It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract.   Requests for a solicitation will not receive a response.   Responses to this sources sought must be in writing.   The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below.   Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition.   Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and If respondent is VOSB or SDVOSB, respondent is to provide proof of www.vetbiz.gov certification. In addition, SDVOSB/VOSB Contractors are asked to acknowledge that they understand the limitations on sub-contracting pursuant to FAR 52.219-14, Limitations on Sub-Contracting, which will be included in the impending solicitation. Furthermore SDVOSB/VOSB contractors are asked to respond that they are capable of providing the requested services keeping within the parameters of this clause. If an Offeror proposes to use any subcontractors, joint ventures, or teaming arrangements the Offeror shall include the percentage and type of work the subcontractor will accomplish to include their certifications. (Any subcontractors, joint ventures, or teaming partners shall have the required certifications as described in the sources sought notice) (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below and (j) provide sample pricing of estimated 40 visits per month in accordance with requirements outlined in the statement of work.   The Network Contract Office (NCO) 22 Gilbert is seeking a potential qualified contractor for: Phoenix, Arizona VA Health Care System (Phoenix VAHCS) located at 650 E Indian School Rd, Az, 85012 SERVICE: Requirement: The Phoenix Veterans Administration Health Care Services hereafter PVAHCS is requesting services for vendor to visit select homes of veterans with the highest Care Assessment Needs. Vendor is to provide in home health and safety assessments as directed by PVAHCS staff. Vendor will have an iPad assigned to them from PVAHCS. iPad will have a VA VMR (Virtual Medical Room) link to access the secure synchronous telehealth visit. Well Check Visit Post-Acute: The purpose of the wellness check is for PVAHCS to preventative medical services for veterans with an elevated level of health care needs. These appointments will, as needed, include: Review and update medical and family history Review and update current medical providers Measure and asses vitals Asses for cognitive impairment Review potential psychological risk factors Review functional ability and safety Inventory medications and asses daily self-care PVAHCS will independently review responses to well check visit/ Health Risk assessment All lab draws will occur at the VA CBOC (Community Based Outpatient Clinic) closet to the Veterans home. Veteran is responsible for traveling to closest CBOC to get blood draw. Vendor Tasks: Vendor will respond to PVAHCS scheduled appointments with Program Participants to provide low acuity Patient Advocate Services, as needed, between Tuesdays - Fridays during the hours of 08:00 AM to 4:30PM. Appointments are expected to be 1-2 hours in length. Prior to the appointment Veteran will prepare by rounding up all of their medications in their home. Vendor is the owner of their medical equipment. Equipment will be used to provide basic physical examination for health and safety assessments as required. Vendor will provide physical presence of staff at Veterans homes. Vendor is expected to provide equipment to measure vitals as directed by PVAHCS. Vendor staff will follow the directions of (PVAHCS) clinicians with regards to physical exam and will report to PVAHCS clinicians findings. The gathering of vitals can be done prior to the link with the VA clinician. Physical examination may include but is not limited to: Brief history of patient health condition Up to 6 vital signs: Body Temperature, Heart Rate/ Pulse, Respiratory Rate/Oxygen Saturation, Blood Pressure, Pain Assessment using Wong-Baker Faces Pain Scale, and Mental Health (thoughts of harm/suicide). General Appearance Heart/ Lung exam Head/ Neck exam Neurological exam Dermatological Exam Extremities Exam Collection of samples for testing or handling of biohazardous material is NOT required. Vendor will at discretion of PVAHCS clinicians conduct inventory of readily available medications in veterans home (a real-time medication reconciliation). Vendor will report to PVAHCS clinicians a visual assessment of home safety finds and/or fall risks to include displaying risks via live synchronous video call using tablet equipment through the VMR. This will allow objectification of PVAHCS clinicians to determine Occupational Therapy consultation needs for Veterans. PVAHCS staff will determine follow on care for Veteran Participant at closing of visit. PVAHCS staff will make arrangement of transportation of Veteran Participant to PVAHCS if needed. If the visit is determined to be an emergency, vendor will call ambulance for rapid transport, remaining with veteran until ambulance arrives. Certifications Vendor staff will, at a minimum, possess a First Responder Certification or be a Registered Nurse within state of Arizona. Place of Performance Unless otherwise mutually agreed to by Vendor, PVAHCS and Veteran Participant, services will be performed at the residences of PVAHCS patients. TRAVEL: Vendor is responsible for all travel-related costs associated with this contract. Veteran homes will be located within Tempe, Az or Chandler, Az municipalities. The vehicle used is at the discretion of the vendor. DELIVERABLES: Live synchronous video visit with associated in-home safety evaluations as noted. Type of Contract Firm Fixed Price PVAHCS will be charged a flat rate for each scheduled visit. 