Description
Reps and Certs SLAC RFP Statement of Work Terms and Conditions COI WSHP Dear Supplier, SLAC National Accelerator Laboratory sometimes referred to as "University," "SLAC," or "Laboratory," invites a firm fixed price proposal per the enclosed specification/requirements: Statement of Work FY 2020 Condition Assessment Survey, dated September 18, 2019. TYPE OF SUBCONTRACT The type of subcontract anticipated is Firm Fixed Price with additional Options. PERIOD OF PERFORMANCE The anticipated Period of Performance shall commence on subcontract award date through September 30, 2020. TERMS AND CONDITIONS The attached SLAC National Accelerator Laboratory Terms & Conditions for Consulting and Professional Services (rev 17 Feb 2015) will be made a part of this order. FAR 52.204-23 - Prohibition on Contracting for Hardware, Software, and Services Developed or Provided by Kaspersky Lab (Aug 2018), will also be made a part of this order. All exceptions must be submitted at the time of proposal submission. PROPOSAL Proposal submissions shall consist of two separate documents: (1) Technical Proposal and (2) Price Proposal. The Technical Proposal shall include a matrix which confirms compliance with every specification requirement in the Statement of Work. Exceptions must be noted and an alternative approach must be provided for consideration. The Price Proposal shall include firm fixed pricing for the Base Work and each Options 1, 2 and 3, including the corresponding RPV calculations per option, priced separately. See Attachment A, B, C, D and E from the Statement of Work for more information. A completed- Price Proposal must be submitted with your proposal. You are requested to identify and mark proprietary/confidential information provided in response to this solicitation. SUBMISSION OF PROPOSALS The proposal will be received by the undersigned SLAC Procurement Representative no later than 3:00 PM Pacific Time on September 24, 2019. The proposal shall be valid for a period of 90 days from the proposal due date. Proposals shall be signed by an authorized representative of the Offeror and emailed to: dviera@slac.stanford.edu Attn: Diana Viera Acceptance of late proposals will be at SLAC's sole discretion. SLAC reserves the right to reject any and all proposals, to waive any minor irregularities in any proposal, or to cancel this RFP at any time prior to award without cost to SLAC. SLAC will respond to questions submitted in writing to the SLAC Procurement Representative on or before September 23, 2019, no later than 3:00 PM Pacific Time. Questions submitted after the specified date and time may not be answered prior to the proposal due date. Questions may be submitted by email only to the SLAC Procurement Representative. Answers to questions that pertain to the interpretation of SLAC's requirements will be issued to all Offerors in writing. ADDITIONAL REQUIRED DOCUMENTATION 1) Representations and Certifications: Complete the attached three (3) page form. The following documents (items 2 and 3) are required since work will be performed on-site at SLAC: 2) Certificate of Liability Insurance: The language under sections "Description of Operations" and "Certificate Holder" must be written exactly as called out in the example form and copied below: "The Board of Trustees of the Leland Stanford, Junior, University (University), the U.S. Dept. of Energy (DOE), and their officers, employees, and agents, are additional insureds under Comprehensive General Liability Policies; such policies are primary as to any loss to which the insurance coverage provided thereby is applicable, including bodily injury, personal injury, property damage, and vehicle liability, without right of contribution from any insurance otherwise maintained by the University or DOE. Insurer waives all rights of subrogation against the University and DOE. The policy shall include a cross liability or severability of interests clause in addition to the waiver of subrogation." The Certificate Holder must be to: The Board of Trustees of the Leland Stanford, Jr. University 2575 Sand Hill Road Menlo Park, CA 94025 Do not state SLAC National Accelerator Laboratory as the certificate holder. 