- NOTICE OF INTENT TO AWARD SOLE SOURCE Title: Security Support Agreement for Facility Surveillance System Located at Salt Lake City, Utah The Department of Veterans Affairs, Network Contracting Office 19 intends to award a sole source contract with the authority of FAR Part 6.301-1, Only One Responsible Source to Stone Security, LLC 361 W Ironwood Drive, Salt Lake City, Utah 84115. This notice is for informational purposes only and is not a request for quotation. Responses to this notice must demonstrate clear and convincing evidence that competition would be advantageous to the Government, would not be cost or time prohibitive, and would not hinder the mission objective of the Department of Veterans Affairs, NCO 19. STATEMENT OF WORK (SOW) Stone Security Support Agreement Salt Lake City Utah Health Care System (SLCHCS) 500 Foothill Drive Salt Lake City, UT 84148-0001 The George E. Wahlen VA Medical Center, SLCHCS Biomedical Engineering Department has a requirement for a Full-Service maintenance contract for facility surveillance system and camera maintenance. Background The SLC HCS over the last 9 years has installed, utilized and expanded the VA Police Surveillance Video Camera System that now requires full maintenance support. We have one Biomedical Technician that is partially trained to support this system. The full maintenance support contract is required due to annual license costs, server and software updates and new camera make and model features that are driven by the software and require a license for each device. The system is installed on our entire Medical Campus that covers 75 acres of area and is installed inside our Hospital Bldg. 1, Bldg. 14, Bldg. 3 IPU (Inpatient Mental Health Unit, Bldg. 3-SRRTP (In-Patient Treatment), Bldg. 16 Outpatient Mental Health, Parking Garage, Bldg. 8, Bldg. 550, Bldg. 590 and the hallway corridors to include all outside areas of the campus. We had 219 digital cameras running on this system. Recently, Stone Security has installed 114 new cameras throughout the hospital per request of SLCHCS. Below is a list of the types of cameras added and the corresponding licenses. This request is for service of the 114 new cameras. This system records 24/7 and has the ability for the Police Department to identify patients, staff, automobiles and any other type of active crisis due to the different cameras and software installed on this system. This system has full recording and playback and is used in court cases when information is provided by the Chief of Police to the courts. The Police Department also relies on this system to spot active shooters and crimes being committed and crimes already committed for playback on campus. This system is the eyes of the VA Police Department. Milestone Software Updates to include all plug-ins. Network based surveillance servers (recording appliances) Network switches and infrastructure (Specific to the video surveillance system) Wireless radios (Specific to the video surveillance system) Includes camera replacement of up to 6 cameras per year. To include maintenance and, firmware updates. Server Preventative Maintenance (PM) which includes physically checking all video serves and cleaning as required. Maintenance to include bandwidth utilization and optimization (capacity required to receive video content). Non-Emergency support requests: 6 emergency support requests are included as part of this service agreement. (An emergency is defined as a service requested after regular business hours, holidays or weekends and will receive a no longer than 2-hour response time. The 7th and Subsequent emergency request will incur an additional charge at a rate of $85.00 per hour. Annually, the vendor will perform a proactive System Evaluation and Upgrade (SEU). The SEU will include upgrades to the latest version of the Milestone software and provide training to the responsible personnel on new features and functionality. Each SEU will also include testing of all surveillance hardware to include, servers, cameras, switches and other surveillance hardware to verify proper functionality. This option includes unlimited system training upon request. Description MILESTONE CARE PLUS The Milestone Software Upgrade Plan (CarePlus) annual licensing fees are also included for the current system licensing levels of 114 cameras. The Milestone CarePlus fees will need to be recalculated at the time this contact is executed to account for licenses that have been added since this document was generated. ANNUAL CAMERA AND SERVER REPLACEMENT PROGRAM This agreement includes replacement (new camera) installation, surveillance cabling and licensing (with one year of care plus included) of (5) fixed vies and (1) pan tilt zoom (PTZ) by Axis Communications. Camera models must match or exceed the specifications for the camera being replaced. One HP by BCDVIDEO recording server replacement which will meet or exceed the specifications of any of the recording servers currently being utilized by the customer will be available each year per this agreement. This agreement will be reviewed for renewal on an annual basis unless cancellation is submitted in writing by the client. Any changes to the support agreement will be approved by the customer signature Stone Security will support all equipment which is part of the surveillance network whether in it is out of warranty for troubleshooting and functionality purposes. Information Security Per VA Handbook 6500.6 Appendix B: Due to the threat of data breach, compromise or loss of information that resides on either VA-owned or contractor-owned systems, and to comply with Federal laws and regulations, VA has developed an Information and Information Technology Security clause to be used when VA sensitive information is accessed, used, stored, generated, transmitted, or exchanged by and between VA and a contractor, subcontractor or a third party in any format (e.g., paper, microfiche, electronic or magnetic portable media). In solicitations and contracts where VA Sensitive Information or Information Technology will be accessed or utilized, the CO shall insert the clause found at 852.273-75, Security Requirements for Unclassified Information Technology Resources. The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. Per VA Handbook 6500.6 Appendix C: Information made available to the contractor or subcontractor by VA for the performance or administration of this contract or information developed by the contractor/subcontractor in performance or administration of the contract shall be used only for those purposes and shall not be used in any other way without the prior written agreement of the VA. This clause expressly limits the contractor/subcontractor's rights to use data as described in Rights in Data - General, FAR 52.227-14(d) (1). (End of Page)