20 Visits per month anticipated in the Tempe, AZ Municipality 20 Visits per month anticipated in the Chandler, AZ Municipality SUBCONTRACTING Subcontracting of this contract is prohibited. All work is to be performed by the Vendor. Performance Period The period of performance will be from 10/01/2019-09/30-2020 with four option years to extend services. GOVERNMENT FURNISHED EQUIPMENT: One single PVAHCS issued iPad for each vendor contracted. iPad shall have high speed internet service for connection with PVAHCS clinicians. iPad shall be provided by the Telehealth Department. No other equipment will be furnished. Vendor will sign for iPad device from the Telehealth Department. Security for the connection is through the VA operated Virtual Medical Room. CONFIDENTIALITY: Videoconferencing with the VA care team and the vendor will be conducted over the internet, but through the VMR secure link only. The videoconference will not be recorded. HIPPA Compliance: PVAHCS must ensure compliance with the HIPAA Privacy Rule when using or disclosing Patient Health Information (PHI) to VENDOR on behalf of PVAHCS Patients. The HIPAA Privacy Rule expressly permits covered entities or a business associate, on behalf of a covered entity, to use or disclose PHI for Treatment or health care operation purposes. Therefore, PVAHCS can identify PVAHCS Patients (or patients to whom it provides care management services) who may benefit from the Program and contact the patient to see if they are interested in participating in the Program. Prior to disclosing such PHI to VENDOR, PVAHCS will obtain a HIPAA patient authorization, unless a mutual Treatment relationship between VENDOR and PVAHCS Patients already exists; provided, however, VENDOR will be responsible to obtain each patient s consent to participate in the Program including an executed HIPAA patient authorization before any further PHI is disclosed in connection with the Program. The parties acknowledge that the VENDOR will have access to PHI of PVAHCS Patients that is confidential. VENDOR agrees that, except as specifically permitted under this Agreement, it will not at any time disclose such information to others unless expressly permitted by HIPAA and/or applicable state law, pursuant to a business associate agreement, if applicable, or pursuant to a valid patient authorization. 6500.6/Appendix A/Item 7 - The service involves the storage, generating, transmitting, or exchanging of VA sensitive and/or patient/information. Contractor must adhere to the provisions of Public Law 104-191, Health Insurance Portability and Accountability Act (HIPAA) of 1996 and the National Standards to Protect the Privacy and Security of Protected Health Information (PHI).   As required by HIPAA, the Department of Health and Human Services (HHS) has promulgated rules governing the security and use and disclosure of protected health information by covered entities, including the Department of Veterans Affairs (VA). The Contractor shall prove to the Phoenix VAHCS that they are identified as healthcare providers and that the PHI is being disclosed and/or used for treatment. Therefore, no BAA is required for Contractor Services. Privacy & Information Security Training: Contractor and their sub-contractors assigned work under the contract are required to receive annual training on patient privacy as established by HIPAA statues. Training must meet VHA s and/or the Department of Health and Human Services Standards for Privacy of Individually-identifiable health information. Privacy and HIPPA training required to be equivalent to VA Privacy and Information Security Awareness and Rules of Behavior (TMS 10176), which COTR must monitor and report to least yearly. Contractor shall provide certification to the VA upon request that all employees and sub-contractor employees assigned work and/or having access to Protected Health Information have received annual training.   6500.6/Appendix A/Item 6 Service does not require connection to a VA Network. Therefore, Certification and Accreditation requirements do not apply and a Security Accreditation Package is not required. http://www.va.gov/vapubs/viewPublication.asp?Pub_ID=471&FType=2 Important information:   The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this Sources Sought Synopsis request.   This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach.   The Government will in no way be bound to this information if any solicitation is issued.   The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Business set aside. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined.   No sub-contracting opportunity is anticipated.   The North American Classification System (NAICS) code for this acquisition is 811219 (20.5 million). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA).   Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at www.vip.vetbiz.gov.   All interested Offerors should submit information by e-mail to April.Graves@va.gov include the solicitation in number in the subject line of email. All information submissions shall be received no later than 3:00 pm (MST-Arizona) on September 29, 2019.   After review of the responses to this announcement, the Government intends to proceed with the acquisition and a subsequent solicitation may be published.
