This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This brand name or equal, Request for Quote (RFQ) solicitation is issued i/a/w FAR Part 13 and FAR Part 12. Award will be presented to the lowest priced technically acceptable offer i/a/w: FAR Part 13. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-06. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510, with a small business size standard of 1,000 employees. The Great Lakes Acquisition Center, 115 S. 84th Street Milwaukee, WI 53214-1476 is seeking to purchase Modular Cooler Heater Ice System. This requirement is a Brand name or equal request. All interested companies shall provide quotations for the following: Line Item Description Quantity Unit Price Total Price MCH-1000 (i) MODULAR COOLER HEATER ICE SYSTEM 120 vac/60hz Includes: 1 each of the following: Model 1000 (i) base unit with 9-gallon ice tank. Remote control with clamp Single wheel set Standard tubing set with connectors Operator s manual 2 MCH-10RMS REFRIGERATION MODULE 1000 (i) 2 MCH-10CSC CARDIOPLEGIA SHARED CHANNEL (CSC) MODULE 1000 (i) 2 Total Amount: Statement of Work Background: The Madison VA Hospital, Operating Room perfusionist are requesting two new cooler heaters. This equipment will be used during coronary artery bypass surgeries. Scope: Vendor shall provide and install two MCH-1000(i) (Modular Cooler Heater standard size heater cooler), two MCH-10RMS (Refrigeration Module which allows to build an ice block in the heater cooler) and two MCH-10CSC (Cardioplegia Shared Channel Module which allows the perfusionist to use only one system to do both heat and cool the patient and cardioplegia without this they may need to use two heater coolers) at the William S. Middleton Memorial Veterans Hospital. This machine is designed to warm and cool patients during coronary artery bypass surgeries. The vendor will offer application training on-site and to be completed within 60 days of purchase. Specific Tasks: Vendor will uncrate and assemble/install all components of the machine. The vendor will dispose of any packaging material used in shipping. The vendor will provide on-site training to clinical staff and to completed within 60 days of purchase. Full parts and labor warranty for 1 year, with the option to purchase an additional four years of service. Delivery Delivery shall be provided no later than 60 days after receipt of order/award of contract. The contractor shall deliver line item(s) 001 thru 003 to William S. Middleton Memorial Veterans Hospital 2500 Overlook Terrance Madison, WI 53705-2286 Attn. Warehouse. Award shall be made to the quoter whose quotation offers the best value to the government. The government will evaluate information based on the following evaluation criteria: Lowest Price Technically Acceptable The full text of FAR provisions or clauses may be accessed electronically at http://acquisition.gov/comp/far/index.html and http://www.va.gov/oamm/oa/ars/policyreg/vaar/index.cfm The following RFQ provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items (Oct 2018) ADDENDUM TO FAR 52.212-1 Instructions to Offerors-Commercial Items Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The following provisions are incorporated into 52.212-1 as an addendum to this solicitation: Gray Market Prevention Language: (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items. FAR 52.212-2 Evaluation Commercial Items As prescribed in 12.301(c), the Contracting Officer may insert a provision substantially as follows: Evaluation Commercial Items (Oct 2014) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: To be considered Technically acceptable Vendor: MCH-1000(i) is the standard sized heater cooler. MCH-10RMS is the refrigeration module that allows them to build ice block in the heater cooler. MCH-10CSC is the cardioplegia shared channel. It allows the perfusionist to use only one system to do both heat and cool the patient and cardioplegia. Without this, it will require using 2 heater coolers. End of Provisions and Clauses Section This is an open-market combined synopsis/solicitation for products as defined herein.    The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoter shall list exception(s) and rationale for the exception(s).