3) Worker Safety and Health Program Acknowledgement (WSHP) Form: Complete the attached one (1) page form. Reference PR number 303598 SOURCE SELECTION CRITERIA Source selection will be based upon the lowest priced technically acceptable proposal and the following evaluation factors: • Past Performance: Please include two (2) examples of previous facility condition assessment projects for Facilities Information Management Systems (FIMS) within a national laboratory. Provide relevant contractual and point of contact information. • Applicable Certifications/Licenses. The prospective Subcontractor shall provide resumes of individuals showing experience and education pertaining to Building Condition Assessment Inspections and the trade divisions of experience. The University shall maintain right of approval, refusal and/or replacement of any resume submitted. • Proposed project schedule. The prospective Subcontractor shall provide the proposed Project Schedule. The University shall maintain right of approval and/or change. • Report formatting. The prospective Subcontractor shall provide copies of the Assessment Report, Cost Data form sheets and method of determining RPV planned for use for University review and comment. The University shall maintain right of approval and/or change. The prospective Subcontractor shall also provide copies of assessment reports in soft copy via FTP. The University shall maintain right of approved and/or change. The University shall be the owner of the data for all assessment reports. EXERCISE OF OPTION(S) 1. SLAC may extend the terms of this contract by written notice to the Contractor within 30 days prior to the contract expiration. 2. If SLAC exercises an option, the exercised option shall be considered to include this option clause. 3. The total duration of this contract, including the exercise of any options under this clause, shall not exceed one (1) year. OPTION TO EXTEND SERVICES The University may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed six (6) months. The Procurement Specialist may exercise the option by written notice to the Subcontractor within 30 days of the contract expiration date. PAYMENT TERMS: Net 30 E-VERIFY REQUIREMENTS This solicitation falls under the reporting requirements of E-Verify. Within thirty (30) days of award of a Subcontract the awardee must provide a copy of the Memorandum of Understanding (MOU) as proof of enrollment in the E-verify Program. If proof of enrollment is not received within the specified timeframe the subcontract may be subject to cancellation. If you are unfamiliar with E-verify the following link will provide you with detailed information: www.dhs.gov/E-Verify SYSTEM FOR AWARD MANAGEMENT REQUIREMENTS For a Subcontractor to be eligible for a subcontract award with the University and the U.S. Department of Energy (DOE), Subcontractor shall be registered with the System for Award Management (SAM), the Official U.S. Government system that consolidated the capabilities of CCR/FedReg, ORCA, and EPLS. There is NO fee to register for this site. Entities may register at no cost directly at the SAM website below: https://www.sam.gov PROPOSAL PREPARATION EXPENSES This solicitation does not commit the University to a subcontract award and all PROPOSAL preparation expenses are the sole responsibility of the Offeror and are not reimbursable. RFP AMENDMENTS The University reserves the right to issue any Request for Proposal Amendment(s) to the documents. Offerors will be required to acknowledge the receipt of Amendments, if issued and submit with their proposal. WAIVERS, ACCEPTANCE OR REJECTION SLAC, notwithstanding any other provision of this Solicitation (including all attached documents), expressly reserves the right to: • Waive any minor informality or irregularity in the solicitation procedures or Proposals, if the objectives of this solicitation are indeed met • Reject any and/or all Proposals, or • Reissue a solicitation. Additionally, the Laboratory reserves the right to conduct parallel negotiations, or to award on the basis of initial Proposals, whichever is deemed most advantageous to the University. Proposals received after the specified date and time, including those delivered by individuals, postal mail, or carrier services may be rejected and returned unopened at the sole option of the University. It is the sole responsibility of the Offeror to ensure that their Proposal is submitted in a timely manner to the procurement representative via one of the methods listed above. Please note that no other department at SLAC has the authority to solicit or accept official, written quotations or Proposals for these services other than the SLAC Procurement Department. Information provided herein is subject to change and in no way binds SLAC to solicit or award a contract. If additional documents and/or a solicitation are released, it will be made available via email upon release. All comments and inquiries are to be directed to the SLAC Procurement Representative's attention. Communication with any SLAC employee or contractor (non-procurement) with regards to this solicitation is not permitted and such actions are grounds for disqualification.
