Description
NOTICE OF INTENT TO SOLE SOURCE REQUIREMENT The Department of Veterans Affairs, VA Healthcare System, VISN 10, Contracting Activity intends to negotiate a sole source contract under FAR Part 13.5, and the authority of FAR Part 6.302-1, Only One Responsible Source for the deliverables outlined below. The vendor will be Materialise LLC. The associated NAICS code is 541511. The anticipated award date is 9/25/2019. A firm fixed price contract for one year shall be awarded. It is the Government's belief that only the proposed candidate possesses the required capabilities to successfully meet this requirement. It is the Government's intention to solicit and negotiate with only this source. This notice of intent is not a request for competitive quotes or proposals. It is not a solicitation or request for offers. No solicitation package is available. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. Responses shall include: (1) Business Name and Address, (2) FSS Contract Number, (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size (6) Type of Business SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUBZone, WOMEN-OWNED etc. Responses shall be submitted to Jordan Carlsen, Contract Specialist, no-later-than Thursday, September 19th, 2019 at 3:00pm EST. The Contractor shall provide the deliverables outlined within the Statement of Work below to the VAMC Indianapolis 1481 West 10th Street Indianapolis, IN 46202. SEE THE STATEMENT OF WORK BELOW FOR SPECIFICATIONS OF THIS REQUIRMENT STATEMENT OF WORK for OrthoView Enterprise Licenses I. SCOPE OF WORK: Contractor shall provide associated scaling objects, software installation, clinical applications training, as well as software support for all associated installations for the OrthoView Enterprise Licenses. The OrthoView system will include pre-operative planning software and the associated scaling objects that assist in plan accuracy. In addition, the system will provide pre-operative planning for orthopaedic applications and will aid clinicians in the designing of digital templates to improve the Veteran experience. This work will include providing the following facilities with OrthoView s pre-operative planning software with the specified concurrent licenses for use across the facilities within VISN 10 below: VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105 John D. Dingell VA Medical Center (Detroit), 4646 John R. Street, Detroit, MI 48201 Richard L. Roudebush VA Medical Center (Indianapolis), 1481 W 10th Street, Indianapolis, IN 46202 Dayton VA Medical Center, 4100 W. Third Street, Dayton, OH 45428 Cincinnati VA Medical Center, 3200 Vine Street, Cincinnati, OH 45220 Software support plan coverage shall include: 24/7 Telephone Support 24/7 Accesses to IT product specialists Remote Diagnostics Support and Installation Safety Updates Annual Performance Enhancing Updates Annual Performance Enhancing Updates to Hardware Equipment Annual Software Support and Maintenance The below table highlights the equipment and services that should be included: Description Quantity OrthoView ver. 6 Enterprise License Agreement 1 Remote Installation and Project Management 1 II. HOURS OF SUPPORT SERVICES: Hours of telephone support services are defined as 24 hours a day/7 days a week. Coverage shall include support for server installation and support for data transfer. Hours of Accesses to IT Product Specialists are defined as 24 hours a day/7 days a week. Coverage shall include enhanced telephone support for the corrective maintenance of IT Equipment covered by the agreement. Safety Update Services are defined as 7 days a week from 8:30 AM to 5:00 PM EST. Coverage shall include any travel, accommodation, labor, and software required to complete the safety update. III. Support Services: Contractor shall provide any travel, accommodation, labor, and software required to complete installation as well as support for clinical applications training at the customer s site, 7 days a week from 8:30 AM to 5:00 PM EST. The contractor shall also provide any travel, accommodation, labor, and software required to complete performance enhancing updates to hardware equipment at the customer s site, 7 days a week from 8:30 AM to 5:00 PM EST. Contractor shall provide the Contracting Officer Representative (COR) and/or a Biomedical Engineering Representative a legible field service report immediately following the service visit. The report shall include detailed information regarding all services provided, software/hardware changes made, field service technician contact information, and dates and times of services rendered. IV. COMPETENCY OF PERSONNEL SERVICING EQUIPMENT: Contractor shall assure fully a qualified staff is available to provide support services described in the statement of work. Upon request by the COR, the contractor shall provide written assurance of the competency of their personnel on the specific systems covered in this agreement. V. SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the COR may determine to be reasonably necessary to protect the lives and health of occupants of the building. The COR shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the contractor or their representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the contractor fails or refuses to comply promptly, the Contracting Officer (CO) may issue an order stopping all or part of the work, and hold the contractor in default.
