- COMBINED SYNOPSIS/SOLICITATION Combined Synopsis/Solicitation IAW FAR Subpart 12.6 Military Health System (MHS) Virtual Health (VH) Telehealth Cart Maintenance, Repair, and Support Services This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W81K0419Q0030 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019- 06 and Defense Federal Acquisition Regulation Supplement Publication Notice 20190809. The North American Industrial Classification System (NAICS) is 811219, Other Electronic and Precision Equipment Repair and Maintenance. The small business size standard is $20,500,000.00. This procurement is set aside for 100% Service Disabled Veteran Owned Small Business (SDVOSB) concerns. DESCRIPTION OF REQUIREMENT: The Government has a recurring requirement for maintenance, repair, and support services for Virtual Health (VH) Telehealth Carts located at Military Health System (MHS) Military Treatment Facilities (MTF) within the Continental United States (CONUS) and Outside of the Continental United States (OCONUS). The Contractor shall provide all equipment, materials, personnel, services, software, licenses and supplies necessary to provide maintenance, repair, and support services for the VH Telehealth carts as outlined in the Performance Work Statement. The Government intends to award a single Indefinite Delivery, Indefinite Quantity contract with a five year ordering period. ORDERING PERIOD 30 September 2019 to 29 September 2024 The provision at FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. See FAR Provision 52.212-1, Instructions to Offerors - Commercial Items and ADDENDUM TO FAR 52.212-1 for requirements regarding preparation of quotes. The provision at FAR 52.212-2, Evaluation -- Commercial Items is applicable to this requirement. The provision and the ADDENDUM TO FAR 52.212-2 have been incorporated in full text within the solicitation below. All quoters shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its quote. The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The clause and the ADDENDUM TO 52.212-4 have been incorporated in full text within the solicitation below. The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. All additional FAR clauses cited within this clause that are applicable to the acquisition have been incorporated into the solicitation below. Any additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices have been included in the solicitation below. The Defense Priorities and Allocations System (DPAS) is not applicable to this requirement. QUESTIONS CONCERNING THE SOLICITATION: Interested parties shall submit any questions concerning this solicitation no later than 12:00 PM Central Time on Friday, September 13, 2019 for the Government's consideration. Those questions not received within the prescribed date may not be considered. Electronic submission via e-mail is required. Only emailed quotes received directly from the quoter are acceptable.
