocked with at least one current/up-to-date complete Spill Kit for a bio-hazard clean-up in case of leakage or accidental discharge from container. The Vendor shall provide comprehensive automobile liability insurance required by the state of Texas. Upon request, the Vendor shall provide certified proof of automobile liability insurance for all vehicles used by the Vendor in the performance of this requirement. Vendor vehicles shall be maintained in a sanitary clean condition, absent of foreign debris and contaminants. All vehicles shall be in good working condition with all doors and trunks able to fully close and lock. Unattended vehicles with VA contents shall be locked at all times. The following types of vehicles are prohibited from use under the requirement: a) Bicycles b) Motorcycles c) Vehicles without a secure cargo area, such as convertibles d) Pickup trucks without fully enclosed and lockable bed cover, unless cargo is secured in the driver s cab. Vendor Drivers/Personnel: Vendor drivers shall maintain a valid driver s license for the type of vehicle operated as required by the state of Texas. The Vendor shall be licensed by the state and/or local health authority in those localities where licenses are required. Vendor personnel, while performing the services of this requirement, shall conduct themselves in a professional business-like manner. The vendor s drivers shall observe and abide by all signage, speed limits, pedestrian right-of-way, emergency vehicle right-of-way, and designated roadways. Under no circumstances shall any animal or person other than the driver ride in the vehicle or handle items being transported. Indemnification: The Vendor shall be liable for and shall indemnify and hold harmless the Government against all actions or claims for loss of or damage to property or the injury or death of persons, arising out of or resulting from the fault, negligence, or act or omission of the Vendor, its agents, or employees. Vendor Support for Employees: This is not a personal services contract. The vendor shall be responsible for supervision of its employees. The Vendor shall be responsible for protecting vendor employee furnishing services. To carry out this responsibility, the vendor shall provide or certify that the following is provided for all their staff providing services under the resultant requirement: Workers compensation Professional liability insurance Health examinations Income tax withholding Wages in compliance with Service Contract Labor Standards Social security payments. Vendor Invoice Submission: The Vendor shall submit invoices monthly in arrears covering the services performed under this requirement. The Vendor shall provide an itemized invoice to the required VA invoicing address or submission online through the appropriate website within 30 days of the end of each month services were provided. Invoices for services provided in the month of September must be received for certification no later than October 30th.
