Description
Attachment 3 - Section L and Section M as revised in Amendment 0005 **Amendment 0005: The purpose of Amendment 0005 is to revise the maximum number of pages in Section L and to clarify the relative importance of factors and subfactors in Section M of the request for quote. Please see Attachment 3 - Section L and Section M as revised in Amendment 0005 for more information. The RFQ response date and time are hereby extended to Friday, 9/27/19 at 1400 EST. All other RFQ terms and conditions remain unchanged.** **Amendment 0004: The purpose of Amendment 0004 is to delete SOW Task 4.5, as it was included in error. Please see Attachment 1 - Statement of Work as revised in Amendment 0004. The RFQ response date and time are hereby extended to Tuesday, 9/24/19 at 1400 EST. All other RFQ terms and conditions remain unchanged.** **Amendment 0003: The purpose of Amendment 0003 is to extend the RFQ response date and time. The RFQ response date and time is hereby extended to Monday, 9/23/19 at 1400 EST. No further extensions will be granted. All other RFQ terms and conditions remain unchanged.** **Amendment 0002: The purpose of Amendment 0002 is to post an industry question and answer as follows: Question 4: Is this requirement subject to prevailing wages? Answer 4: No. Service Contract Labor Standards are not applicable. The RFQ response date and time are NOT extended as a result of Amendment 0002. All other RFQ terms and conditions remain unchanged.** **Amendment 0001: The purpose of Amendment 0001 is to post industry questions and answers as follows: Question 1: Please confirm if the diving operations are to be conducted per EM 385 (9/2014 latest rev). Answer 1: The required work under this request for quote (RFQ) does not fall under EM 385, as there will be no construction work required. All work to be conducted shall be performed in accordance with Applicable Document 3.1 in the Statement of Work (SOW): 29 CFR Part 1910 Subpart T - Commercial Diving Operations, OSHA Standards. Question 2: Please confirm if diving operations will be conducted Monday-Friday or if we should anticipate weekend diving operations. Answer 2: Diving operations are typically scheduled Monday-Friday. However, there may be occasions in which emergent diving services will be requested for after business hours and on weekends. Question 3: What is the typical duration the dive team is required to be on-site during: a) short duration dive support, b) long duration dive support, c) standby swimmer support. I.e. how should we classify a "day" for hours on site? 8/10/12 hours? Answer 3: Section 4.0 of the SOW defines each duration. The durations are in-water times and do not include mobilization or demobilization activities. Offerors shall provide pricing in accordance with Attachment 4 - CLIN 0001 Pricing Table. The RFQ response date and time are NOT extended as a result of Amendment 0001. All other RFQ terms and conditions remain unchanged.** This combined synopsis and solicitation is being posted to the Federal Business Opportunities (FBO) page located at http://www.fbo.gov. It is understood that FBO is the single point of entry for posting of the synopsis and solicitation to the Internet. This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. This combined synopsis and solicitation will be open for less than 30 days, in accordance with FAR 5.203(b). Request for Quotation (RFQ) number is N66604-19-Q-3605. The North American Industry Classification System (NAICS) Code for this acquisition is 334417. The Small Business Size Standard is 1,000 Employees. NUWCDIVNPT intends to purchase the following services, on a firm-fixed price basis: CLIN 0001: Commercial Diving Services, Quantity: 1, LOT. CLIN 0002: Data Associated with CLIN 0001; Quantity: 1, LOT. Not Separately Priced. NUWCDIVNPT requires commercial diving services to be performed in support of the Code 7022 Narragansett Bay Test Facility (NBTF) in accordance with Attachment #1 "Statement of Work" (SOW) and Attachment #2 "Contract Data Requirements Lists" (CDRLs). Required performance terms are F.O.B. Destination to Naval Station Newport, RI, 02841-1708 and the required period of performance is from date of purchase order award through nine (9) months thereafter. Offers shall be submitted in accordance with Attachment #3 "Section L and Section M". Offerors shall complete and provide Attachment #4 "CLIN 0001 Pricing Table" with their offers. Offerors shall include the following additional information with submissions: Offeror CAGE code, and Offeror DUNS number, and shall include a validity date of no less than 30 days from due date for quotes.
