Description

NOTICE OF INTENT TO SOLE SOURCE The Department of Veteran Affairs, Veterans Health Administration, Network Contracting Office 10, Cincinnati, OH 45219, intends to negotiate a sole source, Firm-Fixed Price Contract for sustainment of 3 film array machines to include service, software, help desk support and warranty extension to Biofire Diagnostics LLC., 515 Colorow Drive, Salt Lake City, UT 84108 in accordance with FAR Subpart 13.5. This acquisition is for the Cincinnati VA Healthcare System, 3200 Vine Street, Cincinnati, OH 45220. The Government intends to award the contract on or about September 24, 2019 to Biofire Diagnostics LLC., 515 Colorow Drive, Salt Lake City, UT 84108. DUNS: 079280262. The NAICS code is 811219 and has a size standard of $20.5 Million for this procurement. This notice satisfies the requirement at FAR 5.201(b)(1), requiring publication of the proposed contract action. This notice of intent is not a request for quote and no solicitation shall be made available. IAW FAR 5.207(c)(16)(ii) Contractors may submit a capabilities statement or quotation which shall be considered by the agency due by 4PM EST 09/20/2019. The government shall not pay for any costs incurred by responses to this notice. Statement of Work Services to be Provided Extended Warranty for the BioFire FilmArray analyzers (sn: 2FA00276, sn: 2FA00312, and sn: 2FA02715) VA Medical Center, 3200 Vine St, Cincinnati, Ohio 45220. Services Standard Limited Warranty Extension: BioFire Diagnostics Standard Limited Warranty Applicable to the Instruments and Software will be extended for the Coverage Period of this Extended Warranty Instrument Repairs: BioFire Diagnostics will supply labor, parts, and replacement equipment necessary to repair breakdowns of the instruments (operational or mechanical) Software Upgrades: BioFire Diagnostics will provide new versions, releases, or upgrades of the software Optional Instrument Recalibration and Quality Inspection: No more than 1 time during each yearly coverage period, instruments may be returned for recalibration, maintenance, and quality inspection Telephone Support: 24 hour telephone support Contractor Competency Contractor shall provide written assurance that all service personnel have received training to support the BioFire FilmArray analyzers Contractor Service Report Contactor shall provide a written or electronic service report upon completion of any service activity. Minimum information included shall be date and time, service personnel name(s), problem description, corrective action, parts used, and hours on-site.
