- This is a SOURCES SOUGHT NOTICE for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described in the Statement of Work Statement (SOW). Potential offerors are invited to provide information via e-mail to susan.delacruz@va.gov. All responses will be used to determine the appropriate acquisition strategy for a potential future acquisition according to SOW. Potential contractors shall provide, at a minimum, the following information to susan.delacruz@va.gov: 1) Name, address, point of contact name, phone number, and e-mail address. 2) Brief capability statement with enough information to determine if the company can meet the requirement. The Capabilities Statement for sources sought is not expected to be a Request for Quote nor does restrict the Government to an ultimate acquisition approach, but rather short statement regarding the company's ability to provide the service below. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business Community or to conduct as an Unrestricted Procurement. The Government must ensure there is adequate competition among the potential pool of available contractors using FBO website. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government nor will the Government pays for information solicited. This synopsis is not a solicitation announcement for Request for Quote. No formal solicitation document exists at this time, and no contract will be awarded from this announcement. Respondents will not be notified of the results of the evaluation. All interested contractors should notify this office via email by 12:00 PM (MT) on October 11, 2019. Statement of Work Moving Services- Grand Junction Colorado Description of Services This statement of work (SOP) defines the actions for the contractor to provide Moving Services. A moving contract is required, to facilitate moves throughout the facility and its entities. Scope of Work The contractor shall provide all personnel, equipment, tools materials, vehicles, supervision, certifications, and other services to provide moving services. The contractor shall perform to the standards in accordance with this (SOW). Requirements: Manpower and equipment will be provided on an as-needed basis, no less than 1 day per month. The moving contractor can anticipate approximately 4-8 hours of service work per month during the VA business hours of 8 am and 4:30 pm. Contractor shall move and re-install from the original condition all items (desks, file cabinets, etc.) as outlined by the Contractor Officer Representative (COR.) The COR shall provide the contractor with a list and location of the items to be moved seventy-two hours (72) to ninety-six (96) hours in advance of every move. The list will include the location of identified items, the designated area in which items will be moved, and the placement of said items in the designated area. Contractors will provide a plan of action to corroborate with the COR instructions twenty-four (24) hours prior to scheduled moves. Contractor will determine the number of personnel and the type of equipment needed to handle all moving requirements, while ensuring proper safety precautions are adhered to. The Grand Junction VAMC will notify the contractor at least seventy-two (72) to ninety-six (96) hours prior to the need of the moving detail requested. If a move detail has already been requested and scheduled, the Grand Junction VAMC reserves the right to cancel the move request and will provide a forty-eight (48) hour notification of cancellation. The Grand Junction VAMC estimates (400) hours per year of scheduled work during duty hours of Monday Friday 8:00 A.M to 4:30 P.M. Pay rate will be hourly and the schedule of these man hours will be determined by Grand Junction VAMC using the notification guidelines set forth above. Currently there are two scheduled move dates per month with additional moves added as needed. Specific Tasks Contractor is required to furnish the following items: 3.1 All tools needed to properly move a variety of items. These tools include, but are not limited to: pallet jacks, desks jacks, dolly, moving skates/traps, and a moving truck(s). 3.2 All tools to perform minor tear down and reconstruction of identified items to better facilitate the identified items not be moved. 3.3 All personnel protective equipment (PPE), i.e.; safety boots, gloves, etc. for movers are to be worn when moving identified items. 3.4 All Personnel are expected to have a knowledge of working on moving tools being utilized and knowledge of proper moving safety techniques. 3.5 The Grand Junction VAMC will relocate and stage large items via forklift that cannot be moved by moving personnel. Any items that need to be transported by vehicle will be loaded and unloaded by contractor. Contractor will provide the truck(s) for moving. The Grand Junction VAMC will provide a forklift and an operator for the forklift when applicable. 4. Performance Monitoring The COR or a POC (designee) will be present for each facility move. The COR/POC will ONLY observe the moves in the facility and at Grand Junction entities. Performance is measured by direct observation of movers. Any negative performance will be reported to the moving organization management to be addressed and to allow for correction at the contractor s discretion. 5. Security Requirements Contractors may be required at times to enter sensitive areas in the process of moving furniture and other items. At no time will contractors have access to patient records, data, or VA computer systems. All patient sensitive information is to be secured by staff prior to movers entering any area. Contractors will check in with the VA police to get visitor badges for each move day. 6. Government- furnished Equipment (GFE)/ Government furnished information (GFI). There will be no government furnished Equipment (GFE) or government- furnished Information used by contractors. 7. Inspection and Acceptance Criteria The COR is responsible for certifying all work done under the contract is performed on time and in accordance with standards. The COR is also responsible to assure inspection and acceptance of the products which are provided incidental to services. 8. Property Damage The contractor shall take all necessary precautions to prevent damage to any government property and will notify the Contracting Officer immediately if damage occurs. The contractor shall be licensed and bonded in the state that they are operating in. The contracting Officer will authorize the contractor to remedy the situation in one of the following ways; be assessed current replacement costs for damaged property, replace damaged property in a timely manner, or correct the damages with like materials at no additional cost.
