- Terms & Conditions EFT/ Banking Form W-9 REP & CERTS Photos are a representation similar to requirements Statement of Work. All bids shall be quoted as listed in the Statement of Work. Construction of concrete obstacle course for 71 SFS Military Working Dog. All description of work details and requirements are in the attached statement of work, therefore quotes should be submitted based solely on the statement of work. PART 1 GENERAL Points of Contact: Facility Manager: TSGT Ian Porter, 580-213-7163 Base Construction Management: Jason Weber, 580-213-5197 1.02 SCOPE OF WORK, See attached Statement of Work 1.03 SCHEDULE OF WORK See attached Statement of Work A. The contractor will build forms and pour concrete pad as necessary to set the following 8 obstacles. All surfaces of the obstacles that the dogs may traverse will be covered with a non-skid material. The footing and pad is described with each individual obstacle listed below Item 3 is listed as three (3) separate obstacles. 1. One concrete tunnel 4' x 12' x 32". With a concrete footing that is 1' wide and 1'6" deep with and 8" pad for it to sit on. 2. One stair obstacle that is 24' x 3'7" x 8' with a footing that is 1' wide and 1'4" deep. It will sit on a pad that is 8" thick. 3. Three(3) concrete hurdles, two hurdles being 4' x 30"x 30" and one hurdle being 4' x 18" x 18".They will sit on a concrete pad that has a footing 1' wide and 1'4" deep. The pad is 4" thick. 4. One concrete window that is 7'6" x 6' x 8'. Window will set on a pad with a footing that is 1' wide and 1'4" deep and the pad will be 8" thick. 5. One A-Frame that is 4"x 11" x 6'. A- Frame will set on a pad with a footing that is 1' wide and 1'4" deep - pad will be 6" thick. 6. One dog walk 20' X 1' x 2'2", dog walk will set on a concrete footing and pad that is 3' x 20' x1'4" deep. 1.04 CONTRACTOR'S REPRESENTATIVE, See attached Statement of Work 1.05 PROVIDED SERVICES AND UTILITIES, See attached Statement of Work 1.06 INSPECTION, See attached Statement of Work 1.07 EXCESS MATERIALS (EXCLUDING HAZARDOUS MATERIALS OR WASTE) See attached Statement of Work 1.08 SPECIAL MATERIALS, See attached Statement of Work 1.09 MATERIALS, See attached Statement of Work PART 2 EXECUTION 2.01 INSTALLATION See attached Statement of Work Upon awarding the following will need to be completed : 1. Certificate of Liability will need to be provided. 2. Base Access forms with background check will need to be completed before entering base. 3. The attached forms will need to be completed and returned before award is made. Only quotes that include all Labor and Materials according to the Statement of Work will be considered.
