Eulanda James; Email: Eulanda.James@va.gov DESCRIPTION: NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT TO Pure H20 Technologies, Inc STATEMENT OF WORK STERILE PROCESSING REVERSE OSMOSIS WATER TREATMENT SYSTEM The contractor shall provide preventative maintenance service for the iPure Reverse Osmosis (RO) water treatment system in Sterile Processing Service (SPS) located at the Manhattan VA Medical Center 423 East 23rd St, New York NY 10010 (VAMC-NY). Contractor shall provide all labor, materials, and equipment necessary to service the RO water system including all machines, equipment, and distribution piping. Period of Services: Base Year Plus Three Options. Service contract will be from Award date plus four (4) Option years. The initial contract year shall begin immediately following acceptance of the pumps from the general contractor who installed the system. All preventative maintenance work shall not void manufacturer s equipment warranty. MEMBRANE UNITS 1 month after start-up for the year: Inspect all system components; verify correct component operational cycles and performance within factory specification. Confirm inlet water chemistry and adjust equipment operating parameters as required. Replace cartridge filters and/or de-chlorination elements if required. Inspect system control components. Provide on-site retaining as required At 3, 6, and 9 month intervals Inspect all system components; verify correct component operational cycles and performance within factory specification. Confirm inlet water chemistry and adjust equipment operating parameters as required. Replace cartridge filters and/or de-chlorination elements if required. Check/refill all supply chemicals for system components. Clean and/or sanitize units. Inspect system control components. Calibrate critical system instruments (conductivity instruments, pressure switches, temperature switches ect..) and provide documentation of calibration. 12 month interval: 3,6, and 9 month interval maintenance functions listed above plus: Replace seals on all system pumps. Replace UV lamps in sterilization and/or TOC unit and clean quartz sleeves (if appropriate). Replace final filter cartridges (if appropriate). Calibrate critical system instruments (conductivity instruments, pressure switches, temperature switches ect..) and provide documentation of calibration. MEDIA, STORAGE & DISTRIBUTION UNITS 1 month after start-up for the year: Inspect all system components; verify correct component operational cycles and performance within factory specification. Optional service for critical systems: Calibrate critical system instruments (conductivity instruments, pressure switches, temperature switches ect..) and provide documentation of calibration. 12 month interval: 6 month interval maintenance functions listed above plus: Replace CO2 Removal Unit Media and/or Activated Carbon Media Replace seals on all system pumps. Replace UV lamps in sterilization and/or TOC unit and clean quartz sleeves (if appropriate). Replace final filter and/or storage tank vent cartridges (if appropriate). Clean and/or sanitize units (including distribution networks if appropriate). Calibrate critical system instruments (conductivity instruments, pressure switches, temperature switches ect..) and provide documentation of calibration. The system performance requirement is to provide finished water with a minimum specific resistivity of 0.1 megohm per centimeter. System shall provide water meeting the quality requirements of VHA Directive 1116(2) Sterile Processing Services and Association for the Advancement of Medical Instrumentation (AAMI) Technical Information Report (TIR) 34 Water for the Reprocessing of Medical Devices. Vendor shall provide an additional eight (8) emergency on-site service hours which may be used for any service functions required outside of the preventive maintenance services shown below. If repairs are required and requested during the preventative maintenance visit or for emergency, the vendor must notify the customer so that a purchase order or credit card number can be provided to cover such repairs excessive of the (8) emergency hours mentioned above prior to any work being performed. The vendor shall submit a detailed invoice when requesting payment for repairs, travel and materials provided outside the service contract. Vendor shall provide: Vendor shall provide a detailed service report within two (2) weeks after work is completed. Provide (3) copies of reports detailing all work performed at VAMC-NY. Warranty for parts provided by the vendor under the maintenance contract is one (1) year from the date of installation by vendor. Service is warranted for one ( 1) year from performance of service. Vendor will be liable for direct damages to a facility or property due to floods or leaks caused by or related to the services provided hereunder, provided that such damages are the result of the negligence or willful misconduct of the vendor, and are not the result of the negligence or willful misconduct of VA Personnel. All water systems must be protected from water hammer; have required safety relief valves, vacuum breakers and leak detectors. Floors and walls must be properly sealed and sloped to adequate floor drains. VA is responsible for meeting all Federal, state, and local (and foreign, to the extent applicable) laws, rules, and regulations regarding the monitoring of supplied equipment. Hours of coverage: All work must be done during weekends, on days when the Sterile Processing is not in operation. Preventive maintenance should be negotiated at least two weeks in advance with the Engineering Department. Preventive maintenance will be done at the manufacturer s recommended frequency or greater. Hardware and software upgrades will be scheduled and performed at no additional charge to the VA. These upgrades are those required for the correct function of the system; not those purchased to enhance function. The contractor will provide all hardware and software necessary for unscheduled repairs. The contractor s field service engineer will respond by phone within 2 hours and respond on site within 24 hours to correct the problem. Uptime requirements, excluding scheduled preventive maintenance, will be 97% of the normal working hours and working days during the term of the contract. All broken parts removed to repair the system will become the property of the contractor. The Contractor shall be responsible for all routine maintenance and sampling and testing to ensure acceptable quality of the treated water supplied by the Reverse Osmosis system. The Contractor is responsible for all scheduled maintenance in accordance with manufacturers schedule for maintenance. All replacement parts for normal wear and tear items shall be covered by this contract and provided and installed at no additional charge to the VA. The contractor must have trained personnel within a 90-mile radius of Manhattan VA Medical Center, New York, with an adequate spare parts inventory in some area office and proof of manufacturer s or equivalent maintenance training of field service personnel. Each scheduled and unscheduled visit will result in the production of a written report, by the contractor s representative, in a format negotiated between the VA-COR and the contractor. The contractor will always contact the Contracting Officer Representative (COR) Ulysses Kitovas, at 212-686-7500 Ext.7275 before and after arrival at the site.
