Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify capable firms to provide American Bedding Manufacturers Inc., Blue Nylon Mattresses for the Coatesville, PA VA Medical Center. The units must be rated for commercial use and within hospital specification standards. This Sources Sought is for a Brand Name or Equal requirement. TENTATIVE STATEMENT OF WORK 1.0 GENERAL SCOPE The Veterans Health Administration, Coatesville VA Medical Center, Coatesville, PA has a Brand Name or equal requirement. The following American Bedding Manufacturers, Inc. blue nylon dorm innerspring mattress is the benchmark and basis of design for floors 1 and 2 of Buildings 8 and 39. Furniture that meets the VHA specifications and high-quality threshold while integrating excellent environmental stewardship is required. This requirement includes healthcare furniture, professional assembly, professional installation, and maintenance/warranty services and removal of all existing mattress outside the VA premises. The technical points of contact are Alexis Siriani (technical point of contact) and Regina Cahill (technical point of contact). To standardize furniture and provide a safe environment for patients, staff, and visitors alike, healthcare -grade furniture with the salient characteristics listed below is required. These products meet all needs of the patients and staff while achieving the overall desired look within the facility. The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. The following items, quantities and salient characteristics are required in Brand name or equal: 2.0 PERFORMANCE REQUIREMENTS The contractor shall provide and be responsible for the technical assistance, development and generation for final specifications. Products shall be delivered, assembled and installed by the contractor at the following location: Veterans Affairs Medical Center Building 8, Floors A and B Building 39, Floors A and B 1400 Blackhorse Hill Road Coatesville, PA 19320-2096 Delivery Dates: Building 8 A and B, delivery and install are required on 11/12/2019. Building 39 A delivery and install is required on 11/18/19. Building 39 B delivery and install is required on 11/20/19. Delivery and installation shall be coordinated with the technical point of contact, Alexis Siriani. Delivery hours are Monday thru Friday, 0800 AM -1500 PM EST, excluding Federal holidays. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: Coordinate and provide project management of all products and services to manage, design, order, ship, deliver and install new furniture from the manufacturer until final acceptance by Alexis Siriani. Contractor shall place all orders, track orders, schedule deliveries and coordinate trucks with the manufacturer for manufacturing and delivery according to VA project schedule. Contractor shall provide all necessary materials, equipment, labor, supervision, and Management to: (A) coordinate delivery: (B) load and unload; (C) place/install items in designated rooms/locations; (D) assemble items as required; (E) secure as designated; and (F) properly dispose of all associated packing/crating materials outside the VA premises. Provide maintenance manuals for all products specified. 3.0 EVALUATION CRITERIA Evaluation Factors: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Brand Name or Equal: Evaluation factor for this acquisition will be based on Lowest-Price, Technically Acceptable (LPTA). Technical acceptance is the provision of BRAND-NAME or Equal (substitutions are only acceptable if they meet all salient characteristics). Price This is a Lowest Priced, Technically Acceptable evaluation. Offers will be initially evaluated on a pass / fail basis for adherence to the minimum required specifications. Of those offers that received a pass, the lowest offered price will be selected for award. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing. 5.0 SERVICES Contract Tasks/Requirements The product must not be placed into production without clearance from the VA technical point of contact Alexis Siriani or Regina Cahill. There is a potential for work stoppages that could affect orders from the furniture manufacturer. The contractor shall track manufacturing schedule and notify VA Interior Designer with updates of estimated completion date by email. Installation shall occur during normal working hours and installation crew must be badged prior to entering facility. All crew must bring 2 forms of ID one with a picture and that they should apply for the badges one month before the start of work to ensure time for background checks by CVAMC campus police. Besides the workers/laborers on the job, a dedicated contractor must be utilized to monitor/supervise the work and tools brought onto the hospital ward. Contractor shall ensure that all items brought onto the ward are monitored by them and their work crew 100% of the time. Items can never be left unattended. Behavioral health staff will not be able to monitor any contractors working on the ward. A full count of inventory must be done prior to entering and leaving the site. Every tool, item (dolly, rags, cleaning products) screws etc. must be accounted for. Only bring what you need onto the ward, patients will be on the ward. The main elevator shall be used during the install and protective floor covering must be supplied by contractor for use during the installation. All trash to be removed in covered containers to eliminate dust and debris while walking thru the hospital. Contractor may have to stop work to accommodate patients being moved, this may impact the logistics and timing during the delivery and install. In the event that construction is not completed as scheduled on floors A and B in Buildings 8 and 39, delivery may not be approved for November 12, November 18, November 20, 2019 respectively as indicated. As such, the contractor shall be required to provide free storage of up to roughly 90 days until construction is complete and delivery and installation can be rescheduled. Any storage required beyond 90 days will be arranged with the contractor, technical point of contact, Alexis Siriani. Contractor shall be responsible for the removal and disposal of all existing furniture / mattresses outside of the VA premises. Contractor may have to stop work to accommodate patients being moved, this may impact the logistics and timing during the delivery and install. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission. Work Required by Others All work required by other trades shall be identified under this section by the offeror.
