- Page 11 of 11 COMBINED SYNOPSIS/SOLICITATION EMPLOYEE FUNCTIONAL DEVELOPMENT TRAINING AT THE HUDSON VALLEY VA MEDICAL CENTER AND NORTHPORT VA MEDICAL CENTER, NEW YORK (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR12.6 and 13. Streamlined Procedures for Evaluation and Solicitation for Commercial Items and FAR 13. Simplified Acquisition Procedures for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; packages are requested, and a written pre-solicitation will not be issued. (ii) This solicitation is being issued as a request for quote (RFQ). The Request for Quote will be submitted through fedbizopps.gov on RFQ reference number 36C24219Q1210. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-05 (eff.8/13/2019) (iv) This procurement is being issued as a total small business set aside. The North American Industry Classification System (NAICS) code is 611710, Employee Functional Development Training, with a small business size standard of $15.0 Million. (v) The Contractor shall provide pricing to provide Employee Functional Development Training. See Attached table (It is not mandatory to use this table specifically, but it is encouraged. The contract specialist will need to distinguish between the varying costs of the procurement in order to make a determination on price reasonableness.) This Indefinite Delivery Indefinite Quantity requirement is for a base period of performance of one year and four (4) provisional option year periods of performance. The guaranteed minimum award amount for the resulting contract from this solicitation is for nine (9) students in the base period of performance. The Maximum aggregate of orders that may be placed against the resulting contract is fifty (50) students per period of performance. The Government does not guarantee that it will place any orders under the resulting contract in excess of the guaranteed minimum award amount. Any supplies and services to be furnished under this contract shall be ordered by issuance of delivery orders or task orders by the individuals or activities designated in the Schedule. Such orders may be issued from October 1, 2019 through September 30, 2020 for the base period of performance. The effective period for the resulting contract from this solicitation is the current period of performance as designated in said contract. Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor s and Government s rights and obligations with respect to that order to the same extent as if the order were completed during the contract s effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after September 30, 2019 for the base period of performance. Any orders made against any option period of performance shall not be required after September 30th of that period. In accordance with 52.212-2 award shall be made to the quoter whose quotation offers the lowest price technically acceptable (LPTA) to the government, considering technical capability, past performance, and price. Proposals will be determined by lowest price technically acceptable (LPTA). The government shall evaluate information based on the following evaluation criteria: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation represents the lowest price technically acceptable and providing appropriate past performance. The following factors shall be used to evaluate offers: Evaluation Factors for Employee Functional Development Training technical capability to meet all of the requirements of the SOW, please provide a maximum 10-page capability statement. Past performance, please provide three references for contract of similar scope and magnitude to be evaluated, the contractors Past Performance shall also be reviewed in CPARs Price, the government will evaluate offers for total price. Except when it is determined in accordance to FAR 17.206(b) not to be in the Government s best interest, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. This requirement includes options executed under FAR 52.217-8. The government will evaluate prices for the option under FAR 52.217-8 by using the last year s option prices to calculate the price for six months of efforts and adding that amount to the base and other option years to arrive at the total. Evaluation of options will not obligate the Government to exercise the options. The government intends to award an indefinite delivery/indefinite quantity type contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" The offeror shall list exception(s) and rationale for the exception(s). Quotes received without this statement will be assumed to infer that terms and conditions are acceptable without modification, deletion or addition. Submission shall be received not later than 2:00 PM EST, Wednesday September 18, 2019.
