- SOW This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is 75N94019Q00085 and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-99. The North American Industry Classification (NAICS) Code is 541380 and the business size standard is $15.00. However, this solicitation is not set aside for small business. This acquisition is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) intends to procure on a sole source basis with MAYO COLLABORATIVE SERVICES LLC, 3050 SUPERIOR DRIVE NW, ROCHESTER, MN 55901-1995 for the acquisition of a Standing Order for Laboratory Testing Services for medical diagnostic testing of a variety of biological specimens. In order to provide state-of the art medical care to patients enrolled in numerous intramural research protocols, the Eunice Kennedy Shriver National Institutes of Child Health and Human Development (NICHD), Division of Intramural Program (DIR), Clinical Investigators require outsourced medical diagnostic testing of a variety of biological specimens. With the longitudinal protocols of the intramural research program of NICHD it is essential that the quality of specimens and data be assured. These tests must be performed in a Clinical Laboratory Improvement Act approved setting with testing verified by laboratory professionals' Board (certified by American Board of Genetics or other pertinent Board). In addition, because of the complexity of the tests, the laboratory supervisors must be accessible to the clinicians ordering the tests for academic discussion of the results. The period of performance is as follows: PERIOD OF PERFORMANCE BASE YEAR: 9/15/2019 - 9/14/2020 OPTION YEAR 1: 9/15/2020 - 9/14/2021 OPTION YEAR 2: 9/15/2021 - 9/14/2022 OPTION YEAR 3: 9/15/2022 - 9/14/2023 OPTION YEAR 4: 9/15/2023 - 9/14/2024 This acquisition is conducted under the authority of 41 U.S.C. 253(c) under provisions of the statutory authority of FAR Subpart 6.302- FAR 6.302-1-Only one responsible source and no other supplies or services will satisfy agency requirements 41 U.S.C. 253(c)(1). The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-1 Instructions to Offerors - Commercial Items; FAR Clause 52.212-2, Evaluation - Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation for Simplified Acquisitions applies to this acquisition. FAR Clause 52.204-24, Representation Regarding Certain Telecommunication and Video Surveillance Services or Equipment; FAR Clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment; FAR Clause 52.213-4 to reflect the changes in FAC 2019-05 and FAR Clause 52.244-6 to reflect the changes in FAC 2019-05. The offeror must include their Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size.
