- Modification 001 to update the requirement and extend end date. Modification 001: To clarify like or similar equipment, and to clarify that bids must be for new equipment. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Portland Area Indian Health Service, Yakama Service Unit, 401 Buster Road, Toppenish, WA 98948 has a need to procure Optometry equipment. The combined synopsis/solicitation number is 2019-063-1050EQ and is issued as a request for solicitation (RFQ). The combined synopsis/solicitation RFQ and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-04 and 2019-05, dated August 13, 2019. The associated NAICS code is 333314 with small business size standard of 500 employees. Contractor shall provide Zeiss HFA3 or equal with trade-in for existing HFA. The description for requirements is attached. Date required is 60 days after receipt of order and place of delivery is Yakama Indian Health Center, 401 Buster Road, Toppenish, WA 98948. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Addendum to FAR 52.212-1, Instructions to Offerors - Commercial Items: (a) Offerors are to e-mail complete quotes to prudence.yellowowl@ihs.gov and stacy.hill@ihs.gov no later than 5:00p.m. PDT, September 9, 2019. All provisions and clauses applicable to this solicitation can be viewed in full text using the following web address: http://www.acquisition.gov/far/index.html (b) New equipment only. No remanufactured for grey market or refurbished items. The provision at 52.212-2, Evaluation - Commercial Items, applies to this solicitation. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers in descending order of importance: Meeting the technical requirements and All or None. A best value determination will be made utilizing the lowers price technically acceptable that addresses each line item of the requirement. The clause at 52.213-4, Terms and Conditions, Simplified Acquisitions (Other than Commercial Items) applies to this procurement.
