Description
Sources Sought Notice for Ambulance Service at Columbus VAACC The purpose of this sources sought notice is to seek responses from all qualified Small Business prospective offerors to include SDVOSB/VOSB verified vendors who hold a current state of Ohio Emergency Medical, Fire and Transportation Services (EMFTS) certificate to perform ambulance service in the state of Ohio to determine the type of set-aide that will be utilized for the forthcoming solicitation summarized as follows: Short Title: Ambulance Service at Columbus VAACC for Advanced and Basic Life Support NAICS-621920: Ambulance Service Purpose: The purpose of this sources sought notice is to determine what type of set-aside is appropriate for this acquisition Place of Performance: Department of Veterans Affairs | Chalmers P. Wylie Columbus Veterans Administration Ambulatory Care Clinic (VAACC), 420 North James Road, Columbus, Ohio 43219 Anticipated Periods of Performance: 1-Dec-2019 to 30-Nov-2024 (Base plus Four Option Years) General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as specified in the attached PWS. The contractor shall perform services under this contract in strict adherence to all current laws and regulations applicable to the state of Ohio unless supplemented to exceed those requirements as specified in the PWS. Anticipated Requirements: The estimated annual not-to-exceed (NTE) quantities for the requirement specified in the attached PWS are summarized as follows: BLS Ambulance BASE RATE | One-Way Trip WITHIN 20-miles Columbus VAACC: 3,240 = Estimated one-way trips per year BLS Ambulance MILEAGE RATE | One-Way Trip OUTSIDE 20-miles Columbus VAACC: 2,400 = Estimated miles per year BLS Ambulance WAIT TIME | 15-minute Increments IAW PWS: 650 = Estimated 15-minute wait-time increments per year ALS Ambulance BASE RATE | One-Way Trip WITHIN 20-miles Columbus VAACC: 950 = Estimated one-way trips per year ALS Ambulance MILEAGE RATE | One-Way Trip OUTSIDE 20-miles Columbus VAACC: 20 = Estimated miles per year ALS Ambulance WAIT TIME | 15-minute Increments IAW PWS: 20 = Estimated 15-minute wait-time increments per year Attachments for Reference: A draft version of the Performance Work Statement (PWS) is attached for your reference as appropriate. Request all Small Business prospective offerors interested in this requirement email their company name, DUNS number, CAGE code, small business socio-economic size status and a signed copy of their current state of Ohio EMFTS certification in Adobe PDF format to perform ambulance service for advanced and basic life support in the state of Ohio for the anticipated subject requirement to bradley.burhite@va.gov not later than 27 September 2019. Only those prospective small business prospective offerors who are certified under NAICS 621920 and submit a current signed copy of their Ohio EMFTS certification in response to this sources sought notice will be considered responsive for the purposes of this FBO posting.
