Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation document will not be issued. The Government intends to issue a purchase order in accordance with FAR Part 13 for using Simplified Acquisition Procedures. The solicitation number is 36C24719Q1146. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-70 effective September 30, 2013. The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 and Product Service Code (PSC) 7110. The evaluation of offers received in response to the solicitation will use a tiered or cascading order of precedence. Tiered evaluation of offers, also known as cascading evaluation of offers, is a procedure used in negotiated acquisitions when market research is inconclusive for justifying limiting competition to small business concerns or sub-categories of small business concerns. The contracting officer (1) Solicits offers from both small and other than small business concerns that will be evaluated in the following tier order: (a) service-disabled veteran-owned small business (SDVOSB); MUST BE VET BIZ CERTIFIED (b) veteran-owned small business (VOSB); MUST BE VET BIZ CERTIFIED (c) All other small business concerns; (d) Other than small business. (2) If an award or a sufficient number of awards cannot be made at the first tier, evaluation of offers will proceed at the next lower tier until an award or a sufficient number of awards can be made. The acquisition will be made pursuant to the authority in FAR 13 to use simplified procedures for commercial items. The Department of Veterans Affairs, Birmingham VA Medical Center has a requirement for the following: 1 Dental Mobile Rolling Cart Customized With Seven (7) drawers. The Solid Surface Top is Lockable and Slides from Front to Back The bottom drawers lock by a latch. Carts are made of real plywood. No particle board Laminate # 7933K-OF Cafelle Solid Surface Top # 9070ML Artic Melange 25 EA Rolling Dental Cart Minimum Technical Requirements This requirement includes healthcare grade rolling treatment carts to meet the BVAMC Dental Clinic needs. These needs may require stock pieces to be somewhat customized to address client safety and infection control aspects. This requirement is for product and freight only. It does not include installation. Salient Characteristics Rolling treatment carts to include minimum of the following specifications: Dimensions: 21.5 Wide x 18.75 Deep x 32 High Have 4 locking casters and be mobile. Have seven (7) drawers that can be locked by a security protection latch The bottom drawer shall lock by a security protection latch.   Cabinets constructed of HPL or thermoform over a ½ minimal thickness low VOC substrate with all edges sealed with matching edge-band or thermo-formed radius edges. (No particle board) Laminate shall be Wilsonart # 7933K 07 Cafelle Work top surface shall be Solid Surface Solid Surface top shall slide from front to back and be lockable Solid surface top shall be Wilsonart # 9070ML Artic Melange Drawers extend fully without wobbling or failing, increasing the efficiency of stocking and restocking Components to have rounded exposed surfaces, free from sharp edges to prevent injury to patients, visitors, and staff. Cabinet interiors fully melamine or LPL lined with no exposed substrate. Door pull options that meet ADA Accessibility Guidelines Surfaces must be cleanable with hospital grade germicides and bleach solutions without harm to finishes. Modular system components shall be universal, and the drawers and accessories are interchangeable. Components to have rounded exposed surfaces, free from sharp edges to prevent injury to patients, visitors, and staff. Modular transport carts shall take into account infection control requirements and be fabricated with no exterior cracks, crevices, joints, corners, or angles that may facilitate bacteria. Carts shall be easily moved by one person even when fully loaded. Pull handles are included with double wide carts. Carts shall be made of uniform density, integral, highly durable, impact-resistant plastics and are resistant to rust. modular transport carts are lockable and can be keyed alike. Components shall be capable of being assembled by simple hand action without tools, except for those components fastening to architectural elements or modular counter surfaces. These shall be secured with the use of simple hand tools. (i.e. Allen wrench, screwdriver) Storage modules shall have integrally molded drawer/shelf guides at nominal three-inch vertical increments on both sides of the module. Drawers extend fully without wobbling or failing, increasing the efficiency of stocking and restocking Drawer should have an integrally molded drawer glide to function as a handle. Drawers shall have the capability to accept Snap-On label clips for clinical material designation Drawer glides to have built-in stops to prevent accidental removal. Sub containers and dividers available for storing small loose or irregularly shaped items. Tops are sturdy, built as part of the cart- not a slip-on addition. Casters on mobile storage carts to be a minimum of 4, all 4 casters swivel and 2 front locking casters. A minimum of 12 years, 24/7 shift warranty. See customized specifications Offers for the items described above are due by 9:00AM CST on September 19,2019 to Lynette.Lane2@va.gov and must include solicitation number, FOB destination to the BVAMC, proposed delivery schedule, discount/payment terms, warranty duration, and DUNS number. Offerors shall provide the information required by FAR 52.212-1 (APR 2014), Instructions to Offerors-Commercial Items, which is incorporated by reference. If the end product (s) is other than domestic end product(s) as defined in the clause entitled Buy American Act Supplies, the offeror shall so state and shall list the country of origin. FAR 52.212-2 (OCT 2014) Evaluation Commercial Items- The following factors shall be used to evaluate offers: Technical Capability of the item offered to meet the Government requirement Price Service and maintenance agreement FAR 52.212-4 (DEC 2014), Contract Terms and Conditions-Commercial Items is applicable. FAR 52.212-5 (MAR 2015), Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items is applicable and the following identified clauses are incorporated by reference: 52.233-2, 52.203-13, 52.203-17, 52.204-9, 52.228-5, 852.211-73, 852..219-10, 852.237-70, 852.246-70, 852.246-71, 52.211-16, 52.216-1, 52.222-41, 852.270-1, and 852.273-74. Offerors must include completed copies of the provision at 52.212-3 (MAR 2015), Offeror Representations and Certifications - Commercial Items with their offer.
