Description
STATEMENT OF WORK Corrective Actions for OR 2F Mechanical Systems @ JLM John L. McClellan Memorial Veterans Hospital EXAMPLE PROJECT OVERVIEW The Central Arkansas Veterans Healthcare System (CAVHS) intend to select a contractor to provide professional construction services for the Corrective Actions for OR 2F Mechanical Systems at their VAMC facilities in Little Rock, Arkansas. This project is for renovation of the general corrective actions to the HVAC system for the OR 2F surgical suites, the Medical Gas system in the PCAU (2B-181) and new design drawings and specifications for new CO2 system and WAGD system in the OR 2F surgical suites. Work consists of the contractor providing all labor, equipment, transportation, materials, quality control and supervision necessary to conduct construction in accordance with the project specifications and drawings. The magnitude of construction per VAAR Part 836.204 is more than $250,000.00. This is a 100% SDVOSB set-aside. North American Industrial Classification Standard (NAICS) code assigned to this construction project is 236220. PROJECT ACTIVITIES The entire activity of the project will follow the following sequence of events: Pre-proposal Site Visit Request for Proposal Submission Selection Award Post Award Kick-off Meeting Notice to Proceed Bonding & Insurance Submittals Daily Reports Weekly Progress Reports and Weekly Project Schedule Permits (asbestos abetment) Demolition, Construction, Installation and Commissioning Final Inspection Acceptance Operations and Maintenance Manuals AutoCAD Drawings (Design, Construction, As Built) Payment Warranty SCOPE OF WORK The Contractor shall provide all necessary services to meet the project objectives and requirements and to provide the required outcome in terms of overall quality, timeliness and documentation. The Contractor shall be responsible for obtaining and coordinating the services of any and all third-party Contractors performing other services on site and other entities as part of the contract to ensure the expected quality, timely, and documented final product. Location: John L. McClellan Memorial Veterans Hospital, 4300 West 7th Street, Little Rock, Arkansas 72205. Normal work hours for VA sites are 7:00AM to 4:30PM Monday through Friday except for Federal Holidays. If the Contractor desires to work on Saturday, Sunday, holidays, or outside the project site s normal working hours the contractor must submit a written request for approval to the Contracting Officer Representative (COR) at least seven (7) working days prior to the proposed start. DESCRIPTION OF WORK The plan is to renovate approximately 2,621-GSF of interior space for Mechanical Space on the Ground Floor and 9,453-GSF of interior space for OR 2F Surgical Suites on the 2nd Floor. DELIVERIES The Performance Period for this project is three hundred sixty-five (365) calendar days after receiving the Notice to Proceed. The Notice to Proceed will be issued within 30 days after award of the solicitation. Performance and Payment bonds: Due on Post Award Kick-off meeting. Proof of Insurance: Due on Post Award Kick-off meeting. All personnel assigned on the project and works on the construction site must complete a one-hour CAVHS Safety training. ACCEPTANCE For the purposes of Warranty and payment for the work performed at a location, government Acceptance will be provided by the VA upon the completion of all of the following: The Contractor has demonstrated/verified that the implemented ECMs function as intended at commissioning. All punch list items identified during the commissioning have been resolved. The Contractor has provided O&M manuals. The Contractor has provided Record drawings to include a CD with all as builds CAD files and PDF, and one full set on bond paper. CONFIDENTIALITY AND NONDISCLOSURE The preliminary and final deliverables and all associated working papers, application source code, and other material deemed relevant by the VA which has been generated by the contractor in the performance of this Order are the exclusive property of the U.S. Government and shall be submitted to the VA-COR at the conclusion of the contract. The Contracting Officer will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this contract. No information shall be released by the contractor. Any request for information relating to this contract presented to the contractor shall be submitted to the CO for response. SECURITY. All employees of general contractor and subcontractors shall comply with the VA security management program, be identified by through project and employer badges, and restricted from areas of unauthorized access. Each Contractor shall be vetted by VA Security and each worker will have their ID issued by VA. Issued ID must be worn at all times. General Contractor s employees shall not enter the project site without appropriate badging/identification. They may also be subject to inspection of their personal effects when entering or leaving the project site. No photography of VA premises is allowed without written permission of the Contracting Officer. The Contractor and workforce personnel shall not post or discuss government facility activities on unauthorized public media. General Contractor s employees are subject to rules of CAVHS Medical Center applicable to their conduct. However, the contractor personnel shall not be considered government employees for any purpose under this contract. Execute work to interfere as little as possible with normal functioning of CAVHS Medical Center, including operations of utility services, fire protection systems and any existing equipment, and with work being done by others. Use of equipment and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied, during construction, jointly by patients or medical personnel, and Contractor's personnel, except as permitted by COR where required.
