Teleflex Instruments Background. The Cleveland VAMC (CLE VAMC) is requesting multiple surgical instruments that are currently already in use by CLE VAMC. All instruments requested are listed above. Model numbers and vendors are given for cross reference purposes only. We request that all instruments procured are BRAND NAME OR EQUAL to TELEFLEX. These devices will be used during vascular surgical procedures. C. Scope. CLE VAMC is requesting multiple instruments for our Cardiac, Thoracic, and vascular surgery lines. This is due to an influx in physician staff across all service lines. Vendors will offer products based on the following requirements: All surgical instruments offered shall be made of surgical grade stainless-steel and titanium forged in Germany, Sweden, or the United States. Surgical Stainless-Steel shall be made from either Martensitic 420/2303, 440 Stainless-Steel, or stronger. Stainless-Steel shall follow: ASTM F1089-10 standard test method for corrosion of surgical instruments; ASTM F899-12B standard specification for wrought stainless steels for surgical instruments; and ASTM F1744-96 (2016) standard guide for care and handling of stainless-steel surgical instruments. All titanium implants must be made of surgical grade equal to TI-6AL-4V/Grade 5 or TI-6AL-4V ELI/Grade 23. All surgical instruments offered shall be assembled and finished in Germany, Sweden, or the United States. All surgical instruments offered shall include specific indications for use (IFU) that will describe the complete disassembly, cleaning, decontamination, assembly, and sterilization parameters. All items shall be able to be sterilized at a minimum of 273 degrees Fahrenheit for 5 minutes on a prevacum sterilizer. The surgical instrument shall be inspected under a digital microscope before the entire order is made. These instruments will be checked for: Pitting; Rust; Correct size of the instrument; Welds of the instruments; Approximation of jaws; Finishing process of jaws and overall instrument; Teeth within the jaws of the instrument. Upon delivery, surgical instrumentation will be inspected for quality by the Sterile Processing Service (SPS) Management. Any deficiencies will be communicated to the Contracting Officer. Pitting; Rust; Correct size of the instrument; Welds of the instruments; Approximation of jaws; Finishing process of jaws and overall instrument; Teeth within the jaws of the instrument. Instrument examination may include sterilization of each lot number of instruments. Each instrument will be processed in collaboration with the instrumentation IFU, VHA Directive 1116 (@), AAMI ST-79, sterilizer instructions, and Medline container is IFUS. Instruments will be soaked for a minimum of 5 minutes in neutral PH enzymatic or detergent and rinsed with reverse osmosis/deionized water; Instruments are then ultrasonic in neutral PH enzymatic or detergent for a minimum of 28 minutes; Instruments will then run through a mechanical washer for a minimum of 30 minutes; Instruments are assembled and then sterilized between 270-275 degrees for a minimum of 5 minutes with a 45-minute dry time. Physical and Dimensional Specifications. All Specifications must be exact match to above. This is extremely important due to the clinical need of these instruments. Training. Vendor shall provide in service and training to all SPS and Operating Room Staff for a two-week minimal period 1-2 weeks after delivery of product E. Performance Monitoring Packaging, Packing and Shipping Instructions. Instrumentation shall be delivered to SPS Room 122, Louis Stokes Cleveland VAMC, 10701 East Blvd, Cleveland, OH 44106. Inspection and Acceptance Criteria. All instrumentation will be inspected for damage prior to use. It will the responsibility of the vendor to replace damage instrumentation. Place of Performance. Offer Evaluation and Award. This is a competitive Request for Quotations (RFQ) that will be evaluated under FAR Part 12 and 13. The Government will issue a Firm-Fixed Price Purchase Order resulting from this solicitation to the responsible offeror whose offer conforms to the requirements within this solicitation and will be the most advantageous to the Government. The award will be made to the offeror that provides best value to the Government.   In addition to price comparison, the following factors will be evaluated: Past Performance (CPARS) Probable life of the item selected as compared with that of a comparable instrument such as Teleflex. Warranty Maintenance Availability Delivery Testing of instruments An Offeror s initial quote shall contain the best terms from a price and technical standpoint. Offerors are reminded that representatives from your company SHALL NOT contact any Cleveland VAMC personnel to discuss this RFQ during the solicitation and evaluation process. 5. Procedure for submitting documents to the Department of Veterans Affairs. DOCUMENTS THAT ARE TO BE PROVIDED TO THE CONTRACTING OFFICER WILL BE SUBMITTED USING THE APPROPRIATE VEHICLE BELOW. email: rachelle.hamer@va.gov 6. Submission of Offer. All information shall be submitted in the manner described below and offerors who do not submit all information may not be considered for award. Contractor shall provide an authorized distribution letter. The Contractor shall be an Original Equipment Manufacturer (OEM), OEM authorized dealer, authorized distributor, or authorized Contractor reseller for the proposed equipment/system such that the OEM warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. The Contractor shall provide new items ONLY; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Set-Aside Requirement: Only verified SDVOSB firms are eligible to submit an offer or receive an award of a VA contract that is set-aside for SDVOSBs or VOSBs in accordance with VAAR Part 819. A non-verified vendor that submits a bid or proposal should be rejected as non-responsive or technically unacceptable as listing in VIP is not self-correctable as a minor informality as, for example, a listing in SAM. All other purported SDVOSBs and VOSBs must apply for and receive verified status in accordance with 38 CFR Part 74 and be listed in VIP prior to submitting a bid or offer on an acquisition conducted in accordance with VAAR Part 819. The VIP database will be checked both upon receipt of an offer and prior to award. This requirement is Brand Name or Equal. This requirement will be awarded on all-or-none basis. F.O.B. shall be destination. ARO shall be 60 Days. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Quotes are to be broken out to include line item pricing for each item. All surgical instrument quotes shall be subject to technical review. All vendors shall provide the following documentation prior to technical review: -Document listing the requested items above, their equivalent model number, and size. -Specifications with a picture of each instrument. -Indications for use (IFU) that will describe the complete disassembly, cleaning, decontamination, assembly, and sterilization parameters.
