Notice Type: Posted Date: Sole Source Notification September xx, 2019 Original Set Aside: Response Date: Intent to Sole Source September 27, 2019 @ 11:00 a.m. EST Classification Code: NAICS Code: J065 Equipment Maintenance and 811219 Other Electronic and Precision Repair Services Equipment Repair and Maintenance Intent to Sole Source: The Department of Veterans Affairs, Network Contracting Office 8; hereby provides a notice of its intent to award a firm fixed contract to provide EXTENDED WARRANTY FOR PREVENTIVE MAINTENANCE, REPAIR, AND ANNUAL CALIBRATION SERVICES FOR THE ECHOSENS FIBROSCAN 502 TOUCH SYSTEM, TRANSDUCER FIBROSCAN SYSTEM AND ALL HAND-HELD PROBES located at the Bruce W. Cater VA Medical Center, 1201 NW 16TH Street, Miami, Florida 33125 and the William Bill Kling VA Outpatient Clinic, 9800 West Commercial Blvd, Sunrise, Florida 33351 to the following Contractor. Fidelis Sustainability Distribution LLC 204 West Spear Street Carson City, NV 89703 SAC Contract #VA119-17-D-0033 The Contractor shall perform all repair and services in accordance with the Manufacturer specifications, all applicable Federal, State, Local, and industry: standards, laws, policies, procedures, codes and any other applicable governing body for the proper performance of all equipment. Calibration: Equipment Calibration are due once a year (to be coordinated after award), based on the equipment schedule. Repairs: The Technician shall report to the Biomed Services within 24-hours upon receipt of an emergent repair request, if required provide Loaners as soon as requested. Equipment Loaners shall be delivered during business hours of 7:30 a.m. 4:00 p.m., Monday through Friday, unless otherwise specified. Services: Calibration and repairs shall be performed on site or off-site with Loaner in place. The Contractor shall provide all Preventive Maintenance service, repair parts, and materials as required to maintain the system in a fully operational capacity. The Contractor shall ensure that only fully qualified OEM Certified Technicians and OEM (new) parts for the equipment are utilized to make all necessary and scheduled work to equipment listed below. ECHOSENS EE94689/SN: F62545 FIBROSCAN 502 TOUCH System Transducer EE94690 / SN: S75691 Transducer EE94691 / SN: S92590 The Contractor shall respond to all emergency repair calls and be at the Government facility within two (2) hours during normal working hours, Monday through Friday. For emergency repair service the Contractor shall respond within twenty-four (24) hours after receiving notice. If the malfunction is reported to the Contractor no later than 4:00 p.m. Monday through Friday, the failure elimination shall be started immediately on the same day. In urgent cases, an immediate repair may be requested after 4:00 p.m. Monday through Friday and/or on Saturdays, Sundays and Federal holidays which shall require the Contractor to respond the same day. A copy of all the Contractor s Technicians Certification shall be furnished to the Biomedical Services, as part of the Equipment s Service Records. The Contractor shall ensure test equipment used for PM, Repair, or Emergency Repair purposes be annually calibrated to NIST standards and provide the Government a copy of the Equipment Certification. The Contractor shall submit the following reports (electronic and written) within forty-eight (48) hours after Repair and/or Preventive Maintenance service performance is accomplished. Functional Test Records (if applicable) Service Reports ANY COMPANY WHO MAY BE ABLE TO PROVIDE NECESSARY ORIGINAL EQUIPMENT MANUFACTURER (OEM) MAINTENANCE AND REPAIR TYPE SERVICE FOR THESE SYSTEMS MUST PROVIDE PROOF OF AUTHORITY ALLOWING WORK TO BE PERFORMED ON THIS EQUIPMENT, CAPABILITY STATEMENT TO DEMONSTRATE SERVICE ABILITY, AND ANY TYPE GOVERNMENT CONTRACT INFORMATION IF APPLICABLE TO THIS TYPE OF SYSTEM AND REPAIR. ALL EQUIPMENT MUST MAINTAIN THEIR WARRANTY STATUS AND BE FULLY OPERATIONAL AT ALL TIMES FOR PATIENT UTILIZATION REQUIREMENTS. NO GRAY MARKET OR REFURBISHED REPLACEMENT PARTS WILL BE ACCEPTABLE, MUST HAVE ORIGINAL EQUIPMENT MANUFACTURER (OEM) PARTS. Any prospective candidates must be registered in the System for Award Management (SAM) in order to be eligible for award. All qualified SDVOSB/VOSB candidates must be verified in Vendor Information Pages (VIP) in order to be eligible for award. If you have not conducted business with the Bruce W. Carter VA Medical Center located in Miami Florida, you will be required to complete Vendor/Payment Forms to be entered into the local systems in order to be eligible for award. All interested parties shall submit clear and concise documentation demonstrating their capabilities to satisfy the requirements listed above to Contracting Specialist, Cynthia Machuca at cynthia.machuca@va.gov no later than September 27, 2019 @ 11:00 a.m. EST (Eastern Standard Time).
