- Reclamation has a need for the REPAIR OF SMALL ELECTRIC MAN LIFT. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement and attachments constitute the only solicitation; proposals are being requested and a written solicitation will not be issued. Consider this solicitation (#140R6A19Q0012) as a Request for Quotes (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2019-03. This solicitation is set-aside for small business. NAICS 811310. The small business size is $8 million. The resulting purchase order will be a firm-fixed price type contract. Line Item #10 Repair of E450AJ Man Lift PERFORMANCE WORK STATEMENT 1.0 BACKGROUND The JLG E450AJ requiring repair has been inoperable since at least 2015 due to failure of traverse motors and controllers. The serial number for the JLG in need of repair is 300064595. 2.0 SCOPE The contractor shall furnish all labor, equipment and materials to inspect, advise, install, repair and/or replace the following identified components: replace and install (2) Two traverser motors, power module, contactor panel or intermediate power cables. 2.1 LOCATION The work to be performed under this contract will be performed at the contractor ¿s facility. 2.2 SHIPPING: The FOB destination is the Bureau of Reclamation, 205 Chamberlain Rd. Mills WY 82644. The contractor will contact the Contract Specialist Audrey Whetham at 307-261-5619 or e-mail at awhetham@usbr.gov three days before picking up and before returning with the date and the time they will be at Reclamation so personnel are available to operate the crane for loading and unloading. Loading hours are Monday through Thursday from 7:30 am to 4:00 pm excluding holidays. 3.0 TASKS Initial Inspection: The contractor shall perform an incoming inspection noting all abnormal conditions or damage to the JLG man lift. Record all findings and document. Documents from the initial inspection shall be sent to the Quality Assurance Evaluator (QAE). After review of the initial inspection documents, the QAE will provide approval for the continuance of work. Replace and Install (2) Two Drive Motors: The contractor shall replace and install (2) Two traverse motors, within the original equipment manufacturers (OEM) specifications. Repair the Electrical System: The contractor shall replace burnt cables for drive controls of the traverse motors, within the original equipment manufacturers (OEM) specifications. Perform Final Inspection of the JLG: The contractor shall test JLG for performance and working conditions according to manufactures checklists. The contractor shall provide Reclamation with documentation of the performance. The contractor shall notify the Government as soon as possible when it is apparent that extra work is required outside of the base scope of work. If deficient conditions are found, the contractor shall notify the Government. Deficient means out of limit conditions for inspections or tests specified herein or found in accepted national or industry standards. Deficient also means conditions that would not permit the exciter to be returned in an ¿as-new ¿ condition. Approval from the Government is required for work outside the scope contained herein or at additional cost above the contract award amount. 3.1 GOVERNMENT-FURNISHED PROPERTY & MATERIALS The following table summarizes the description of Government property, cost if damaged or lost, unique item identifier and condition for the exciters that the Government will be releasing to the contractor while repairing. Description of Property Quantity and Unit: JLG E450AJ Electrical Man Lift; Quantity and Unit of Measure: 1 Each; Acquisition Cost if Damaged or Lost $40,000 if lost Actual cot to repair, if damaged; Condition: New 4.0 PERIOD OF PERFORMANCE The term of the PO will be October 1, 2019 through January 31, 2020. 5.0 CONTRACTOR PERSONNEL Contractor employees shall comply will all federal, state, and local laws. 6.0 CONTRACTOR QUALITY CONTROL PLAN Quality control procedures shall ensure that bearing dimensions are within all requirements of the manufacturing drawings and/or Reclamation data in-lieu of a supplied manufacturing drawing. The contractor shall ensure that the bearing surfaces are totally clean and free of contaminants. High quality tinning will be achieved only if the surfaces are totally clean and free of contaminants. The contractor shall ensure that non-bonding surfaces are protected from accumulation of tinning material. 7.0 QUALITY ASSURANCE EVALUATOR (QAE) The CO has designated the employee listed in Section 10 of this Performance Work Statement as the QAE to act as the authorized representative, who assists in the technical administration of the PO. The CO has not authorized the QAE to make any contractual commitments, or to authorize any contractual changes on the Government ¿s behalf. If any doubt exists as to the extent of the authority of the QAE, the contractor shall contact the CO for guidance before taking action on the matter. The QAE will determine whether the contractor has satisfied the expected quality level as per the contractual requirements. 8.0 PERFORMANCE REQUIREMENTS SUMMARY TABLE (PRST). The following table summarizes the tasks, the performance standard, monitoring method, and disincentives for not meeting performance standards. Task Description: All Tasks; Performance Standard: All tasks with no deviation in timeliness or quality; Method of Monitoring: 100% Inspection; Disincentives: (1) Re-work at contractor ¿s expense (2) Payment deductions as specified in Section 10. 9.0 GOVERNMENT REMEDIES All work shall be subject to inspection, approval, and acceptance by the Government. The Government will compare the contractor's performance to industry standards and will not exclude common sense considerations as applied by the QAE. The CO will follow the requirements of FAR Clause 52.212-4 for the contractor ¿s failure to correct nonconforming services. The Government will consider work incomplete when any one of the following conditions exits: - The contractor did not perform the task. If the performance of any required service is unsatisfactory, and poor performance is clearly the fault of the contractor, the Government may reduce the PO price to reflect the reduced value of defective services, if the Government gives the contractor a written Purchase Order Discrepancy Report prior to executing such payment deductions. The CO will determine the payment deduction amount by calculating the percentage of acceptable services performed by the contractor. The Government will use the total number of deficiencies found to determine the percentage found unacceptable.
