- Attachment 9 SCLS Wage Determination, State of Tennessee Attachment 9 Map_Betty Cherry Attachment 7 Map_Don Childress II 5447411601L7V Attachment 6 Map_Don Childress II 5447411601L7Q Attachment 5 Map_Friends Farming Attachment 4 Map_Koko Farms Attachment 3 Map, Jolley Springs Attachment 2 Tree Planting_Construction Specifications 409 Attachment 1 Site Preparation and Tree Planting Price Schedule, Terms and Conditions, Clauses, and Provisions Standard Form 1449 USDA NRCS Tennessee State Office Easement Program Division has a requirement for pre and post site preparation and tree planting on Wetland Restoration Easement (WRE) sites located in the following counties; Obion, Dyer, and Haywood. This procurement is a TOTAL SMALL BUSINESS SET-ASIDE ACQUISITION and will be released under Solicitation Number 12FPC119Q0052. The small business size standard is based on the North American Industry Classification System (NAICS) Code 115112 Soil Preparation, Planting, and Cultivating with a size standard of $7.5 million average annual income. PROJECT DESCRIPTION. The work will consist of site preparation, furnishing, and planting all trees of the kinds and sizes specified, at the location shows on the drawing and as required in the specifications. (See Specifications for more details). PROJECT DURATION. 183 calendar days after the Notice to Proceed (NTP) is issued. Contract approximate Period of Performance September 2019 through April 2020. SUMMARY. Contractor shall furnish all supervision, labor, materials, tools, equipment, transportation, supplies and travel necessary to provide the required services to improve restoration practice of this conservation easement in accordance with NRCS specifications, and terms and conditions of the contract, and other attachments provided. WORK TO BE PERFORMED BY PRIME CONTRACTOR. The contractor shall perform, with its own organization, work equivalent to at least 50 Percent of the value for the work of the contract. SITE VISIT. One site showing will be held, September 17, 2019, 8:45 am Central, at the USDA NRCS Dyersburg Field Office located at 400 Community Road, Dyersburg, Tennessee 38204. The purpose of this site showing will be to answer questions regarding, or requests for clarification of the requirements and solicitation documents. Although questions during the site visit are permissible, statements or representations made during the site visits are not legally binding. Changes resulting from the site visits are official only if issued through an Amendment to the Solicitation. ADDITIONAL INSTRUCTIONS - PREPARATION OF QUOTES In order to receive complete consideration for award of a contract, prospective Quoters are requested to submit quotes consisting of the following: 1. Completed STANDARD FORM (SF) 1449, sign and date 2. Completed PRICING SCHEDULE (page 4) 3. Complete all other representations and certifications required by Section K and not already completed in the System for Award Management (SAM), page 20, 4. Provide evidence of registration in SAM and that status is "Active". 5. Complete 52.209-5 Certification Regarding Responsibility Matters. (OCT 2015), page 22, 6. Complete 52.209-11 Representation by Corporations Regarding Delinquent Tax Liability or a Felony Conviction under any Federal Law. (FEB 2016), page 24. Failure to provide any of the documents required above will result in a non-responsive quote.
