Description
DRAFT Statement of Work Vendor Feedback Form THIS IS A REQUEST FOR INFORMATION (RFI) ONLY - THIS IS NOT A SOLICITATION FOR QUOTES. This RFI is being issued in accordance with Federal Acquisition Regulation Part 10, Market Research. It is not to be construed as a commitment by the Government to issue a solicitation or ultimately award a contract. The U.S. Food and Drug Administration's (FDA), Office of Acquisitions and Grants Services (OAGS), is conducting market research for Access to Hospital Drug Utilization Data. The purpose of this requirement is to provide the FDA with access to an existing commercially available data resource for 1) adult and pediatric hospital drug utilization data and 2) pediatric drug utilization data in short-stay and/or free-standing pediatric hospitals. The data shall provide FDA personnel with the ability to use the data to describe national patterns of drug utilization and treatment procedures. In addition, the Contractor shall provide technical and analytical support for custom reporting and drug utilization studies. The purpose of this RFI is to identify small businesses that are capable of providing the full range of services described in this RFI and the draft statement of work (SOW). Responses to this Request for Information (RFI) will be utilized for a potential requirement for an anticipated contract. The anticipated period of performance for this contract is 5 years (base year plus four option years). The Contractor shall provide their capability to support all task areas described in the draft Statement of Work (SOW). Any businesses with the availability and capability to meet the requirements in the attached draft Statement of Work (SOW) shall provide a response to this notice with the following information: - Cover Page that includes: >Company Name >Company Address >DUNS number >Socio-economic status (i.e. small, VOSB, women-owned, HUBZone, ANC etc.) >Point of Contact (Name, email and phone number) - Completed Vendor Feedback Form - Capability Statement showing their ability and willingness to complete this requirement. - CPARS evaluations for contracts identified in the Vendor Feedback Form - Capability to meet the requirements of FAR 52.219-14 Limitations on Subcontracting Responses shall be submitted by the Response Date via email to Robin.Goon@fda.hhs.gov. Late submissions may not be considered. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted.
