Description

Intent to Sole-Source Notice The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO 10), Kettering, Ohio 45409 intends to solicit a RFQ for a sole-source procurement to provide service and maintenance for the Picture Archiving and Communication System (PACS) (See Below), using a Firm-Fixed Price Sole-Source contract as the procurement method. The VA anticipates issuing a Firm-Fixed Price Contract (Sole Source) to Vital Images Inc. on or about 10/01/2019 for the ongoing maintenance and repairs needed. James Road Columbus, Ohio 43219 614-257-5200 Station Code: 36C757 In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in System for Award Management (SAM) and in VetBiz database, if applicable (VetBiz registration is required for Service Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses), under the applicable NAICS. Contractors may obtain SAM information at https://www.sam.gov and Vet Biz information at http://www.vip.vetbiz.gov. To be eligible for award, contractors must be registered in the System for Award Management database. Department of Veterans Affairs Network Contracting Office (NCO) 10 3140 Governor s Place Blvd., Suite 210 Kettering, Ohio 45409 Station Code: 36C250 Vital Images Vitrea Enterprise Suite Annual Support and Maintenance Service Contract Statement of Work 1.0 GENERAL Contractor shall provide personnel to complete the task of providing Premier support and maintenance service to the Vital Images Vitrea Enterprise Suite, located at the Department of Veterans Affairs Chalmers P. Wylie Ambulatory Care Center (VA ACC), Columbus, Ohio. 1.1 CONTRACTING OFFICER S TECHNICAL REPRESENTATIVE (COTR) The Contracting Officer s Technical Representative (COTR) will be the principal liaison for the Contractor during the duration of the contract and must be contacted prior any services performed on the equipment. 2.0 SPECIFIC TASKS Provide technical support and answer any questions regarding the Vitrea Enterprise Suite at the VA ACC during the period of 10/01/19 09/30/20. Provide maintenance service that includes access to new versions, updates, and patches to the Vitrea Enterprise Suite Components. Bug fixes, and enhancements, including any feature requests made by users which are approved by Vital Images are provided in these updates during the period of 10/01/19 09/30/20. SUPPORTING INFORMATION Service Employee Qualifications: Service employees shall be competent to provide technical support through phone calls/emails and perform the specific repairs. Hours of Work: Contract employees will work during hours 8:00am 4:30pm, M-F excluding Federal Holidays. Location of Work: All on site work will be performed at the VA ACC, 420 N James Road, Columbus, Ohio 43219. Period of Performance: Base Year: 10/01/2019 09/30/2020 Option Year 1: 10/01/2020 09/30/2021 Option Year 2: 10/01/2021 09/30/2022 Option Year 3: 10/01/2022 09/30/2023 Option Year 4: 10/01/2023 09/30/2024
