This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation 36C24719Q0976 is issued as a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99 06-15-2018 & 07/15/2018. The Department of Veterans Affairs intends to award a Firm -Fixed Price Contract for services to Retrofit (5) Cold Chambers in the Atlanta VA Research Department. The North American Industry Classification System (NAICS) Code and size standard are 333415 and 1,250 employees respectively. All offerors must be registered in the System for Award Management (SAM) at http://www.sam.gov at the time an offer is submitted. All SAM records must be active, complete, and not missing elements such as representations and certifications. PLACE OF PERFORMANCE Atlanta VA Medical Center 1670 Clairmont Road Decatur Ga. 30033 PRICE/COST SCHEDULE (See Attachment B) BASIS OF AWARD 52.212-2 Evaluation Commercial Items (Oct 2014), the Government will award a firm fixed price contract, resulting from this solicitation to the responsible Offeror whose quote represents the lowest priced, technically acceptable offer. The following factors shall be used to evaluate offers, (i) technical capability (technical proposal) to meet the Government requirement; (ii) past performance; and (iii) price. Nonprice factors will be rated on an Acceptable/Unacceptable basis. To be eligible for award, an offer must be rated acceptable in the non-price factors. For price evaluation purposes, the lowest price technically acceptable offer will be selected for award. The offeror s technical proposal, and experience shall be evaluated to determine if Offeror understands the requirement and has the capabilities to provide Cold Chamber Retrofit Service as detailed in the Statement of Work (SOW). To be considered technically acceptable for award each offeror must provide with their submitted quote/offer: Technical Capability /Technical Proposal: Offerors will describe and provide information on their company s ability to provide the requested service. The offeror shall provide a technical proposal (submittal of the approach, methods, procedures, and resources) it will use to complete the requested services. The proposal must include a detailed work plan for completing the required service to include the task/subtasks associated with the deliverables using the template in Attachment A.1. Documentation of all required license must be included with the technical proposal. The technical proposal must provide evidence that the offeror understands the nature of the required work and has the capability and resources to complete the project. Past Performance: Offerors must provide a minimum of (2) Past Performance Reference Surveys for work successfully completed that is similar in nature, scope, and complexity of this requirement. Offerors must request that their References complete and submit Attachment E - Past Performance Survey on the Offeror s behalf. Surveys completed and submitted by the Offerors themselves will not be considered valid Past Performance References. Past Performance Surveys must be completed by the Company serving as the Contractor s reference, then emailed by that company to Lasonja.harvey@va.gov no later than the deadline for all offers. Past performance surveys will be verified by Contracting to ensure offer can perform the required services. The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources. Price: Offeror shall complete the Price/Cost Schedule (See attachment), with their proposed contract line item numbers (CLIN) and price inserted in appropriate spaces. The Government will evaluate offers for award purposes by adding the total of all CLIN prices to ensure that pricing is correct. Price will be evaluated for fair and reasonableness in accordance with FAR 12.209. Pricing should be considered FOB Destination and must be good for 90 calendar days after the solicitation closes. (End of Evaluation Criteria) DEADLINES: This announcement constitutes the only solicitation and interested parties are solely responsible for monitoring this solicitation and any amendments thereto. All Quotes/Offers and required documentation must be emailed to Lasonja Harvey, at Lasonja.harvey@va.gov by 1:00 p.m. EDT on September 19, 2019. No exceptions will be provided to allow additional time to submit offers and required documentation.
